BACKGROUND: The second cervical vertebra (C2, axis) is included in head-and-neck cone-beam computed tomography (CBCT) and may provide a target for forensic sex estimation. Whether odontoid-only segmentation preserves whole-C2 radiomic information remains unclear.
OBJECTIVE: To develop and internally validate paired CBCT radiomics models using whole-C2 and odontoid-only regions of interest (ROIs), and to compare classification performance while describing feature-level reproducibility for each ROI.
METHODS: This retrospective single-device study included 140 adult CBCT examinations (68 female

This study aims to investigate the feasibility and efficacy of anterior atlantoaxial motion preservation fixation (AMPF) in treating axis complex fractures involving the odontoid process fracture and Hangman's fractures with C2/3 instability. A retrospective study was conducted on eight patients who underwent AMPF for axis complex fractures at the General Hospital of Central Theater Command from February 2004 to October 2021. The types of axis injuries, reasons for injuries, surgery time, intraoperative blood loss, spinal cord injury classification (American Spinal Injury Association, ASIA), a

STUDY DESIGN: This is a unique case report.
OBJECTIVES: To describe a successful case of Teriparatide treatment of odontoid nonunion.
SUMMARY OF BACKGROUND DATA: Animal models suggest that this drug may enhance fracture healing, and there are 3 similar cases previously published elsewhere.
METHODS: We describe the evolution of our case over time with serial computed tomography scan imaging confirming the treatment success.
RESULTS: Drug treatment of the odontoid nonunion was successful.
CONCLUSION: Drug treatment of the odontoid nonunion may be an option and merits formal study.

INTRODUCTION: Odontoid process fractures in elderly patients are common and significant for their poor clinical and radiologic prognosis. This controversy debates the role of MRI in odontoid process fractures in elderly patients without a neurological deficit.
CONTROVERSY: Elderly patients generally sustain odontoid process fractures following low-energy mechanisms. Ligamentous structures provide a significant secondary constraint to progressive fracture displacement. The additional contribution of MRI after CT and/or radiographs is controversial.
SUMMARY: Potentially the most justifiable re

Four cases of operated odontoid process fractures associated with a fracture of the posterior arch of the atlas are presented. Three types of surgery were performed: atlas-axis fusion, occipitocervical fusion, and odontoid process screw fixation. Based on a literature review and our experience, the therapeutic management is discussed according to the type of odontoid fracture and the presence of neurological involvement, with a reminder that wiring is not indicated when C1 posterior arch continuity is compromised.

OBJECTIVE: Traumatic posterior atlantoaxial dislocation (TPAD) is uncommon, and related research is very limited. By analyzing the imaging characteristics of the anterior arch-odontoid process-transverse ligament complex in patients, the authors classify the outcomes of TPAD and elucidate its mechanisms of injury.
METHODS: This is a retrospective review of CT and MRI data on patients with TPAD treated at two clinical centers from June 2014 to March 2024. Through analysis and statistics on injuries to the anterior arch, odontoid process, and transverse ligament complex, the authors proposed a

Osteoid osteoma (OO) is a primary benign tumor that accounts for up to 3% of all bone tumors. The cervical spine is less affected by OOs, and very few cases of C2 OOs have been reported in the literature, both in adults and children. Surgery may be required in case of functional torticollis, stiffness, and reduced range of motion (ROM) due to cervical OOs refractory to medical therapy. Several posterior and anterior surgical techniques have been described to remove C2 OOs. In particular, anterior approaches to the cervical spine represent the most used surgical route for treating C2 OOs. We de

Congenital clefts and other malformations of the atlas are incidental findings identified while investigating the cervical spine following trauma. A persistent bifid anterior and posterior arch of the atlas beyond the age of 3-4 years is observed in skeletal dysplasias, Goldenhar syndrome, Conradi syndrome, and Down's syndrome. There is a high incidence of both anterior and posterior spina bifida of the atlas in patients with metabolic disorders, such as Morquio's syndrome [Baraitser and Winter in London dysmorphology database, Oxford University Press, 2005; Torriani, Lourenco in Rev Hosp Clin

