Total hip arthroplasty (THA) is a standard procedure to alleviate pain and improve joint function in patients with advanced degenerative hip osteoarthritis. Hemipelvectomy, or hindquarter amputation, is a complex, technically demanding procedure with a challenging prosthetic rehabilitation journey. It is often indicated for malignancies or traumatic injury. Hemipelvectomy often leads to a lack of structural stability and altered weight distribution, requiring customized approaches to ensure stability and functional restoration. Thus, THA in patients who have undergone contralateral hemipelvect

INTRODUCTION: Hemipelvectomy procedures result in massive soft tissue defects. The standard approach is to reconstruct the defect with anterior or posterior hemipelvectomy flaps. Certain situations preclude the use of local tissue flaps, and an alternative is the use of leg fillet flaps, circumferential pedicled or free flaps harvested from the amputated part. The purpose of this study is to present our institution's experience with using pedicled and free fillet flaps to reconstruct hemipelvectomy soft tissue defects.
METHODS: We performed a retrospective chart review of patients who underwe

A bucket-type transpelvic socket was fabricated for a man with paraplegia from spinal cord injury, who underwent right partial pelvic amputation and left hip disarticulation. His main problem was inability to sit due to asymmetrical pelvic shape. We prescribed a transpelvic prosthetic socket to enable him to sit again. The socket consisted of a dual structure: a hard frame and soft liner. The main features of the socket were redistribution of pressure to prevent recurrence of pressure ulcer, and a slightly backward tilt to maintain a comfortable sitting position. In addition, the socket had sm

Background: Chondrosarcomas account for approximately 20% of bone sarcomas, with the most common site being the lower limb. Hemipelvectomies that involve hindquarter amputation have previously been the treatment of choice for pelvic tumors. However, with advances in chemotherapy, radiotherapy, imaging and surgical techniques more patients are now being treated with limb salvage surgery. The rehabilitation outcomes following an internal hemipelvectomy have not been well identified and there is currently little, if any, gait analysis data on rehabilitation following hemipelvectomy in limb salvag

BACKGROUND: Internal hemipelvectomy is a challenging procedure often complicated by wound healing issues. To guide surgical planning and reduce the risk of wound complications, we developed and validated a preoperative risk-stratification model.
STUDY DESIGN: We reviewed clinical, surgical, and pathologic data from 411 patients who underwent en bloc internal hemipelvectomy for malignant pelvic tumors between 1996 and 2018. The sample included a training set (N = 123) from a US tertiary hospital and a validation set (N = 288) from centers in Canada and The Netherlands. Using logistic regressio

Traumatic hemipelvectomy (THP) is a rare, life-threatening injury characterized by the dislocation of the hemipelvis, often resulting from high-energy trauma. It carries a high mortality risk due to severe vascular injuries, with survival contingent on rapid hemorrhage control and a multidisciplinary approach. Limb salvage is rarely achievable due to the extent of the injury and complications such as infection and tissue nonviability. We present a case of incomplete THP in a 29-year-old woman who sustained severe soft tissue, bony, and vascular injuries following a boating accident. On arrival

OBJECTIVE: To compare the clinical efficacy of the posterior hemipelvectomy of the greater trochanter approach with the standard Kocher-Langenbeck(K-L) approach in the treatment of posterior acetabulum wall fractures and to explore a more optimal approach for the treatment of posterior acetabulum wall fractures.
METHODS: Total of 26 patients with posterior acetabulum wall fractures were retrospectively analysed and divided into two groups:the posterior hemipelvectomy of the greater trochanter group (test group) and the standard K-L approach group (control group). In the test group, there were

BACKGROUND: Pregnancy after childhood hemipelvectomy is rare, and contemporary guidance on fertility, preconception counseling, and delivery planning remains limited. Altered pelvic anatomy may affect fetal descent and rotation ; however, the optimal mode and delivery timing should be individualized.
CASE: A 30-year-old primigravida, with a history of right hemipelvectomy for osteosarcoma, conceived spontaneously after regular menstrual cycles. Fertility preservation was not performed during childhood treatment, and formal preconception counseling with an obstetric team was not provided. Afte

BACKGROUND: Rehabilitation after hip disarticulation and hemipelvectomy amputations can be challenging, and many opt to forego prosthetic limb use. There is limited evidence on characteristics that result in successful prosthetic use; therefore, our study aimed to identify these to help guide patient expectations.
METHODS: A retrospective review was performed on patients seen at a UK tertiary prosthetic center in the past 5 years with hip disarticulation or hemipelvectomy amputations. Details and etiology of amputation, length between assessment and delivery of prosthesis, goals, reasons for

The free fillet flap is a well-established reconstructive option following external hemipelvectomy. However, mitigating prolonged warm ischemia during the extensive tumor resection poses a significant challenge to flap viability. Presented herein is a case illustrating the novel use of temporary intraoperative vascular shunting to overcome this limitation. During hemipelvectomy for a 17.7 × 16.9 × 19.3 cm osteosarcoma of the proximal left femur in a 74-year-old man, unplanned early ischemia of the lower extremity was addressed by placing carotid shunts between the external iliac and popliteal

