Numerical and experimental analysis of factors leading to suture dehiscence after Billroth II gastric resection.

The main goal of this study was to numerically quantify risk of duodenal stump blowout after Billroth II (BII) gastric resection. Our hypothesis was that the geometry of the reconstructed tract after BII resection is one of the key factors that can lead to duodenal dehiscence. We used computational fluid dynamics (CFD) with finite element (FE) simulations of various models of BII reconstructed gastrointestinal (GI) tract, as well as non-perfused, ex vivo, porcine experimental models. As main geometrical parameters for FE postoperative models we have used duodenal stump length and inclination b

Gastric re-resection in emergency.

BACKGROUND/AIMS: We reviewed a consecutive series of patients with acute complications following gastric resection, in order to evaluate the role of gastric re-resection as the operation of choice in emergency. METHODOLOGY: Records of 90 patients with acute complications following gastric resection observed from January 1991 to January 1996 were retrospectively analyzed. RESULTS: Hemorrhagic events occurred in the vast majority of cases (87), either as acute complications after a long time since surgery (78 cases) or as early postoperative complications (9). Among late acute complications, t

[New technologies for the treatment of complicated duodenal ulcers].

Billroth I gastric resection with end-to-end gastroduodenal anasthamosis was carried out to 73 patients with complicated duodenal ulcers using new technologies: duodenal mobilization only to the level of ulcer defect, the use of one-row (uninterrupted or nodal) everted suture with adaptation zone of submucous base of gastric stump and muscle coat of duodenal stump. Good early and long-term results were achieved.

Safety and Feasibility of Linear Stapling Device with Bioabsorbable Polyglycolic Acid Sheet for Duodenal Closure in Gastric Cancer Surgery: A Multi-institutional Phase II Study.

BACKGROUND: Duodenal stump fistula (DSF) after gastrectomy is of low frequency but a critical complication in gastric cancer surgery. Manual oversewing for reinforcement of the duodenal stump is not applicable when free longitudinal margin is short and has technical difficulties in laparoscopic surgery. This trial evaluated the safety and feasibility of using a linear stapler with bioabsorbable polyglycolic acid (PGA) sheet for duodenal stump closure and reinforcement in gastric cancer surgery. METHODS: This multi-institutional, prospective phase II trial included gastric cancer patients who

Glycogen content in the gastric mucosa of partially resected stomach; a possible relationship with the development of cancer.

Since gastric carcinoma, unlike uninvaded mucosa, has a relatively high glycogen content and the risk of gastric carcinoma is especially high in patients with a partially resected stomach, the aim of this study was to follow glycogen storage in both uninvaded gastric stump mucosa and carcinoma developing in the stump postsurgery performed for benign and malignant conditions. One hundred patients were recruited, including controls with non-operated stomachs (duodenal ulcer or gastric cancer patients). In endoscopically taken biopsies glycogen was determined by the anthrone method. It was found

[Surgical methods in recurrent peptic ulcer after vagotomy].

In treatment of recurrent and peptic postvagotomy ulcer, the performance of gastric resection is indicated. Of 53 patients with a recurrent ulcer, 20 had a "difficult" low penetrating ulcer. With the aim ot prevent duodenal suture failure, gastric resection in these patients was performed using the developed by the authors method which permitted to ensure safety of the single-layer suture of a duodenal stump with preservation of food passage through the duodenum. After truncal vagotomy with Jaboulay drainage operation, a defect in the duodenal wall was eliminated by means of creation of anasto

Duodenogastric reflux and foregut carcinogenesis.

Epidemiologic cohort studies have established that after distal gastric resection, there is a higher risk of gastric carcinoma. It is likely that a main factor of this higher risk is the excessive duodenogastric reflux induced by surgery, because the incidence of stump carcinomas is higher in Billroth II than in Billroth I, and most of the stump carcinomas are located near the stoma. In addition, several groups of investigators have suggested that duodenogastric reflux per se induces stump carcinomas in rats. There is another human duodenogastric reflux, the primary duodenogastric reflux, thro

Gastric tube oesophagoplasty for oesophageal atresia: a follow-up study. Part II: Radiologic, endoscopic and histologic controls.

In 1982 a programme of multi-disciplinary follow-up on 12 patients that underwent a gastric tube oesophagoplasty for long-gap oesophageal atresia was started. Radiological examination, performed on all gastric tubes but one (died a few days after surgery), assessed the morphology and dynamics of gastric tube, either distally or proximally based, and stomach 2 to 8 years after surgery. Endoscopic backward exploration was performed on 7 cases, to allow a closer analysis of stomach, gastric tube mucosa and upper oesophageal stump, demonstrating that the gastric transplant is viable and almost und

[The histology of gastric mucosa in B II-stomach (author's transl)].

131 patients operated on for gastric ulcer according to Gillroth II were investigated with gastroscopy and biopsy. The histology of the gastric mucosa was correlated with the time elapsed since resection. In most cases gastritis shows no difference between anastomosis and stump. In up to 12% gastritis in the stump was more pronounced that at the anastomosis. In the stump any form of gastritis can be seen even more than 20 years afer resection. Atrophic changes are more often to be found in the resected stomach and develop more rapidly than in the normal stomach. They probably result from the c

Role of New Anatomy, Biliopancreatic Reflux, and Helicobacter Pylori Status in Postgastrectomy Stump Cancer.

