Clarification of Extensor Tenotomy for Finger Deformities.

This article clarifies the terminology and different applications of proximal Fowler extensor tenotomy and distal Dolphin extensor tenotomy. Although Fowler is a popular eponym for all finger extensor tenotomies, proximal Fowler tenotomy is specifically indicated for swan neck deformities, whereas distal Dolphin tenotomy is indicated for boutonniere deformities. For swan neck deformities with a distal interphalangeal joint extensor lag of greater than 40°, central slip tenotomy can be combined with terminal tendon imbrication or reinsertion. More distal Dolphin tenotomy of the lateral conjoine

A modified Terrono classification for Type 1 thumb deformity in rheumatoid arthritis: a cross-sectional analysis.

The most common thumb deformity in rheumatoid arthritis is Nalebuff Type 1 deformity (boutonniere deformity). Type 1 deformity severely impairs hand function, and this impairment is evaluated by the Terrono classification. In some cases, the Terrono classification incorrectly categorizes advanced thumb deformity into earlier stages. We modified the Terrono classification by assessing the active range of motion of the interphalangeal joint prior to assessing the passive range of movement of the metacarpophalangeal joint. An active range of movement of the interphalangeal joint was strongly corr

Lupus foot: deforming arthropathy of the feet in systemic lupus erythematosus.

Passively correctable joint deformities of the feet were found in 13 of 14 (82.8%) systemic lupus erythematosus (SLE) patients with deforming arthropathy of the hands (Jaccoud's type of arthropathy). These included hallux valgus, subluxation of the metatarsophalangeal joints and widening of the forefoot. Erosive or cystic changes of bone and cartilage were not seen. In contrast, only 4 of 16 (25%) SLE patients without hand deformities, matched for age and duration of the disease, showed similar joint deformities of the feet. Jaccoud's type of arthropathy in SLE is characterized not only by pas

[Pseudo-pseudo-hypoparathyroidism (PPH) in a family: rheumatic manifestations in 3 cases, one of which associated with a tricho-rhino-phalangial syndrome (Giedion)].

Three cases of familial pseudo-pseudohypoparathyroidism (PPH) are described. Two patients presented the essential features of this rare syndrome, while the third had the signs of PPH associated with those of tricho-rhino-phalangeal dysplasia (GIEDION). All of our patients had spondylolisthesis of L5 as well as arthrosis of the hips (secondary to coxa plana in two subjects and to coxa vara in one). The phosphocalcic metabolism and determinations of serum immunoreactive parathyroid hormone were normal. The autosomal dominant genetic transmission is in agreement with observations in the literatur

Naviculocuneiform Sag in the Acquired Flatfoot: What to Do.

Sag at the naviculocuneiform (NC) joint represents an important aspect of the flatfoot deformity. Failure to address medial column instability could lead to continued deformity and poor patient outcomes. No single procedure is enough to address the complexity of the adult acquired flatfoot deformity. Whether in combination with other procedures or in isolation, NC fusion and Cotton osteotomy are important pieces of the armamentarium to address all aspects of the flatfoot deformity.

Total dorsal capsulectomy for the treatment of mucous cysts.

PURPOSE: To describe the technique of total dorsal capsulectomy alone for mucous cysts of the distal interphalangeal (DIP) joint and evaluate its outcomes and complications. METHODS: Nineteen patients (18 women and 1 man) with 19 mucous cysts were treated by a total dorsal capsulectomy without cyst excision or osteophyte removal. The average age at surgery was 63 years. The thumb was involved in 4 patients, index finger in 1, middle finger in 7, ring finger in 4, and little finger in 3. Twelve patients had nail deformities associated with the mucous cyst. The average period of postoperative f

Biomechanics and clinical analysis of the adult acquired flatfoot.

The adult acquired flatfoot is a deformity that results from the loss of dynamic and static supportive structures of the medial longitudinal arch. The severity of the deformity is dependent upon the role of ligamentous disruption on the hindfoot that can be determined by careful clinical examination. Treatment of the adult flatfoot requires an understanding of the biomechanical effects of deforming forces, tendon dysfunction, ligament disruption, and joint sublaxation.

[Swan-neck and buttonhole deformities on rheumatic long fingers].

