Аннотация

BACKGROUND: Relaxation is a commonly used, albeit controversial, therapy technique to support anxious youth. Increasingly, clinical trial data suggest little added benefit, and potentially a contraindication, for the use of relaxation strategies for pediatric anxiety and obsessive-compulsive disorders above and beyond that of exposure therapy. However, many clinicians, even those with substantial experience delivering exposure therapy, will at times incorporate relaxation into their treatment. How and when it is best to do so remains poorly articulated in the clinical trial literature. Delineating guidelines for how and when to incorporate relaxation into exposure-based treatments would advance training and implementation efforts aimed at increasing access to high-quality care for anxious youth.

OBJECTIVE: To gather empirical data on the perspectives of clinicians with expertise in delivering exposure therapy to anxious youth about how they think about and use relaxation, as well as mindfulness (a distinct but related technique), in treatment.

METHODS: We conducted an electronic survey of identified specialists in pediatric exposure therapy treatment to elicit their perspectives (N = 79; 69.6% female, 11.5 years of experience on average, 59.5% doctoral degree).

RESULTS: More than 80% of clinicians in the sample reported relaxation techniques should be used sparingly (if at all) within exposure practices. The sample also exhibited strong consensus that relaxation strategies have utility in the treatment of some anxious youth as an independent coping strategy. However, there was limited consensus regarding the extent to which relaxation should be incorporated and for what clinical presentations. Mindfulness was viewed as more helpful and clinically necessary than relaxation.

CONCLUSIONS: Additional work is needed to refine recommendations for how and when to incorporate relaxation into pediatric anxiety treatment.
