A minimally invasive palliative approach to advanced pancreatic and papillary cancer causing both biliary and duodenal obstruction.

BACKGROUND: In patients with both duodenal and biliary obstruction in whom endoscopic drainage is not feasible, the standard approach has been gastroenterostomy plus biliodigestive anastomosis. We present our results of percutaneous biliary drainage in combination with gastroenterostomy. PATIENTS AND METHODS: Twenty-one patients, who received permanent percutaneous transhepatic biliary drainage (PTBD) and gastroenterostomy in case of symptomatic gastric outlet obstruction were retrospectively evaluated. RESULTS: PTBD insertion succeeded in all patients; minor complications were encountered i

[The nonresectable pancreatic tumor, an interdisciplinary problem. The surgeon's viewpoint].

In 204 patients with cancer of the pancreas (1973-1984), palliative treatment in 157 non-resectable tumors is analyzed retrospectively. 124 patients underwent biliodigestive anastomosis, 11 with simultaneous gastroenterostomy. In 13 patients a secondary gastroenterostomy was necessary due to duodenal obstruction. The mortality rate in this group was 38.5% compared with 9% in patients who underwent simultaneous gastroenterostomy. Where there is a calculated risk, the aim should be a single, definitive intervention, especially where the patient can no longer be provided with an endoscopic discha

Choice of gastric drainage procedures.

The need for drainage after truncal or selective vagotomy is well accepted, but there is no agreement on the most suitable type of drainage procedure. A review of the literature, particularly of papers published in the last 3 years, suggests that whereas vagotomy with gastroenterostomy is followed by a relatively high incidence of bile vomiting, vagotomy with pyloroplasty carries a significantly higher risk of recurrent ulceration. The surgeon, therefore, should not hesitate to perform gastroenterostomy if the pyloroduodenum is severely scarred. As pyloroplasty is irreversible, gastroenterosto

Maximizing oral intake tolerance in malignant gastric outlet obstruction - a Markov decision tree analysis comparing duodenal stenting, endoscopic ultrasound-guided gastroenterostomy and surgical gastrojejunostomy based on a meta-analysis of randomized controlled trials.

INTRODUCTION: Malignant gastric outlet obstruction (GOO) has a significant impact on quality of life. Endoscopic ultrasound-guided gastroenterostomy (EUS-GE) has shown promising results. Traditional isolated outcome measures do not sufficiently address critical considerations for end-of-life patients like oral intake tolerance. This study aimed to determine via a probabilistic approach, the optimal management strategy for GOO patients that maximizes their oral intake tolerance. METHODS: A Markov decision model was developed, with input variables based on a systematic review and meta-analysis

Acid secretion after gastric operations.

In the early 20th century, the commonest surgical treatment of peptic ulcer was gastroenterostomy. Crohn and Wilensky demonstrated that this operation did not achieve its aim of markedly reducing gastric acidity or of accelerating motility. These results were highly controversial, but led to Lewisohn visiting Haberer in Austria in 1922, and convincing Dr. A.A. Berg to abandon gastroenterostomy and use partial gastrectomy as the standard ulcer operation, with additional vagotomy in those patients with duodenal ulcer with high acidity. In 1929, a few patients were treated by vagotomy and gastroj

The influence of staple height on postoperative complication rates after laparoscopic gastric bypass surgery using linear staplers.

BACKGROUND: The use of circular staplers with a low staple height is associated with a lower risk for complication when used to construct the gastroenterostomy in laparoscopic gastric bypass surgery. The influence of staple height on outcome when using linear staplers has not been studied. OBJECTIVES: To investigate the influence of staple height when constructing the gastric pouch and gastroenterostomy using a linear stapler in laparoscopic gastric bypass surgery. SETTING: Nationwide, Sweden. METHODS: A retrospective, register-based cohort study, including all primary laparoscopic gastric

Gastric Outlet Obstruction: Current Status and Future Directions.

Gastric outlet obstruction (GOO) is a relatively common condition in which mechanical obstruction of the pylorus, distal stomach, or duodenum causes severe symptoms such as nausea, vomiting, abdominal pain, and early satiety. Its etiology includes both benign and malignant disorders. Currently, GOO has many treatment options, including initial conservative therapeutic protocols and more invasive procedures, such as surgical gastroenterostomy, stent placement and, the most recently implemented procedure, endoscopic ultrasound-guided gastroenterostomy (EUS-GE). Each procedure has its merits, wit

Isolated Superior Mesenteric Vein Tumor Thrombus in a Patient with Gastric Cancer.

Tumor thrombus in the portal vein can rarely originate from gastric cancer via hematogenous spread, with only few case reports published in the literature. Isolated superior mesenteric vein tumor thrombus in gastric cancer has not been previously reported. A 61-year-old male patient who had undergone distal gastrectomy and gastroenterostomy for gastric ulcer 20 years ago was diagnosed with an obstructive tumor originating from the gastroenterostomy anastomosis site on upper gastrointestinal endoscopy that was performed for complaints of fatigue, oral feeding problems, and anemia. The PET-CT im

Five- to eight-year results of truncal vagotomy and pyloroplasty for duodenal ulcer.

