Falsely decreased triglyceride concentration in a patient with acute pancreatitis due to insufficient configuration of alarm rules.

Acute pancreatitis is a potentially life-threatening complication of severe hypertriglyceridemia. The accurate measurement of triglyceride concentration is essential for diagnosis and therapeutic monitoring. This article presents a case of acute hypertriglyceridemia pancreatitis in which the patient's triglyceride concentration appeared to drop rapidly from 128.04 mmol/L to 1.53 mmol/L within 12 hours of admission. Subsequent retesting revealed an actual triglyceride value of 88.92 mmol/L. This case underscores a critical and underreported preanalytical challenge: the failure of the analyzer t

Obstetric Emergency Update: Severe Acute Respiratory Syndrome COVID-19 and Hypertension.

The purpose of this article is to bring attention to 2 topics of significant importance to the care of pregnant women and women who have recently given birth. Coronavirus disease 2019 and hypertension can have a significant impact on morbidity and mortality for these pregnant and postpartum women. Although women in their childbearing years are for the most part young and healthy, it is critically important to recognize that these patients have more stringent guidelines for vital signs and laboratory values, which should be noticed as abnormal for rapid treatment as necessary.

Elevated serum potassium values: the role of preanalytic variables.

Potassium is one of the most frequently tested analytes in the clinical laboratory. Because of its critical role in body homeostasis, laboratory errors that cause inaccurate potassium results can significantly affect patient safety. This review is limited to the spurious increase of serum potassium levels (pseudohyperkalemia), ie, instances of elevated potassium results that cannot be explained clinically and do not correspond to the status of the patient; when these specimens are recollected, potassium values usually drop substantially without clinical intervention. Because the workup of fals

Standardization of scanning electron microscopy analysis of fibrin fiber diameter measurement: communication from the ISTH SSC subcommittee on FXIII and fibrinogen.

BACKGROUND: The microscopic characteristics of the fibrin clot structure, such as fibrin fiber length and diameter, network fiber density, and pore size, can be correlated with mechanical properties, permeability, susceptibility to lysis, and diseases. Several of these properties are commonly assessed using scanning electron microscopy (SEM); however, there are currently no standardized protocols on clot preparation for fiber diameter determination using SEM. This has led to a large discrepancy in values reported for healthy individuals and precludes interlaboratory comparisons. OBJECTIVES: T

Critical diagnoses in surgical pathology: a retrospective single-institution study to monitor guidelines for communication of urgent results.

Recent attention has shifted toward defining critical values in surgical pathology, as used in clinical pathology for urgent laboratory results, which require immediate physician notification. The Association of Directors of Anatomic and Surgical Pathology recently proposed a schema for critical values in surgical pathology, better defined as critical diagnoses (CDs). To this end, our department established guidelines defining the timely communication and documentation of urgent findings. To monitor policy effectiveness and to refine a customized list of CDs, we analyzed reports over two ident

Smartphone-based point-of-care analyzer for rapid hemoglobin and potassium monitoring in end-stage renal disease and perioperative renal transplantation patients.

This study aimed to develop a smartphone-based analyzer capable of detecting hemoglobin and potassium levels, with clinical validation using a perioperative renal transplant cohort. We implemented azide-methemoglobin and potassium-tetraphenylborate reactions to detect hemoglobin and potassium levels respectively. A smartphone-based system using ambient light sensors and a Phyphox application was used to measure the light intensity changes, with concentrations calculated via calibration curves. 461 hemoglobin measurements and 481 serum potassium test results from 133 patients with end-stage ren

The effect of hemodialysis on electrolytes and acid-base parameters.

BACKGROUND: Hemodialysis patients are treated with bicarbonate dialysate to correct the metabolic acidosis, which results from the metabolism of dietary and endogenous protein. The concentration of plasma tCO(2) is used to gauge the success of therapy. Reported low values in pre-dialysis blood suggest incomplete correction of acidosis in a substantial percent of the dialysis population. However, questions have been raised about the reliability of tCO(2) determination in dialysis patients. METHODS: Pre- and post-dialysis blood specimens were obtained from chronic hemodialysis patients and anal

Validating CALIPER pediatric reference intervals in a U.S. population using retrospective outpatient data and RefineR.

BACKGROUND: Accurate pediatric reference intervals (RIs) are critical for proper interpretation of laboratory results, yet their development is challenged by age-related physiological variation, limited sample sizes, and preanalytical variability. This study evaluates the applicability of the Canadian Laboratory Initiative on Pediatric Reference Intervals (CALIPER) RIs to a US-based, diverse pediatric outpatient population using both the RefineR inverse modeling approach and the Clinical and Laboratory Standards Institute (CLSI) EP28-A3c recommended approach. Comparing different viral strains in identifying risk factors for the development of venous thromboembolism in hospitalized COVID-19 patients.

