How health maintenance organizations prepared for the dental treatment reform for older adults in Israel.

BACKGROUND: The oral health of older adults in Israel is still worse than in many developed countries. In 2019, a Universal Health Coverage reform of dental services for older adults was initiated in Israel to improve their access to dental services and to mitigate some of the barriers they face, mainly the high cost. The aim of the study was to assess how the Health Maintenance Organizations (HMOs) prepared for the reform of dental treatments for older adults. METHODS: Semi-structured telephone interviews were conducted with ten key figures and senior officials in the HMOs who were involved i

PATHA's vision for transgender healthcare under the current health reforms.

AIM: The Aotearoa New Zealand healthcare system does not adequately meet the needs of transgender people. Due to healthcare reforms and increases in funding and awareness of transgender health, the Ministry of Health has met with the Professional Association for Transgender Health Aotearoa (PATHA) to discuss ways to improve the healthcare system. We developed a vision for a transgender healthcare document to enable a process for our members to collaborate and to increase transparency about what advice PATHA has provided to the Ministry. METHOD: Feedback from PATHA's committees was incorporate

Challenges toward achieving universal health coverage in Ghana, Kenya, Nigeria, and Tanzania.

Although many sub-Saharan African countries have made efforts to provide universal health coverage (UHC) for their citizens, several of these initiatives have achieved little success. This study aims to review the challenges facing UHC in Ghana, Kenya, Nigeria, and Tanzania, and to suggest program or policy changes that might bolster UHC. Routine data reported by the World Bank and World Health Organization, as well as annual reports of the national health insurance schemes of Ghana, Kenya, Nigeria, and Tanzania, were analyzed. The data were supplemented by a review of published and gray liter

[Changes in the environment of medical practice].

Changes in medical practice have been very deep in the last 50 years. These changes have happened together with other external variations in the economy, demography, culture and information, generating a sense of uneasiness and unhappiness among the medical profession. This problem is visible when interviewing clinicians in a wide range of medical settings, independently of their contractual arrangement. The medical work model in Chile has changed from a situation in which there was a sole employer, the National Health Service, one single training school, the University of Chile, and a small m

Colombia and Cuba, contrasting models in Latin America's health sector reform.

Latin American national health systems were drastically overhauled by the health sector reforms the 1990s. Governments were urged by donors and by the international financial institutions to make major institutional changes, including the separation of purchaser and provider functions and privatization. This article first analyses a striking paradox of the far-reaching reform measures: contrary to what is imposed on public health services, after privatization purchaser and provider functions are reunited. Then we compare two contrasting examples: Colombia, which is internationally promoted as

Elevated body weight in Mexico: Structural drivers, policy challenges, and the limits of health policy reform.

Mexico has one of the world's highest prevalence rates of elevated body weight among adults, with more than one-third of children and adolescents also affected. These patterns create major challenges for life expectancy, healthcare system sustainability, and long-term economic productivity. This article examines current patterns of elevated body weight in Mexico and their links with chronic disease. It explores the sociohistorical, political-economic, and cultural factors that have shaped these trends and critically evaluates President Claudia Sheinbaum's Healthy Republic Plan as a policy fram

How hospitals' goal setting, feedback, and process standardization capacity impact provider payment reforms.

BACKGROUND: Provider payment reforms (PPRs) can improve providers' efficiency, but they often generate mixed results. Since organizations mediate PPR effectiveness, examining hospitals' management capacity's association with PPR effectiveness can be useful. In the context of clear strategies, hospitals' management characteristics related to goal attainment would be key to PPR adaptation. This study examines hospitals' capacity to set goals at appropriate difficulty or specificity, provide feedback, or standardize processes. METHODS: We leverage a matched-pair, cluster randomized controlled PP

Global Mental Health in South Lebanon: Psychoeducation, Translation, and Culture.

In this article I examine the unintended consequences of global mental health reform in postwar South Lebanon. I follow the integration of mental health in primary health care centers. Three key tensions emerge: First, integrating mental health led to separating mental health from socioeconomic conditions. Second, culture was simultaneously conceived as a barrier to therapy and a local context requiring adaptation. Third, the use of personality disorders as psychological interpretations of social life resulted in psychologizing gender and class. The article contributes to growing ethnographic

A scoping review on community health care centers: Evidence in support of Italian policymakers to redesign the Italian primary care system.