From January 1963 to December 1965 inclusive 192 men with duodenal ulcer were treated by elective truncal vagotomy and pyloroplasty with one death. Ten subsequent deaths were due to causes unrelated to the ulcer or operation, and 17 patients became untraceable. The remaining 164 patients have been followed up for five to eight years. The late results have been compared with those obtained in a previous study of patients five to eight years after truncal vagotomy and gastroenterostomy, truncal vagotomy and antrectomy, and subtotal gastrectomy respectively for duodenal ulcer.Of the various postg

Unveiling hidden outcomes in malignant gastric outlet obstruction research - insights from a "Pancreas 2000" review.

Malignant gastric outlet obstruction (mGOO) is a major condition affecting patients with periampullary tumors, including pancreatic cancer. The current treatment options include surgical gastroenterostomy, endoscopic stenting and more recently EUS-guided gastroenterostomy. Most studies comparing the outcomes of the three procedures focus on technical success, clinical success and safety. Several "occult" outcomes relevant to the patient's viewpoints and perspective may ultimately impact on cancer-related and overall survival, such as body mass composition, nutritional biomarkers, chemotherapy

Giant marginal ulcer.

Marginal ulcer is a well-known complication of gastroenterostomy. It occurs in 3% of patients post-Billroth II subtotal gastrectomy; it occurs in less than 1% if truncal vagotomy is included but in up to 30% of patients with gastroenterostomy without vagotomy. These ulcers occur at the anastomosis, but always on the jejunal side, and are known to develop complications of their own--e.g., intractable pain; hemorrhage, obstruction, perforation, and fistula formation. Prior to the advent of upper-GI endoscopy the main method of diagnosis was by history and upper GI series but the accuracy of the

[What is certain in the pathogenesis and incidence of primary carcinoma involving the operated stomach?].

It has been postulated that resection therapy for peptic ulcer disease will result in an increased frequency of gastric stump cancer depending on the ulcer type and type of operation (Billroth I, Billroth II, gastroenterostomy). However, an extensive review of the literature shows that after Billroth II resection for gastric ulcer without enteroanastomosis the frequency of primary gastric stump cancer is not greater than in the case of medically treated benign gastric ulcers. The frequency of cancer of the gastric stump seems lower after resection for duodenal ulcer disease. It is possible tha

Endoscopic stenting for gastric outlet obstruction in patients with unresectable antro pyloric cancer. Systematic review of the literature and final results of a prospective study. The point of view of a surgical group.

BACKGROUND: The authors report the final results of a prospective single-center randomized study whose aim was to compare the endoscopic placement of self-expandable stents with open surgical gastroenterostomy to relieve gastric outlet obstruction (GOO) in patients with advanced antropyloric adenocarcinoma. A systematic review of the medical literature from December 1999 to December 2011 was carried out to determine the results of endoscopic stenting in patients with GOO from unresectable primary cancer of the antropyloric region. METHODS: In the prospective study, 18 patients with advanced a

Lumen-apposing metal stents (with videos).

BACKGROUND AND AIMS: Lumen-apposing metal stents (LAMSs) are a novel class of devices that have expanded the spectrum of endoscopic GI interventions. LAMSs with their dumbbell configuration, short saddle length, and large inner luminal diameter provide favorable stent characteristics to facilitate anastomosis formation between the gut lumen and adjacent structures. METHODS: The MEDLINE database was searched through April 2021 for articles related to LAMSs by using additional relevant keywords such as "walled-off pancreatic necrosis," "pseudocysts," "pancreatic fluid collection," "cholecystiti

Pylorus-preserving versus pylorus-resecting pancreaticoduodenectomy for periampullary and pancreatic carcinoma: a meta-analysis.

BACKGROUND: The aim of this meta-analysis was to compare the long-term survival, mortality, morbidity and the operation-related events in patients with periampullary and pancreatic carcinoma undergoing pylorus-preserving pancreaticoduodenectomy (PPPD) and pylorus-resecting pancreaticoduodenectomy (PRPD). METHOD: A systematic search of literature databases (Cochrane Library, PubMed, EMBASE and Web of Science) was performed to identify studies. Unresectable pancreatic carcinoma: correlating length of survival with choice of palliative bypass.

The preferred method of biliary bypass and the need for prophylactic gastroenterostomy in unresectable pancreatic carcinoma are dependent on the length of survival of the patient. From 1980 through 1996, 60 patients with biopsy-proven pancreatic cancer were found to be unresectable at exploration. The reasons for unresectability included major vascular involvement in 21 patients (35%), liver metastases in 16 (26.7%), celiac or portal lymph node metastases in 13 (21.7%), carcinomatosis in 5 (8.3%), and advanced age and/or comorbid medical condition in 4 patients (6.7%).
