Evidence-based practice in child and adolescent mental health2025eng
BACKGROUND: Relaxation is a commonly used, albeit controversial, therapy technique to support anxious youth. Increasingly, clinical trial data suggest little added benefit, and potentially a contraindication, for the use of relaxation strategies for pediatric anxiety and obsessive-compulsive disorders above and beyond that of exposure therapy. However, many clinicians, even those with substantial experience delivering exposure therapy, will at times incorporate relaxation into their treatment. How and when it is best to do so remains poorly articulated in the clinical trial literature. Delinea

There is evidence that exposure and response prevention (ERP) is efficacious in the treatment of obsessive-compulsive disorder (OCD). As an alternative to ERP metacognitive therapy (MCT) was developed. Two previous randomized controlled trials (RCTs) did not find significant differences between MCT and ERP. However, from non-significant results, non-inferiority of a treatment cannot be concluded. For this purpose, non-inferiority studies are required. Exner and colleagues carried out such a non-inferiority study whose results were recently published in this journal. The authors concluded from

Acceptance and commitment therapy (ACT) combined with exposure and response prevention (ERP) is an effective treatment for obsessive-compulsive disorder (OCD). Due to the influence of sociocultural factors on OCD symptomatology, it is crucial to customize and assess ACT within diverse cultural settings. We present an online group protocol for ACT-ERP, tailored for Brazilian adults with OCD, based on a prior pilot study and the work of Twohig and colleagues. The protocol adaptations for online use in a group format included technical arrangements, adjusted session frequency, and cultural adapta

Cognitive-behavioral therapy (CBT) is the strongest evidenced-based therapy for childhood anxiety disorders (CADs). However, CBT's impact is limited by its lack of clear superiority over treatment as usual, excessive length, and greater than 50% of patients remaining symptomatic. Parent-coached exposure therapy (PCET) is designed to treat CADs more effectively and efficiently through a focus on exposure and working with parents and youth together. In a randomized controlled trial, 78 patients (78% female) aged 7 to 17 with CADs were assigned to PCET or the gold-standard CBT. The primary outcom

Background: Refugees with exposure to multiple traumatic events are at high risk for developing posttraumatic stress disorder (PTSD) and depression. Narrative exposure therapy (NET) is an effective treatment for the core symptoms of PTSD, but it does not reliably reduce depressive symptoms. Endurance exercise on the other hand was consistently found to be effective in treating depression making it a promising adjunct to NET. Up to date, no studies exist investigating the combination of NET and endurance exercise in a sample of refugees with PTSD and comorbid depression. Objectives: In the propo

Family accommodation (FA) is key in the maintenance of OCD. However, most studies are cross-sectional, and have not identified long-term trajectories and correlates of FA changes over treatment. This study investigated changes in clinician-rated FA over 10 sessions of cognitive behavioral therapy (CBT) for 142 children with OCD, as well as several key predictors/correlates: (a) clinician-rated OCD symptom severity, (b) child- and parent-rated functional impairment, (c) parent-rated treatment expectancies, and (d) child- and parent-rated therapeutic alliance. Multi-level models indicated that F

In the presence of asthma, the risk of having an anxiety disorder is increased twofold. The few trials conducted on cognitive behavior therapy (CBT) for anxiety and asthma have mainly targeted panic disorder, and with mixed results. Experimental laboratory research indicates that increased anxiety may lead to hypervigilance toward asthma. Hence, fear and avoidance associated with increased anxiety due to asthma may be an important treatment target. A treatment that learn participants to differentiate between anxiety and asthma through gradual exposure to situations that risk triggering anxiety

Exposure therapy is a potential method for the treatment of eating disorders. The current paper reviews the literature on exposure interventions for eating disorders, including studies (N = 60) on exposure and response prevention (ERP), in vivo feared food exposure, mirror exposure, family-based treatment with exposure, and virtual reality exposure therapy. Mirror exposure alone or in the context of cognitive-behavioral therapy (CBT) can decrease body dissatisfaction. The few controlled trials on ERP for binge and purge cues show only marginal benefit of ERP for binge or purge cues over and ab

There is a high prevalence of anxiety disorders including specific phobias and panic disorder in the United States and Europe. A variety of therapeutic modalities including pharmacotherapy, cognitive behavioral therapy, systematic desensitization, hypnosis, in vivo exposure, and virtual reality exposure therapy have been applied. No one modality has been entirely successful. There has been only a limited attempt to combine psychological therapies in the treatment of specific phobias and panic disorder and what has been done has been primarily with systematic desensitization or cognitive behavi

