Complications after Metatarsal Osteotomies for Hallux Valgus: Malunion, Nonunion, Avascular Necrosis, and Metatarsophalangeal Osteoarthritis.

Avascular necrosis (AVN), nonunion, malunion, and metatarsophalangeal (MTP) osteoarthritis following hallux valgus osteotomies, as well as pathophysiology, diagnosis, prevention strategies, and treatment are discussed in this article. AVN and nonunion are very infrequent, and they can be effectively prevented taking into consideration local anatomy preservation, biomechanics, and patient comorbidities. Shortening, elevation, plantarflexion, varus/valgus, and rotational of the first metatarsal are the most common types of malunion. They can lead to pain, stiffness, deformity recurrence, and tra

Hallux valgus in a historical French population: paleopathological study of 605 first metatarsal bones.

OBJECTIVES: To estimate the prevalence of hallux valgus in a historical population in France, to identify associated skeletal abnormalities, and to look for an influence of footwear changes through time. METHODS: We studied the 605 first metatarsals found in the necropolis of the Notre-Dame-du-Bourg cathedral in Digne in the Alpes-de-Hautes-Provence region of southern France. The necropolis contains remains from the 5th to the 17th century. RESULTS: Hallux valgus was identified only in individuals older than 30years at death, and its prevalence increased significantly with age. Exostosis at

Juvenile hallux valgus deformity: surgical management by lateral hemiepiphyseodesis of the great toe metatarsal.

Surgical correction of juvenile hallux valgus (JHV) by soft tissue balancing or skeletal realignment is associated with a high rate of recurrence of the deformity. An alternative treatment strategy for the management of symptomatic or progressive JHV, consisting of lateral hemiepiphyseodesis of the great toe metatarsal physis, has been used at our institution since 1996. A review of these cases was performed to determine the outcomes in the technical and patient satisfaction domains. Preoperative and follow-up radiographs of the foot were analyzed to measure the intermetatarsal angle (IMA), th

Hallux valgus correction with metatarsal osteotomy: effect of a lateral distal soft tissue procedure.

The purpose of this study was to compare the results of hallux valgus correction using a proximal metatarsal osteotomy with or without a lateral distal metatarsophalangeal soft tissue procedure. This procedure included release of the lateral joint capsule and transfer of the adductor tendon into the neck of the metatarsal. Two groups of patients were reviewed retrospectively. In the first group (17 patients and 25 feet), a proximal closing wedge osteotomy and an Akin osteotomy were performed. In the second group (22 patients and 31 feet), in addition to the osteotomies, the adductor tendon and

Relationship Between the Relative Length of the Second Metatarsal and Occurrence of Metatarsalgia in Patients With Hallux Valgus.

BACKGROUND: Patients with hallux valgus (HV) may develop metatarsalgia, which is partly attributed to second metatarsal relative length (RL2M). However, no study has analyzed RL2Ms measured by various methods as predictors for metatarsalgia in HV patients. This study aimed to investigate the predictors for metatarsalgia in HV patients and calculate the cutoff values for metatarsalgia in preoperative planning for lesser metatarsal shortening osteotomy. METHODS: In this retrospective cohort study, 103 female patients (131 feet) with HV were investigated for metatarsalgia (metatarsalgia-positive

Correction of Severe Hallux Valgus with Metatarsal Adductus Applying the Concepts of Minimally Invasive Surgery.

The combination of hallux valgus and metatarsus adductus presents a surgical challenge even for the experienced foot and ankle surgeon, as the position of the lesser metatarsals restricts the space for metatarsal head displacement. We describe the application of concepts of minimally invasive techniques to correct this deformity. Proximal metatarsal osteotomy to correct the position of lesser metatarsals, followed by minimally invasive bunion surgery, shows promising results. In a short series, proximal metatarsal osteotomy showed excellent correction of the deformity. At final follow-up, all

Geometric analysis of indications for minimally invasive distal metatarsal osteotomy in treatment of hallux valgus.

BACKGROUND: Minimally invasive distal metatarsal osteotomy (MIDMO) is to be indicated for all patients with angles of IMA <20° and HV <40°, but many authors doubt whether this procedure is capable of correcting all types of hallux valgus deformities. The aims of this study were to perform a geometric analysis of MIDMO indications and to show which preoperative radiological parameters are necessary to achieve sufficient contact between bone fragments and sufficient correction with this operative technique. METHODS: A geometric mathematical model in AP and lateral radiographic plane was created

Articular chondral damage of the first metatarsal head and sesamoids: analysis of cadaver hallux valgus.

