Brain and Optic Chiasm Herniation into Sella after Pituitary Tumor Apoplexy.

Brain and optic chiasm herniation has been rarely reported following dopamine agonist treatment for large prolactinomas. We report a case of brain and optical chiasm herniation, secondary to an empty sella due to apoplexy of a prolactinoma, and we focus on the specific presentation of this case. A 32-year-old female presented to a neurologist complaining of headaches. Her past medical history was significant for acute vision loss in both eyes accompanied by right third nerve palsy when she was 16 years old. She does not recall any endocrine or imaging evaluation at that time and she had sponta

Isolated primary central nervous system lymphoma arising from the optic chiasm.

A 58-year-old previously healthy woman rapidly developed progressive bilateral visual loss. Magnetic resonance imaging revealed a bulging appearance of the optic chiasm, with homogeneous enhancement after gadolinium administration, which suggested an optic glioma or inflammatory disease. In the absence of (para)clinical clues for a specific diagnosis despite extensive investigation, a biopsy of one optic nerve was performed, resulting in a diagnosis of non-Hodgkin B-cell lymphoma. There was no evidence of any other ocular or systemic involvement, therefore the conclusion was that this immunoco

Isolated optic nerve, chiasm, and tract involvement in Bing-Neel Syndrome.

Central nervous system infiltration of Waldenström's macroglobulinemia is referred to as Bing-Neel Syndrome. We describe 2 patients whose clinical presentation was due to isolated involvement of the anterior visual pathways. The mechanism of visual failure in Bing-Neel Syndrome may involve both infiltrative and autoimmune processes.

[Functional assessment before and after interventions on the optic chiasm system].

OBJECTIVES: In the course of a tumor in the area of the optic chiasm the optochiasmal system is often exposed to compressive and vascular irritations. This study deals with the functional results (visual acuity and visual field) before and after surgery based on the primary situation and morphology of the tumor in order to give a prognosis on this matter. METHODS: 62 patients (41 female, 21 male) suffering from tumors in the area of the optic chiasm were retrospectively reviewed. The duration of the follow-up was an average of 25.82 months, +/- 20.04. Among the tumors investigated there were

Vascular architecture of the human optic chiasm and bitemporal field defects.

The system of blood supply of the optic chiasma was studied on 85 human fresh bodies by several methods. The optic chiasma is supplied by the Willis' circle, arterioles from which, after forming anastomosis at two levels, enter the chiasma and break into capillaries. The authors found that the median portion of chiasma is a weak point of microcirculation, as demonstrated by rarefied and transverse capillaries. When the blood supply of chiasma is compromised by lesions near the sella turcica, this weak point of microcirculation is apt to be disturbed first and becomes ischemic, causing disorder

Chiasmal apoplexy: haemorrhage from a cavernous malformation in the optic chiasm.

We present a patient who experienced sudden onset of orbital headache, visual loss and bitemporal visual field defect. MRI of the optic chiasm suggested a diagnosis of haemorrhage and hence a vascular malformation. Pterional craniotomy revealed an intrachiasmatic haematoma with a cavernous angioma. The malformation was totally excised and vision improved after surgery. The syndrome of chiasmal apoplexy is discussed.

Chiasmal apoplexy due to ruptured cavernous hemangioma of the optic chiasm.

A 30-year-old obese patient with a rapidly progressing chiasmal syndrome presented with acute bitemporal hemianopia, severe bilateral amblyopia, mental confusion, and lethargy. X-ray films of the skull showed a normal sella turcica and computed tomography scan as well as angiography revealed a vascular mass within the chiasmatic cistern. At operation, via left subfrontal approach, an intrachiasmal hematoma was evacuated and biopsies of the hematoma cavity revealed a cavernous hemangioma. Visual symptoms improved markedly postoperatively. This rare association of chiasmal apoplexy and intrachia

Optic chiasm manual and automated measurements in sub-acute optic neuritis with OCT and MRI correlations.