OBJECTIVE: There are several options for soft tissue coverage following external hemipelvectomy; however, in cases of war-related blast trauma, standard flaps are not always available as a result of the extensive soft tissue damage.
METHODS: We detail a novel closure technique following a subtotal hemipelvectomy with exposed abdominal viscera using a residual hamstring myofascial cutaneous flap.
RESULTS: This flap allowed for fascial tissue to fill the pelvic defect and provided excellent soft tissue coverage for future prosthetic wear.
DISCUSSION: In the current literature, there is limite

BACKGROUND: Pelvic reconstruction after type I + II (or type I + II + III) internal hemipelvectomy with extensive ilium removal is a great challenge. In an attempt to anatomically reconstruct the hip rotation center (HRC) and achieve a low mechanical failure rate, a custom-made, 3D-printed prosthesis with a porous articular interface was developed. The aim of this study was to investigate the clinical outcomes of patients treated with this prosthesis.
METHODS: This retrospective cohort study included 28 patients with type I + II (+ III) internal hemipelvectomy through the articular interface

Hemipelvectomy is surgery for pelvic bone neoplasms. In the case of pubic bone osteosarcoma, the distal end of the rectus abdominis muscle is severed from the pubic and ischium bones, and the pelvic floor muscles are resected en bloc with the bone, which leads to stress urinary incontinence. Cancer control is prioritized over complications, and stress urinary incontinence is generally disregarded. A 25-year-old woman presented with stress urinary incontinence. She had undergone a hemipelvectomy for left pubic bone osteosarcoma, and stress urinary incontinence appeared and persisted since the s

PURPOSE: To report a rare case of lumbosacral instability following a right external hemipelvectomy for malignant peripheral nerve sheath tumor (MPNST), successfully treated with rigid sacropelvic fixation using a combination of a unilateral iliac screw and an S2 alar-iliac screw.
METHODS: MPNST is a malignant soft tissue sarcoma with a high recurrence rate and poor prognosis. A 44-year-old woman was diagnosed with MPNST in the right pelvis. Despite multiple courses of chemotherapy, the tumor lesion continued to grow. A right external hemipelvectomy was performed, and histopathological analys

PURPOSE: Limb-sparing resection of malignant pelvic tumours provides the opportunity for patients to obtain better post-operative mobility. However, because few studies have examined in detail the gait function of patients following pelvic tumour resection, the factors affecting gait performance remain to be clarified. Here, with the laboratory-based computer-assisted gait analysis, we evaluated these patients' gait objectively and the impact of a hip-stabilising supporter on gait improvement was simultaneously examined.
METHODS: Three-dimensional gait analysis was performed to obtain cross-s

BACKGROUND AND OBJECTIVES: Internal hemipelvectomy is a limb sparing procedure most commonly indicated for malignant bone and soft tissue tumors of the pelvis. Partial resection and pelvic reconstruction may be challenging for orthopedic oncologists due to late presentation, high tumor burden, and complex anatomy. Specifically, wide resection of tumors involving the periacetabular and sacroiliac (SI) regions may compromise adjacent vital neurovascular structures, impair wound healing, or limit functional recovery. We aimed to present a series of patients treated at our institution who underwen

BACKGROUND: Hip disarticulation and hemipelvectomy are alternatives to limb-salvage procedures for patients with extensive tumors of the upper thigh and buttocks. In cases when neither the conventional posterior gluteus maximus flap nor the anterior quadriceps flap can be used because of the location of the tumor, a medial adductor myocutaneous flap may be an alternative.
DESCRIPTION OF TECHNIQUE: The flap is outlined over the anteromedial thigh. The distal extent is at the level of the adductor hiatus. The common femoral vessels and nerve are traced, preserved, and protected. The adductor mu

BACKGROUND: Type 1 internal hemipelvectomies and total sacrectomies cause significant biomechanical instability, demanding pelvic ring reconstruction for ambulation and torso support. Previously described methods include autografts, allografts, and implants, commonly with poor long-term outcomes. The authors hypothesized that the free fibula flap for spinopelvic reconstruction is safe and effective, and associated with a high bony union rate and superior functional outcomes.
METHODS: The authors performed a retrospective review of all patients who underwent free fibula flap surgery after inte

INTRODUCTION: In patients undergoing limb-salvage internal hemipelvectomy, pelvic ring reconstruction is mandatory to maintain the stability of the pelvis and the spinal column, which finally expected to achieve a good functional outcome. However, no optimal reconstruction method has been established. In addition, no previous reports have highlighted the long-term complications of pelvic ring reconstruction after internal hemipelvectomy. We aimed to analyze the outcome of pelvic ring reconstruction using a double-barreled free vascularized fibula graft (VFG) after internal hemipelvectomy with