BACKGROUND: The COVID-19 pandemic underscored the central role of primary health care in managing patients with complex and chronic health conditions. In response, the Italian government launched a reform of the National Health Service, introducing Community Health Care Centers ("Case della Comunità") as a key pillar of the new primary care model. Evidence is needed on which organizational features of such centers best support integrated, high-quality care. OBJECTIVE: This scoping review synthesized international evidence on community health care centers characterized by integrated care and c

Providers' constructions of the role of women in cervical cancer screening in Bulgaria and Romania.

The social and institutional context of health-care reform in Eastern Europe has important implications for cervical cancer screening and prevention. The incidence and mortality from cervical cancer in Bulgaria and Romania have risen, which is in sharp contrast to the steady decline in most other countries in Europe during the last 2 decades. To analyze these dynamics we conducted a multi-component study of health systems and psychosocial aspects of cervical cancer screening in Bulgaria and Romania. Following the disappearance of organized preventive programs, the initiative for cervical cance

The Soviet export of sleep therapy to the Eastern Bloc countries after the Pavlovian session in 1950.

Following the decisions of the scientific session 'For the further flourishing of Pavlov's doctrine' of the Academy of Sciences and the Academy of Medical Sciences of the USSR in 1950, important reforms were introduced under political control in the USSR and the Eastern Bloc countries. Research plans of science institutions and medical university curricula were changed according to these decisions. Scientists and university professors were forced to adopt courses in Pavlovian doctrine. The reforms affected the work of hospitals and sanatoriums, whose staff was instructed to reform the everyday

Male infertility and somatic health - insights into lipid damage as a mechanistic link.

Over the past decade, mounting evidence has shown an alarming association between male subfertility and poor somatic health, with substantial evidence supporting the increased incidence of oncological disease, cardiovascular disease, metabolic disorders and autoimmune diseases in men who have previously received a subfertility diagnosis. This paradigm is concerning, but might also provide a novel window for a crucial health reform in which the infertile phenotype could serve as an indication of potential pathological conditions. One of the major limiting factors in this association is the poor

You might expect a reelected President Clinton and the Republican Congress to approach health care legislation like a postman who's been bitten by a dog-very carefully. But waiting isn't an option, not with Medicare woes unsolved. Will bipartisanship prevail?

Bureaucratic Itineraries in Colombia. A theoretical and methodological tool to assess managed-care health care systems.

Steady increases in the number of Colombians insured by the health care system contrasts with the hundreds of thousands of legal actions interposed to warrant citizen's right to health. This study aims to analyze the relationships among patients' experiences of denials by the system, the country's legal mechanisms, and the functioning of insurance companies and service providing institutions. We conducted a mixed-methods case study in Bogotá and present a quantitative description of 458 cases, along with semi-structured interviews and an in-depth illness history. We found that Colombians' deni

Physician strikes.

Throughout medical history, physicians have rarely formed unions and/or carried out strikes. In a profession faced with the turmoil of health reform and increasing pressure to change their practices and lifestyles, will physicians resort to unionization for collective bargaining, and will a strike weapon be used to fight back against the array of corporate and government powers involved in the transformation of the American health-care system? This article examines the question of whether there could be such a thing as an ethical physician strike. Although physicians have not historically used

Managing the challenges of paying for gene therapy: strategies for market action and policy reform in the United States.

Gene therapies delivered through a single administration have revolutionized treatment possibilities for many patients living with serious or fatal conditions such as spinal muscular atrophy, hemophilia and sickle cell disease. However, shadowing the excitement about the transformational potential of many gene therapies has been widespread concern about the combination of uncertainty in the durability of their benefits over the long term and the short-term financial shock of high prices. As the healthcare payment ecosystem prepares for the growing number of gene therapies entering the market,

Looking at CER from the managed care organization perspective.

BackgroundThe amount of available comparative effectiveness research (CER) is increasing, giving managed care organizations (MCOs) more information to use in decision making. However, MCOs may not be prepared to integrate this new and voluminous data into their current practices and policies.ObjectivesTo describe ways that health care reform will affect MCO populations in the future, to examine examples of how MCOs have utilized CER data in the past, and to identify questions that MCOs will have to address as they integrate CER into future decision making.SummaryUnquestionably, health care ref
