Mitral valve replacement (MVR) using a prosthesis of transcatheter aortic valve implantation for mitral annular calcification (MAC) is an alternative procedure which is very different from a conventional MVR and is safe. High transvalvular pressure gradients following aortic valve replacement can be caused by several possible mechanisms. We present the case of an elderly woman with an elevated pressure gradient across an aortic valve bioprosthesis in the setting of complete heart block. OBJECTIVES: Assessment of the long term outcome of mechanical valve prosthesis at pulmonary position in a population of grown-up congenital heart disease patients from a tertiary referral center.
METHODS: From 1977 to 2007, 22 consecutive patients underwent a total of 25 pulmonary valve replacements with mechanical prosthesis. The most frequent underlying cardiac condition was tetralogy of Fallot (n=16, 64%) and the mean age at the time of pulmonary valve replacement was 32 ± 11 years (range 14-50 years).

BACKGROUND AND AIMS: The longevity of the mechanical heart valve prosthesis is an advantage when compared with its biological counterpart, although the former carries a risk of thrombosis depending on valve design, materials and host-related interface; therefore, a patient with a mechanical valve prosthesis, particularly in mitral position, is at risk for systemic thromboembolism even when properly anticoagulated.

Operations on the open heart, of them in 62 (32.9%)--the valves prosthesis, were conducted on the base of Clinical Treatment-Diagnosis Centre "Simedgroup" of Ivano-Frankivskiy National Medical University in 2012 - 2015 yrs in 188 patients. Hemodynamically significant failures of a mitral valve and/or the aortal valve were considered as indications for the operative treatment conduction. Combined surgical treatment--the valve prosthesis and coronary shunting--were conducted in coincident ischemic heart disease. More durable rehabilitation is needed for elderly and senile patients.

Single heart-valve replacement with a Björk-Shiley prosthesis was undertaken in 118 patients between Jan. 3, 1970 and Dec. 31, 1977. BACKGROUND: The current generation of prosthetic heart valves are excellent examples of engineering that work well, for long periods of time. Unfortunately, host-related problems and consequent valve failure continue to occur, including infective endocarditis. Reports of experience with the St. Jude Medical (SJM) valve state that thrombosis of the prosthesis is a rare complication.
