BACKGROUND: Mandibular reconstruction is a critical procedure in the management of defects caused by trauma, tumors, or congenital anomalies. Traditional methods often face challenges such as poor fit, extended surgery time, and postoperative complications. The advent of 3D printing technology allows for the precise fabrication of titanium implants tailored to individual anatomy. This study evaluates the clinical outcomes of using 3D-printed titanium implants in mandibular reconstruction.
MATERIALS AND METHODS: This prospective study involved 30 patients (mean age: 45 ± 12 years) with mandibu

BACKGROUND: Comminuted fractures of the jaws are complex injuries requiring special attention. In the past, treatment included closed reduction using maxillomandibular fixation. With advancements in technology and fixation systems, open reduction became a prevalent option. These fractures are difficult to reconstruct during the primary treatment phase, thus resulting in higher complication rates. The introduction of three-dimensional (3D) planning and printing brought about superior outcomes, yet these focus on secondary reconstruction due to the need for outsourcing planning and titanium prin

The aim of this study was to design for mandibular reconstruction of large lateral defect with minimum target reliability with designated confidence interval under bite force range of 300 ± 102 N. The performance of the models has been evaluated by numerical analysis considering the uncertainty of input parameters. Computer-Aided design was used to develop the models of three designs according to the patient's anatomy and to achieve to near symmetry of the mandible. Stress-strength modeling was utilized for the probabilistic physics of failure analysis under assumption of a quasi-static load.

Patients with end-stage temporomandibular joint (TMJ) pathology present with loss of vertical ramus height with resulting retrognathia, anterior open bite, and restricted jaw function requiring joint reconstruction. Costochondral grafts, long considered the gold standard for TMJ reconstruction, carry risks of bony ankylosis and resorption. Custom-made alloplastic TMJ prostheses show excellent objective and subjective outcomes but are also associated with complications such as infection or metal sensitivity, and necessitate hardware explantation in some cases. Revision surgeries are rare but pr

Free-flap mandibular reconstruction is a highly specialized procedure associated with severe complications necessitating re-interventions and re-hospitalizations. This surgery is expensive in terms of health workers' time, equipment, medical devices and drugs. Our main objective was to assess the direct hospital cost generated by osseocutaneous free-flap surgery in a multicentric prospective micro-costing study. Direct medical costs evaluated from a hospital perspective were assessed using a micro-costing method from the first consultation with the surgeon until the patient returns home, thus

The majority of the paediatric oral and maxillofacial tumours are benign and the mandible is involved in one-third of these cases. A review of the literature reveals only a handful of studies pertaining exclusively to benign paediatric mandibular tumours. The basis of this study was to fulfil the need to assess the suitability of major mandibular reconstructions using a vascularised fibular graft in cases of benign tumours in children. From April 1999 to April 2011 we have managed 18 cases of benign paediatric mandibular tumours. All the reconstructions were done using vascularised fibular gra

BACKGROUND: "Bare bone graft" with a vascularised iliac crest is a procedure involving no soft tissue for intraoral lining, and the intraoral defect is covered with epithelial cells through secondary healing of the exposed bone.
METHODS: A vascularised iliac crest flap is transferred to a segmental mandibular defect intraorally in the position of the osteotomized stump upwardly. Granulation tissue is usually observed on the stump of the bone graft about 1 week after surgery. When sufficient granulation is observed after approximately 4 weeks, "resurfacing" is performed to prevent interference

BACKGROUND: This study focused on using the mandibular outer cortex in the reconstruction of old orbital wall fractures.
METHODS: Old orbital wall fractures of 46 patients were reconstructed with the mandibular outer cortex. The surgical results were evaluated by performing clinical follow-up and by measuring the changes in the orbital volume. Donor-site complications were also evaluated.
RESULTS: All patients had good aesthetic and functional results, with an accurate repair of orbital wall defects and the restoration of orbital volume. There were no obvious major donor-site complications.

Goldenhar syndrome is a well-known developmental anomaly of the maxillofacial skeleton and hemifacial soft tissue. Maxillofacial anomalies of that syndrome can be managed by a variety of means such as inlay- or onlay-applied nonvascularized bone grafts, vascularized osteocutaneous/osseous flaps, or distraction osteogenesis. Vascularized full-thickness calvarial bone grafting is an important option for mandibular reconstruction for cases in which, for one reason or another, other techniques are not available, not applicable, or have failed. A mandibular defect of a 6-year-old boy presenting wit

The free osteofasciocutaneous fibular transplant is a direct development of the free fibular bone graft used from 1983 in reconstructive surgery of the long bones. In 1989, Hidalgo was the first to publish a method of reconstruction of the mandible by composite free fibular transplants. The aim of this study was to specify the anatomy of the osteofascio-cutaneous fibular graft and particularly of the vessels to the skin. The sensory innervation of this transplant is described.

To reconstruct a composite mandibular defect, we have simultaneously transferred a vascularized bone graft or osteocutaneous flap together with a forearm flap. The radial forearm flap, being thin, pliable, and having a long vascular stalk, served as mucosal lining and/or an interpositional flap acting as a vascular bridge. Between 1982 and 1989, we used this procedure in 17 patients with a mandibular defect or deformity which developed following treatment of oral cancer. Our clinical experience has demonstrated that this dual free tissue transfer has many advantages. It is useful for obtaining

Since 1978, 35 patients have undergone mandibular reconstruction with vascularized iliac crest. During this time, the technique of raising and shaping the iliac crest has undergone a series of modifications. Initially, osteocutaneous segments based first on the superficial circumflex iliac system and later on the deep circumflex iliac system were used. More recently, only the inner table of the ilium has been employed, and where intraoral lining is required, an ulnar forearm free flap has been added. Thirty-two patients were reconstructed successfully. Of the three anastomotic failures, one bo

Autotransplant bone is superior to other bone, cartilage, or alloplastic materials for the reconstruction of orbital floor defects. The bone used for filling orbital floor defects is taken from the symphysis of the lower jaw. The piece of bone, up to 3.5 X 1.5 cm in diameter is strong enough to support the orbital contents, while at the same time it is easily modelled and positioned. This method is indicated for reconstruction of the orbital floor, with or without rim defects. Taking of the transplant, while retaining the lower margin of the mandibular symphysis, preserves the contour of the c

BACKGROUND: Early postoperative complications after microvascular mandibular reconstruction impact outcomes in patients with advanced oral cancer. This study evaluates three reconstruction techniques: vascularized bone graft (VBG), mandibular reconstruction plate (MRP) with a non-osseous flap, and soft tissue flap (STF), using the Comprehensive Complication Index (CCI).
METHODS: In this retrospective study of 101 patients, early postoperative complications (≤ 30 days) were assessed using the CCI, calculated from Clavien-Dindo classification grades. Differences among the three techniques were

OBJECTIVE: To report a patient of infected mandibular fracture nonunion managed using the Masquelet induced membrane technique (MIMT), followed by successful implant-supported dental rehabilitation.
METHODS: A 3-stage surgical protocol was implemented. The first stage consisted of radical debridement of infected and necrotic tissues, removal of failed internal fixation devices, stabilization with a reconstruction plate, and implantation of a polymethyl methacrylate bone cement spacer to induce membrane formation. In the second stage, the cement spacer was removed, and the resulting mandibular