Patients suffering from rheumatoid arthritis in many cases develop typical swan-neck and buttonhole deformities. In the further course of the disease we observe several stages. In the beginning active and later passive correction are still possible, while ultimately a fixed contracture is present. The activities of daily life may be severely reduced. The pathology of the swan-neck deformity is initiated at the level of the metacarpophalangeal joint, while at the origin of the buttonhole deformity the synovitis of the proximal interphalangeal joint is obvious. In the early stages, synovectomy a

Planar dominance. A major determinant in flatfoot stabilization.

For treatment of a symptomatic flat foot in a child or an acquired flatfoot from posterior tibial dysfunction in the adult, an understanding of the biomechanics and function of the foot and leg is essential to achieve an excellent outcome. Evaluation of supramalleolar deformity directly influences the procedure choice and eventual success of the procedure. An understanding of the level of deformity also indicates the procedure to be used. Subtalar and midtarsal joint function, although related, have their own primary source of deformity. Correction through the subtalar and midtarsal joints mus

Hexapod Frame Correction of Proximal Tibial Recurvatum Caused Indirectly by a Femoral Aneurysmal Bone Cyst in an Adolescent: A Case Report and Systematic Review.

CASE: A 16-year-old boy presented with a recurrent distal femur aneurysmal bone cyst accompanied by a combined sagittal knee deformity (20° of femoral antecurvatum and 26.8° of tibial recurvatum) and limb shortening. After preoperative planning, the treatment involved new intralesional curettage, phenolization, and bone allograft filling. Additional procedures included distal extension femoral osteotomy with plate fixation, and proximal tibial osteotomy with, gradually corrected through a hexapod frame. At 2-year follow-up, lower limbs exhibited normoalignment and equal length. CONCLUSION: Co

Development of an In Vitro Swan Neck Deformity Biomechanical Model.

BACKGROUND: Injury to the finger's extensor mechanism is a common cause of swan neck deformity (SND). Progression of extensor and flexor tendon imbalance negatively affects laxity of the volar plate, resulting in the inhibition of proper finger motion. The complexity of finger anatomy, however, makes understanding the pathomechanics of these deformities challenging. Therefore, development of an SND model is imperative to understand its influence on finger biomechanics and to provide an in vitro model to evaluate the various treatment options. METHODS: The index, middle, and ring fingers from

Total knee arthroplasty for severe valgus knee deformity.

BACKGROUND: Primary total knee arthroplasty (TKA) in severe valgus knees may prove challenging, and choice of implant depends on the severity of the valgus deformity and the extent of soft-tissue release. The purpose of this study was to review 8 to 11 years (mean, 10 years) follow-up results of primary TKA for varient-III valgus knee deformity with use of different type implants. METHODS: Between January 2002 and January 2005, 20 women and 12 men, aged 47 to 63 (mean, 57.19 ± 6.08) years old, with varient-III valgus knees underwent primary TKA. Of the 32 patients, 37 knees had varient-III de

Joint-preserving surgery of asymmetric ankle osteoarthritis with peritalar instability.

A varus or valgus talar tilt that increases under weight-bearing is commonly seen in osteoarthritic ankles. Loss of peritalar stability may be the underlying cause for the talus shifting and rotating on the calcaneonavicular surfaces, as given by applied forces. The instability pattern and the resulting deformity can be assessed and classified using weight-bearing conventional radiographs. Appropriate osseous balancing may be the most appropriate treatment to restore a regular position of talus within the ankle mortise. In cases with severe peritalar instability, subtalar fusion may be advised

Management of hip deformities in cerebral palsy.

Hip abnormalities affect most children with cerebral palsy. Dedicated surveillance programs have been shown to be effective means of identifying hips at risk and preventing pathologic dislocation. Patients who are ambulatory and correlate with Gross Motor Function Classification Score I and II experience deformities that affect mobility and gait, but rarely dislocations. Marginal and nonambulatory patients have an increasing risk of dislocation. Once subluxation has been identified, early surgical intervention is indicated. Long-term postoperative follow-up is needed to monitor for recurrence.

Current concepts in the surgical management of rheumatoid and osteoarthritic hands and wrists.

Rheumatoid arthritis (RA) is a progressively destructive disease. Gradual loss of hand function in RA patients affects their ability for self-care and interferes with their productivity in society. The continuing improvement in the medical management of RA has markedly decreased the incidence of RA hand surgery. In contrast to RA, osteoarthritis (OA) has less inflammatory reaction in the joints and is characterized by degradation of cartilage, resulting in joint destruction and osteophyte formation. The initial treatment of OA is medication and therapy. Steroid injection into affected joints c