Fifty-seven consecutive patients treated surgically for nonunited fractures of the odontoid process were reviewed. All patients presented late, exhibiting neurological deficits subsequent to nonunion. Delay in presentation was between 6 and 120 months (mean 32 months) after the original injury, due to missed diagnosis or inappropriate management. Seven patients who were reduced in traction underwent a Gallie atlantoaxial fusion. In the remaining 50 patients who were unreducible, an occipitocervical arthrodesis was performed. They were followed up for a minimum of 2 years, except one who died f

PURPOSE: Traumatic atlantoaxial dislocation (AAD) is usually associated with fatal high-velocity road traffic accidents (Xu et al. in Medicine (Baltimore) 94:e1768, 2015). There are few reports of survival following posterior AAD without odontoid fracture (Xu et al. 2015; Zhen et al. in Arch Orthop Trauma Surg 131:681-685, 2011; de Carvalho and Swash in Handb Clin Neurol 119:435-448, 2014).
METHOD: We present a previously undescribed case of posterior AAD associated with a C1 Jefferson fracture but no odontoid fracture and bilateral vertebral artery occlusion without neurological deficit.

Congenital spinal fusion of an odontoid process to an atlantal hemiarch is very rare. The unfamiliarity of the medical fraternity with this congenital malformation can easily be mistaken for an acute fracture, chronic infection or inflammatory disease. We present our experience of managing an adult who presented with neck pain after a motor vehicle accident. Radiological investigation revealed congenital fusion of the odontoid process to the atlantal hemiarch. The prevalence, embryology and clinical significance of this anomaly are discussed. As the natural progression of this anomaly is not w

The axis from twenty-two cadavera was removed at autopsy and was sectioned in the sagittal plane to a thickness of one millimeter with use of a surface-stained block-grinding technique. Combined two and three-dimensional analysis included an evaluation of the volume of the trabecular bone, the trabecular interconnection, and the cortical thickness as well as qualitative investigation of the structure of the cancellous bone. The body of the axis, the base of the odontoid process, and the odontoid process were analyzed separately. The base of the odontoid process is a region of least resistance

Injuries to the cardinal spine are quite uncommon in pediatric patients. Nonetheless, the second cervical vertebra are crucial for the human neck's extension and rotation. Surgery is the most typical treatment for patients who have a type II fracture of the cardinal vertebra's odontoid process along with a considerable displacement of the odontoid process. Nonetheless, surgical intervention is characterized by challenges and problems following surgery. For the first time, a youngster who had a type II fracture of the cardinal vertebra's odontoid process and substantial odontoid process displac

BACKGROUND: Traumatic atlantoaxial dislocation combined with locked atlas lateral mass and odontoid process fracture is a complex injury and is extremely rare. We describe the surgical technique by presenting a clinical case study in managing a traumatic lateral atlantoaxial dislocation combined with a locked atlas lateral mass and a type II odontoid fracture (Grauer type IIB).
CASE DESCRIPTION: This is a clinical case study of a 38-year-old female patient who presented with severe neck pain without neurological deficit following a traffic accident. Computed tomography showed a type IIB odont

STUDY DESIGN: Kinematics of the cervical spine was investigated using cone beam computed tomography (CBCT) images combined with three dimensions to three dimensions (3D-3D) registration technology in patients after anterior odontoid screw fixation (AOSF) surgery.
OBJECTIVE: The aim of this study was to investigate in vivo 3D cervical motion characteristics of patients who had undergone AOSF surgeries.
SUMMARY OF BACKGROUND DATA: AOSF surgery is a classic surgical method for the treatment of odontoid fracture, but there are few studies that investigated its effect on in vivo biomechanics of t

BACKGROUND: In type II odontoid fractures with intact transverse ligament as classified by the Anderson and D'Alonzo system, anterior screw fixation offers the best anatomical and functional results. The goal of this study is to review the results of the double screw technique in anterior odontoid surgery using a headless 3.0-mm.-diameter cannulated Herbert screw on patients with odontoid process fractures.
METHODS: From January 2015 through March 2019, 12 cases of acute traumatic type II odontoid fractures were treated with double anterior screw fixation using 3.0-mm Herbert screws, slightly