Cognitive behavioral therapy (CBT) is an evidence-based treatment for mental disorders. Several meta-analytical reviews supported its efficacy and effectiveness in the treatment of panic disorder with agoraphobia (PD/AG). Recently, it has been shown that neurobiological changes are associated with the process and outcome of CBT. However, the general and specific neurobiological effects of CBT are still widely unknown. Therefore, the potential of applying neuroscience to clinical practice and optimizing CBT is still limited. The current review summarizes recent findings about the neural correla

Post-Traumatic Stress Disorder (PTSD) is connected with the experience of traumatic events and is significantly related to war and forced displacement. Refugee populations are characterized by a high degree of vulnerability for the development of PTSD, as they are confronted with stressors associated with all three distinct phases of migratory journey. The present article is an attempt to systematic review the therapeutic interventions based on the Cognitive Behavioral Approach and applied to refugees diagnosed with PTSD. For this reason, a review of the international literature was carried ou

Obsessive-compulsive symptom profiles vary widely among individuals and may be affected by cultural backgrounds. In the case of scrupulosity, moral and religious principles are the target of obsessive-compulsive symptoms. Cognitive biases and beliefs have special relevance in the origin and maintenance of obsessive scrupulosity. In addition, rigid and exaggerated beliefs about morality are held by these patients. Moral and religious principles are mainly transmitted by family. These influences may be more prominent in cultures, such as the Spanish culture, where family and religion are importa

Obsessive-compulsive disorder (OCD) is a frequent condition in the pediatric population, with significant clinical impact and high comorbidity. This umbrella review aimed to synthesize evidence from systematic reviews and meta-analyses on the efficacy of interventions for OCD in individuals under 18 years of age. Searches were conducted in five electronic databases up to July 2025. A total of 28 systematic reviews and meta-analyses published between 2003 and 2025 were included, assessing the efficacy of various treatments and encompassing a total of 24,762 participants. Arachnophobia is a common specific phobia, yet access to therapy is limited. This single-case study examined the use of a mixed reality exposure therapy (MRET) for spider phobia, enabling room-aware spatial mapping and passthrough vision for safety. Its core feature is a spider behavior system that simulates naturalistic, unpredictable movements animated procedurally for ecological validity. The study participant was a 56-year-old female patient with chronic spider phobia who had shown limited response to two previous cognitive behavioral treatments. Progress was assessed at nine time points o

BACKGROUND: Specific phobia of vomiting (SPOV), also called emetophobia, is a debilitating condition that shares features with several other anxiety disorders and obsessive-compulsive disorder (OCD). Approximately half of sufferers from SPOV do not fully benefit from current treatment modalities.
AIMS: Bergen 4-day treatment (B4DT) is a highly concentrated form of exposure and response prevention developed for OCD. This case series reports on the first participants undertaking the treatment for SPOV.
METHOD: Five female participants underwent the B4DT adapted to SPOV. The Specific Phobia of

Interpersonal trauma can be disruptive at any point in the lifespan; however, it carries a unique consequence profile when repetitive betrayals and abuse are present in early childhood. In the case of Ruby, she developed various parts to manage her distress, some of whom focused on engaging in risky or other destructive behavior without the knowledge of the rest of the system, as well as significant substance use to mediate her pain. This case study focuses on what has worked over the course of her 15-year treatment history with this author. Although she was not in therapy for that entire time

BACKGROUND: The RESET (Reconsolidation, Exposure, and Short-term Emotional Transformation) clinical protocol is an intensive, structured trauma-focused intervention designed to treat post-traumatic stress disorder (PTSD) within six daily sessions. While Prolonged Exposure (PE) therapy remains a leading evidence-based treatment, challenges such as high dropout rates and relapses highlight the need for innovative approaches.
OBJECTIVE: This paper outlines RESET's theoretical foundations, session structure, and clinical implementation, offering a detailed framework for its integration into psych

Exposure therapy (ET) forms a vital part of effective psychotherapy for anxiety-related presentations including anxiety disorders, obsessive-compulsive disorder (OCD), and post-traumatic stress disorder (PTSD), and is often underutilised in clinical practice. Using the Theoretical Domains Framework (TDF), this systematic review synthesised existing literature on the determinants of ET implementation for anxiety-related presentations and examined differences across presentations and developmental subgroups. Fifty-two eligible studies were assessed using the Mixed Methods Appraisal Tool, with 38

Obsessive-Compulsive Disorder (OCD), characterized by the combination of obsession and compulsion, is a clinical and therapeutic challenge. Many patients with OCD do not respond to first-line treatments such as serotonin selective reuptake inhibitors (SSRIs) and exposure and response prevention psychotherapy (ERP). For these resistant patients, some preliminary studies have shown that ketamine, a non-selective glutamatergic NMDA receptors antagonist, could improve the obsessive symptoms. A number of these studies have also suggested that the combination of ketamine with ERP psychotherapy may j