Distal gastrectomy for benign gastroduodenal peptic disease has become rare, but it still represents a widely adopted procedure for advanced and, in some countries, even for early distal gastric cancer. Survival rates following surgery for gastric malignancy are constantly improving, hence the residual mucosa of the gastric stump is exposed for a prolonged period to biliopancreatic reflux and, possibly, to Helicobacter pylori (HP) infection. Biliopancreatic reflux and HP infection are considered responsible for gastritis and metachronous carcinoma in the gastric stump after oncologic surgery.

[The surgical treatment of stomach ulcers difficult to remove].

Experience in surgical treatment of 154 patients with large gastric ulcers and ulcers of the cardiac part of the stomach is discussed. All the patients underwent resection of the stomach in various modifications with a 2.6% mortality rate. Penetration was encountered in 62.4% of patients with large ulcers and in 69.2% of those with ulcers of the cardia. In 13.6% of patients with "difficult" gastric ulcers ligation of the main trunk of the left gastric artery becomes necessary, which may be the cause of incompetence of the gastrointestinal anastomosis in performance of tubular resection. Novel thoracoscopic intrathoracic esophagogastric anastomosis technique for patients with esophageal cancer.

BACKGROUND: This article describes a novel, easy, and secure thoracoscopic intrathoracic esophagogastric anastomosis procedure that uses a circular stapler with transoral placement of the anvil for patients with esophageal cancer who underwent thoracoscopic esophagectomy. METHODS: After the thoracoscopic esophagectomy, the esophagus was transected obliquely at the level of the upper posterior mediastinum using a linear stapler. The Orvil™ (Autosuture, Norwalk, CT) anvil was placed at the edge of the staple line of the esophageal stump, which was relatively at an acute angle to the stump. Next

Chromoendoscopic evaluation of gastric mucosa after partial gastrectomy by use of modified endoscopic Congo red test.

The need for partial gastrectomy has decreased as a result of reduced incidence and improved endoscopic and medical treatment of peptic ulcer disease. Nonetheless, several patients with resected stomach remain in the population, and it is well known that important pathological changes can occur in the gastric remnant. We evaluated the morphological and functional status of the gastric stump by use of modified endoscopic Congo red test (MCRT). For this purpose, 87 partially gastrectomized (Billroth I and II) patients referred for elective gastroscopy were consecutively enrolled. We found a high

[Duodenal stump closure--still a live problem. A case report].

Included is a case study of duodenal peptic ulcer bleeding. The patient was treated by Finsterer-Bancroft modification of II-type gastric resection. An early reoperation was indicated by reason of duodenal stump leak. The situation solved by means of duodenostomy. The authors discuss causes of duodenal stump insufficiency. Possibilities of prevention of this complication - which are cited in literature - are mentioned. Duodenostomy is a legitimate optimal solution of duodenal stump suture insufficiency.

Precancerous conditions and epithelial dysplasia in the stomach.

A distinction can be made between a precancerous condition and a precancerous lesion. The former is a clinical state associated with a significantly increased risk of cancer, whereas a precancerous lesion is a histopathological abnormality in which cancer is more likely to occur than in its apparently normal counterpart. Up to the present time atrophic gastritis, gastric ulcer, pernicious anaemia, gastric stumps, gastric polyps, and Ménétrier's disease have all been considered as precancerous conditions and lesions of the stomach. Of these, only atrophic gastritis, pernicious anaemia, gastric

Sigmoid and gastric tonometry during infrarenal aortic aneurysm repair.

BACKGROUND: The value of the sigmoid tonometer in predicting sigmoid ischaemia and postoperative enteric organism infection has been reported but the value of tonometric measurement has been challenged. The purpose of this study was to examine the use of tonometric measurements in a series of patients undergoing infrarenal aortic aneurysm repair. METHODS: We assessed the results obtained when sigmoid (n=11) and gastric (n=8) tonometry were performed in patients undergoing infrarenal aortic aneurysm repair (n=11). We measured blood flow ultrasonically (n=6) in the inferior mesenteric artery(IM

A possible variant of Bouveret's syndrome presenting as a duodenal stump obstruction by a gallstone after Roux-en-Y gastrectomy: a case report.

INTRODUCTION: Bouveret's syndrome is characterized by gastric outlet obstruction due to a gallstone in the duodenum, usually in association with a cholecystoduodenal fistula. CASE PRESENTATION: We report the case of a 69-year-old Caucasian man who developed duodenal stump obstruction due to an impacted gallstone after having previously undergone Roux-en-Y gastrectomy. CONCLUSIONS: Duodenal stump obstruction after Roux-en-Y gastrectomy is rare, and may be difficult to manage. Patients who present with upper gastrointestinal or pancreatobiliary pathology after previous gastric surgery should b

[Patient Helicobacter pylori infectivity after gastric resection].

The article is devoted to investigation of the Helicobacter pylori (HP) infection in 14 patients after resection of the stomach for ulcers of the gastroduodenal zone. Different methods were used for the assessment of the state of the gastric stump and gastroenteric anastomosis in the postoperative period. In most cases different degree of inflammatory alterations of the gastric mucosa were detected. In 2 cases (14.3%) peptic ulcers of the anastomosis developed. Changes to the gastric stump of 11 patients were associated with the HP infection which involved the mucosa of the anastomosed intesti
