Intraoperative monitoring of the inferior alveolar nerve during mandibular sagittal-split osteotomy.

In order to evaluate the risk of nerve injury and to prevent iatrogenic damage at different stages of bilateral sagittal-split osteotomy (BSSO) of the mandible, we monitored the function of the inferior alveolar nerve (IAN) continuously on both sides in 13 orthognathic patients undergoing BSSO. The IAN was stimulated at the mental foramen with two monopolar needle electrodes fixed to the dental splint, and the orthodromic sensory nerve action potentials (SNAP) of the IAN were recorded with a silver-wire electrode inserted near the oval foramen on each side. The latencies, amplitudes, and senso

[Neurologic examinations for facial nerve damage in surgically treated mandibular collum fractures].

Transcranial magnetic stimulation is a new method for objective evaluation of the facial nerve without constraints for the patient. By the first time, especially cortical lesions can be detected objectively. Using this method, a permanent disturbance of the ramus marginalis mandibularis after operative treatment of fractures of the mandibular condyle could be certainly excluded. Therefore, because of the clear presentation and the possibility of enlargement, the deep submandibular approach, using a correct operative technique, can be strictly recommended.

Continuous Mandibular Nerve Block for Intractable Mandibular Pain Due to Antiresorptive Agent-Related Osteonecrosis of the Jaw: A Case Report.

Antiresorptive agent-related osteonecrosis of the jaw (ARONJ) causes severe pain that cannot be controlled well by common analgesic drugs. This is a first case report of intractable mandibular pain due to ARONJ that was eliminated by a continuous mandibular nerve block. A 72-year-old woman with osteoporosis had been administered bisphosphonate. One year after extraction of her tooth, she was diagnosed as having ARONJ. Jaw pain was so severe that she was unable to open her mouth and eat. We performed a continuous mandibular nerve block through an indwelling catheter with levobupivacaine for pai

Anatomic relationship between impacted third mandibular molar and the mandibular canal as the risk factor of inferior alveolar nerve injury.

Our aim was to explore the relation between the site of the mandibular canal and neurosensory impairment after extraction of impacted mandibular third molars. We organised a retrospective study of 537 extractions in 318 patients in which the affected tooth was intersected by the mandibular canal. This was verified by cone-beam computed tomography (CBCT), and we analysed the relation between the site of the canal and the likelihood of injury to the inferior alveolar nerve (IAN) after extraction of the third molar. The relation between the position of the root of the tooth and the mandibular can

Marginal mandibular branch of the facial nerve - anatomical peculiarities and clinical aspects.

BACKGROUND: The growing number of parotid ablations, of reconstructive and cosmetic surgeries of the head and neck has considerably increased the risk of marginal mandibular branch (MMB) injury. Our study aimed to determine the anatomical peculiarities of the MMB in terms of the facial nerve branching pattern (FNBP), gender and cephalometric type. MATERIALS AND METHODS: The MMB was dissected on 75 hemiheads of adult embalmed cadavers. The origin, number of branches, topography and connections of the MMB were analysed. RESULTS: Seven FNBPs were identified: Type I (18.7%); Type II (14.7%); Typ

Mental nerve shielding from possible injury during mandibular surgical procedures: technical note.

Injuries to the mental nerve are not an uncommon complication in maxillofacial surgeries. Manipulation close to the mental nerve poses a great risk of nerve injury from drills, bone cutting and trimming burs, and oscillating/reciprocating saws. Nerve injuries can be painful and affect the patient's quality of life. The accompanying complication of the nerve injury depends on the severity of the damage inflicted and can range from transient hypoesthesia to neuropathic pain or trigeminal neuralgia. It is considered that direct injury to the nerve may lead to permanent damage and more severe post

Intravenous cannula used for continuous intraoral mandibular nerve block in trigeminal neuralgia.

Trigeminal neuralgia is a chronic pain condition affecting one or more distributions of the trigeminal nerve. Patients with this condition experience short, sharp, shooting pain attacks, which can progress to longer, more frequent durations. The pain is often difficult to control. We report of a man who was admitted with severe neuralgia of the third division of the trigeminal nerve. Talking and any oral intake triggered a severe agonising pain. The latter made the regular oral intake of analgesia challenging. The pain was temporarily controlled with frequent local anaesthesia (LA). Dental cor

Systematic review of lingual nerve retraction during surgical mandibular third molar extractions.

The lingual nerve is at risk of damage during mandibular third molar (M3M) surgery. Current trends to avoid lingual retraction and prevent injury to the nerve run counter to the surgical principles of locating and protecting important structures. To our knowledge, the last review of this subject only considered repurposed lingual retractors such as the Howarth. Other studies have reported the use of purpose-built retractors with improved outcomes for lingual nerve injuries. This systematic review aims to assess the current view regarding lingual nerve retraction in M3M surgery. A search of Pub

Periodontal ligament and intraosseous anesthetic injection techniques: alternatives to mandibular nerve blocks.

BACKGROUND: and Overview. The provision of mandibular anesthesia traditionally has relied on nerve block anesthetic techniques such as the Halsted, the Gow-Gates and the Akinosi-Vazirani methods. The authors present two alternative techniques to provide local anesthesia in mandibular teeth: the periodontal ligament (PDL) injection and the intraosseous (IO) injection. The authors also present indications for and complications associated with these techniques. CONCLUSIONS: The PDL injection and the IO injection are effective anesthetic techniques for managing nerve block failures and for provid

Incidence and risk factors for nerve injuries in mandibular third molar surgery.

BACKGROUND: The inferioralveolar (IAN), lingual (LN) and long buccal nerves (LBN)are the three terminal branches of the trigeminal nerve which are susceptible to injury during surgical extraction of impacted mandibular third molars. While it is not always possible to accurately predict the patients that will be affected with these complications, understanding and identifying the risk factors may allow the adoption of appropriate technique and expertise for specific cases. We embarked on this study to document the incidence and duration of injury to the inferior alveolar nerve (IAN), lingual ne

[Evaluation of sensory deficit of the inferior alveolar nerve after mandibular osteotomy].

INTRODUCTION: Injury of the inferior alveolar nerve (IAN) is one of the most common complications of mandibular osteotomy. The wide differences in results published to date led us to evaluate the incidence of sensorial disorders persisting for one year in a series of 40 patients. MATERIAL AND METHOD: This study included 40 patients aged 14-58 years. We studied subjective expression of sensorial disorders using a questionnaire with items for the different orofacial functions related to the IAN. We studied the clinical expression of these sensorial disorders by testing the three modes of extero

Depressor labii inferioris resection: an effective treatment for marginal mandibular nerve paralysis.

In patients who show their lower teeth during smiling and facial animation, paralysis of the marginal mandibular nerve (MMN) causes a noticeable asymmetry of the lower lip due to the absence of depressor function. This paper presents a balancing technique for this lower lip asymmetry that involves resection of the depressor labii inferioris (DLI) on the nonparalysed side. The anatomy of the muscle, the operative technique, and the effectiveness of the procedure are outlined. A retrospective chart review was performed for 42 adult patients who were treated for MMN palsy with a DLI resection. Se