Since the onset of the coronavirus disease 2019 (COVID-19) pandemic, there have been nearly seven million deaths associated with COVID-19 and over thirteen billion total vaccine doses administered worldwide. This study aimed to evaluate the patient demographics and laboratory values that might help predict individuals at risk of having a deep vein thrombosis (DVT) formation while hospitalized with COVID-19. Focusing on the four major strains alongside the associated vaccination availability periods in the United States to devise risk stratification and management algorithms. This retrospective

Development and implementation of a scalable and versatile test for COVID-19 diagnostics in rural communities.

Rapid and widespread testing of severe acute respiratory coronavirus 2 (SARS-CoV-2) is essential for an effective public health response aimed at containing and mitigating the coronavirus disease 2019 (COVID-19) pandemic. Successful health policy implementation relies on early identification of infected individuals and extensive contact tracing. However, rural communities, where resources for testing are sparse or simply absent, face distinctive challenges to achieving this success. Accordingly, we report the development of an academic, public land grant University laboratory-based detection a

Coagulopathy in newborns with hypoxic ischemic encephalopathy (HIE) treated with therapeutic hypothermia: a retrospective case-control study.

BACKGROUND: Newborns with hypoxic ischemic encephalopathy (HIE) are at risk for coagulopathy due to systemic oxygen deprivation. Additionally, therapeutic hypothermia (TH) slows enzymatic activity of the coagulation cascade, leading to constitutive prolongation of routinely assessed coagulation studies. The level of laboratory abnormality that predicts bleeding is unclear, leading to varying transfusion therapy practices. METHODS: HIE infants treated with TH between 2008-2012 were included in this retrospective study. Initial, minimum (min) and maximum (max) values of International Normalized

Early diagnosis pathway for laboratory testing in the context of pertussis resurgence.

The resurgence of pertussis continues to be a global public health concern, especially in China in recent years. It is critical to identify atypical cases in adolescents and adults and to improve sensitivity to early laboratory diagnosis. The subjects were laboratory-confirmed cases in Chaoyang district between 2018 and 2023, detected through pertussis symptom surveillance, or reported in the National Notifiable Disease Reporting System. Logistic regression and decision tree models were used to examine potential factors influencing laboratory diagnostic sensitivity. Among the 446 laboratory-co

Reproducibility in Studies on Dental Restorative Materials: A Critical Review.

The evaluation of dental restorative composites relies largely on laboratory studies performed under controlled conditions using standardized testing protocols. However, despite the apparent simplicity and standardization of many commonly used methods, published studies often report surprisingly heterogeneous results. This critical review examines the issue of reproducibility through three representative laboratory tests that differ in their apparent methodological complexity: bond strength, degree of conversion, and flexural strength. Bond strength testing is well recognized for its limited r

Radial haemolysis in gel used in serological studies on Venezuelan equine encephalomyelitis virus.

Radial haemolysis in gel was successfully used for the detection of antibody in convalescents after laboratory infection with Venezuelan equine encephalomyelitis (VEE) virus at remote intervals after infection (up to 23 years). The antibodies reacted only with VEE antigen but not with Sindbis and Chikungunya alphavirus antigens. Radial haemolysis in gel was sensitive as the haemagglutination inhibition test but the antibody could be titrated by the diameter of the haemolysis zone omitting the serum dilutions. Slight deviations in the virus dose used for sensitization of erythrocytes or in pH v

Toxicodynamic approach to management of the poisoned patient.

We recommend a toxicodynamic approach to the management of the poisoned patient. We define the period between ingestion and onset of toxic manifestations (clinical or laboratory) as the preclinical phase, during which the management of the patient necessarily depends solely on the history of ingestion and the predicted toxicity. In the toxic phase during which the patient shows clinical or laboratory evidence of toxicity, the history, clinical status (signs, symptoms, drug levels, laboratory parameters), and toxicodynamics should guide the therapy. In the resolution phase, when the patient sho

Analytical Validation of an ELISA Assay for Maternal Autoantibody-Related Autism.

Background/Objectives: Genetic and environmental factors during early development contribute to autism pathogenesis. Maternal autoantibodies recognizing specific fetal brain proteins can predict autism risk in a subset of cases. These antibodies cross the placenta and bind to their target antigens, which play critical roles in neurodevelopment, thereby increasing autism risk in this mechanistically defined subtype, Maternal Autoantibody-Related Autism (MARA). A multi-ELISA assay that detects maternal autoantibody combinations associated with increased autism risk has been described in the lite