BACKGROUND: Evidence of successful correction on postoperative hallux valgus imaging studies may not always correlate with patient satisfaction. Recent attention to the association of cartilaginous degeneration and hallux valgus may provide new insight into treatment algorithms and patient expectations. The purpose of this cadaveric study was to evaluate the degree of chondral damage as it relates to increasing hallux valgus deformity. METHODS: A total of 39 cadaver first metatarsophalangeal joints were evaluated by radiography, and then dissected to evaluate for chondral damage. Chondral les

The shape of the lateral edge of the first metatarsal head as a risk factor for recurrence of hallux valgus.

BACKGROUND: The relationship between the shape of the first metatarsal head and hallux valgus deformity remains controversial. The purpose of the present study was to retrospectively analyze differences in the radiographic appearance of the shape of the lateral edge of the first metatarsal head between women with normal feet and those with hallux valgus and to clarify the relationship between the shape of the lateral edge and the postoperative recurrence of hallux valgus deformity. METHODS: Dorsoplantar weight-bearing radiographs of sixty normal feet in women (the control group) and sixty fee

Stress fractures of the lesser metatarsals after a Wilson osteotomy for correction of hallux valgus deformity.

This article describes a patient with lesser-metatarsal stress fractures resulting from an oblique Wilson displacement first metatarsal osteotomy. The shortening of the first metatarsal forces the lesser metatarsals to bear the weight previously borne by the first ray and increases the compression stress on the adjacent metatarsal heads. The proximal displacement of the osteotomy must be minimized in order to limit the risk of stress fracture of the lesser metatarsals.

Comparative study between Wilson and Mitchell metatarsal osteotomies for the treatment of hallux valgus in adults.

Although Mitchell osteotomy and Wilson osteotomy are two popular methods for the treatment of hallux valgus, there are no studies directly comparing their results. Fifty-six patients (73 feet) who underwent a Wilson osteotomy and 30 patients (34 feet) who had a Mitchell osteotomy were followed for a mean period of 33.7 and 38.9 months, respectively. The results were comparable in terms of hallux valgus angle correction and first intermetatarsal angle correction, although symptomatic improvement was higher in the Mitchell group. Moreover, the incidence of postoperative metatarsalgia was signifi

Metatarsal Pronation on Radiographs: A Prospective Reliability Study of Visual Rotation Markers in Hallux Valgus.

BACKGROUND: Rotational malalignment of the first metatarsal is increasingly recognized as a key feature of hallux valgus deformity, but the reliability of radiographic rotation markers remains uncertain. This study assessed the inter- and intraobserver reliability of 4 commonly used radiographic parameters: metatarsal pronation angle (MPA), tibial sesamoid position (TSP), lateral head shape (LHS), and round head sign (RH). METHODS: In this prospective reliability study, 3 senior clinicians independently evaluated weightbearing anteroposterior and axial sesamoid radiographs of 75 hallux valgus

Relationship of Dome Height of the First Metatarsal Head with Hallux Valgus Angle and Metatarsophalangeal Alignment.

OBJECTIVES: Hallux valgus (HV) is a progressive foot deformity in which the first metatarsophalangeal (MTP) joint is affected. The relationship between the dome height of the first metatarsal head and the HV deformity has not been studied previously. This study aimed to investigate a possible relation of the dome height of the first metatarsal head with articular alignment and the hallux valgus angle (HVA), which is frequently used to evaluate HV. METHODS: A total of 129 feet of 68 patients were included in the study. Anteroposterior digital radiographic images of the foot taken in a weightbe

Shortening Proximal Chevron Metatarsal Osteotomy for Patients With a Hallux Valgus Deformity With Advanced Arthritis.

To correct hallux valgus deformities in patients with advanced arthritis of the first metatarsophalangeal joint, we designed a new reverse chevron-type shortening osteotomy technique that could be used to correct valgus deformities at the proximal metatarsal level, as well as shorten and lower the metatarsal, in a 1-time procedure. Sixteen feet in 16 patients with a minimum of 18 months follow-up who underwent a shortening proximal chevron metatarsal osteotomy for a hallux valgus deformity with advanced arthritic change between January 2014 and March 2016 were reviewed in this study. Tensile Properties of the Deep Transverse Metatarsal Ligament in Hallux Valgus: A CONSORT-Compliant Article.

The deep transverse metatarsal ligament (DTML) connects the neighboring2 metatarsal heads and is one of the stabilizers connecting the lateral sesamoid and second metatarsal head. In this study, we aimed to determine the tensile properties of the DTML in normal specimens and to compare these results with hallux valgus specimens. We hypothesized that the tensile properties of the DTML would be different between the 2 groups of specimens. The DTML in the first interspace was dissected from 12 fresh frozen human cadaveric specimens. Six cadavers had bilateral hallux valgus and the other 6 cadavers
