In our department, we studied 243 patients who underwent open or close tympanoplasty, from 1984 to 1989, generally caused by cholesteatoma (229 cases) or by non-cholesteatomatous chronic suppurative otitis media (14 cases). Extrusion of the prosthesis occurred in 15 (11.11%) of a total of 136 cases after an average time of 12.46 months during a 4 to 8 years evolution period. From these cases 5 out of 59 were made with plastipore Porp (8.47%) and 10 out of 76 with plastipore Torp (13.15%). autologous ossicles did not extrude in any case. In the cases of cholesteatoma, open (62.7%) and close (37

The presigmoid approach (PSA) is selected to obtain more lateral access to cerebellopontine angle tumors, brainstem cavernous malformations, or vertebrobasilar artery aneurysms than the standard retrosigmoid approach. However, mastoidectomy for the PSA can be considered time-consuming and to carry a higher risk of complications due to the anatomical complexity of the region. The authors established a method of minimized mastoidectomy focused on exposing Trautmann's triangle as the corridor for the PSA while maximizing procedural simplicity and safety and maintaining a sufficient operative view

BACKGROUND: In order to remove a cholesteatoma in the mastoid under transcanal endoscopic ear surgery, it is necessary to perform transcanal endoscopic mastoidectomy. Bone dust and blood, however, obscure the surgical field. A novel endoscopic hydro-mastoidectomy technique was developed, in which the operator performs the mastoidectomy 'underwater' using a lens cleaning system that provides saline perfusion in the surgical space.
METHODS: A curved round coarse diamond bur is attached to an otological drill. A lens cleaning sheath is fitted to the endoscope. The surgeon controls the infusion o

BACKGROUND: Canal wall down (CWD) mastoidectomy has many drawbacks including chronic otorrhea, granulations, dizziness on exposure to cold or hot water and tendency of debris accumulation in the mastoid cavity demanding periodic cleaning. Many of these problems can be solved by reconstruction of the posterior meatal wall (PMW).
OBJECTIVES: To assess the results of PMW reconstruction after CWD mastoidectomy for cholesteatoma using bioactive glass (BAG) that is fabricated and built up intraoperatively.
PATIENTS AND METHODS: This study was applied on 20 patients had atticoantral chronic suppura

A canal wall down mastoidectomy (CWDM) is an effective technique for eradication of advanced chronic otitis media or cholesteatomas. A retrospective study was conducted at a Malaysian Tertiary Medical Center between June 1996 to December 2003 to evaluate the outcome of Modified Radical Mastoidectomy(MRM), a form of CWDM for patients with chronic active otitis media (OM) with cholesteatoma, chronic mastoiditis or chronic active OM with cholesteatoma and mastoiditis. All new cases of MRM which fulfilled the selection criteria were reviewed. The main outcome measures were the hearing outcome and

Simulation-based training plays a vital role in otolaryngology education, allowing learners to safely and effectively develop fundamental microsurgical skills. However, limited access to cadaveric dissection and high-fidelity temporal bone models due to cost and availability continues to present significant barriers to training in essential otologic techniques such as cortical mastoidectomy. This technical report outlines the design and construction of an accessible, low-cost mastoidectomy trainer intended to support early surgical education. The model is assembled from easily obtainable mater

Middle ear paragangliomas (glomus tympanicum) are rare vascular tumors that pose significant surgical challenges due to their location and vascularity. The inside-outside mastoidectomy combines transcanal and retroauricular approaches to optimize resection and preserve hearing. Methods: We conducted a retrospective observational study of eight female patients with stage A1-B3 glomus tympanicum treated at Carlos Andrade Marín Specialty Hospital between 2018 and 2024. Clinical presentation, imaging, intraoperative findings, complications, and audiological outcomes were analyzed. Results: All p

The objective of this study was to evaluate, over the long-term, the anatomic and functional outcome of canal wall-down mastoidectomy performed for chronic otitis media with cholesteatoma and chronic otomastoiditis resistant to all conservative treatment. The study was made through a retrospective review of 338 cases of consecutive primary canal wall-down mastoidectomies performed between 1974 and 1998. Included were 259 cases with sufficient data. In all cases, functional reconstruction was performed at the same time as the mastoidectomy. Demographic and clinical data were collected from each

OBJECTIVE: We describe the technique of cosmetic reconstruction after mastoidectomy in the transpetrosal-presigmoid approach for petroclival lesions.
TECHNIQUES: This technique involves raising a single temporo-occipital bone flap and cosmetic mastoidectomy, removing the mastoid bone fragments (by means of a rongeur instead of an air drill) for later reconstruction. Replacement of the bone fragments mixed with fibrin glue enables tamponade against the dura to be achieved without a fat graft.
RESULTS: Computed tomographic scans and plain cranial films taken a few years later showed successful

The middle ear is a cavity surrounded by solid bone, lined with mucosa with a lumen filled with gas. To examine this unique ventilation system under atmospheric pressure, the middle ear pressure of 50 normal ears was previously examined. Positional tympanometry, whereby the middle ear pressure is increased as the subject assumes the lateral position under atmospheric pressure was used. As a result, (1) The middle ear pressure was elevated by the change from the sitting position to the lateral position. Venous pressure was regarded as causative factor of this pressure elevation. (2) The elevati