PURPOSE: The optic chiasm (OC) is a central structure in the visual pathway and can be visualized in conventional MRI, but no consensus regarding its measurement has been defined. We aim to investigate the most reproducible manual approach to OC measurement and to explore associations of OC with optical coherence tomography (OCT) parameters, and automatic brain segmentation (FreeSurfer) in subacute optic neuritis (sON), multiple sclerosis without optic neuritis (MSwoON), and healthy subjects (HS). MATERIALS AND METHODS: We reproduced two previously reported methodologies and implemented a new

Assessment of Optic Pathway Structure and Function in Patients With Compression of the Optic Chiasm: A Correlation With Optical Coherence Tomography.

PURPOSE: The purpose of this study was to investigate correlations between retinal fiber thickness measured by optical coherence tomography (OCT) and anterograde functional and structural differences in the optic pathway of patients with compression of the optic chiasm. Our hypothesis was that loss of visual acuity caused by chronic compressive pathologies may lead to an irreversible decline in vision because of permanent neurodegeneration of the optic radiations and visual cortex. METHODS: Quantitative OCT, functional magnetic resonance imaging (MRI) and diffusion tensor MRI measurements wer

Dolichoectatic arterial compression of the chiasm and optic nerve: a case report.

Dolichoectatic arterial compression of the anterior visual pathway is a rare but recognized condition. The functional impact, however, is still less clear. We describe a case of a 56-year-old male with diminished visual acuity in the left eye and incongruent inferior visual defect in both eyes caused by dolichoectasia due to a congenital hypoplasia of one horizontal segment of the circle of Willis and subsequent arterial compression of the chiasm and left optic nerve.

Glioblastoma multiforme of the optic chiasm: A rare case of common pathology.

BACKGROUND: Malignant optic and chiasmatic gliomas are extremely rare, and are classified pathologically as anaplastic astrocytoma or glioblastoma multiforme (GBM). Approximately 40 cases of optic GBM in adults have been reported in the literature, and only five of them were described to originate from the optic chiasm. CASE DESCRIPTION: An 82-year-old male patient with a past medical history of diabetes mellitus type 2, melanoma, and bladder cancer presented with gradual vision loss of the left eye in a period of 1 month. After neuro-ophthalmological examination, the decision of thither magn

Tumor-like enlargement of the optic chiasm in an infant with Alexander disease.

We report a patient with infantile Alexander disease (AXD) due to the recurrent p.Arg79Cys GFAP mutation. In addition to typical AXD abnormalities, magnetic resonance imaging demonstrated a tumor-like lesion of the optic chiasm suggestive of a glioma. A transient papilloedema appeared during the follow-up and the lesion partially regressed despite a worsening of white matter involvement. Rare radiological and pathological tumor-like lesions have already been reported in AXD patients. This patient confirms that enlargement of the optic chiasm is a rare feature of AXD, possibly linked to abnorma

The optic chiasm and the corpus callosum: their relationship to binocular vision in humans.

The semidecussation of the optic nerve fibers in humans serves to facilitate the motor coordination of the two eyes. The Corpus Callosum in humans serves to facilitate the integration of stimuli from opposite cerebral hemispheres. Stereopsis and motor fusion are two distinct and separate qualities of binocular vision, and each alone can indicate central and/or peripheral binocular vision. Stereopsis is not an indicator for binocular stability.

Congenital aplasia of the optic chiasm and esophageal atresia: a case report.

INTRODUCTION: The complete absence of the chiasm (chiasmal aplasia) is a rare clinical condition. Hypoplasia of the optic nerve and congenital nystagmus are almost invariably associated characteristics. Microphthalmos or anophthalmos are common features in chiasmal aplasia, while central nervous system abnormalities are less frequent. Esophageal atresia can be isolated or syndromic. In syndromic cases, it is frequently associated with cardiac, limb, renal or vertebral malformations and anal atresia. More rarely, esophageal atresia can be part of anophthalmia-esophageal-genital syndrome, which

Wilbrand's knee of the primate optic chiasm is an artefact of monocular enucleation.

PURPOSE: The anterior chiasmal syndrome consists of a temporal hemianopia or complete visual field loss in one eye, plus a superior temporal hemianopia in the other eye. The superior temporal hemianopia in the other eye is thought to result from injury to Wilbrand's Knee of the optic chiasm. Wilbrand's Knee is a loop of decussating fibers which detours into the contralateral optic nerve before entering the optic tract. I studied the organization of fibers in the optic chiasm of monkeys and humans to verify the existence of Wilbrand's Knee and to elucidate further the pattern of visual field lo
