Breathing maneuvers as a coronary vasodilator for myocardial perfusion imaging.

PURPOSE: A combined breathing maneuver of hyperventilation, followed by a long voluntary breathhold leads to coronary vasodilation. We investigated the impact of breathing maneuvers on MR first-pass cardiac perfusion imaging and its potential clinical utility. MATERIALS AND METHODS: We studied 24 healthy volunteers (37 ± 12 years; 62.5% men) on a clinical 3 Tesla MRI system and performed first-pass perfusion MR at rest, during a short breathhold (S-HVBH) following 60 s of hyperventilation, and at the end of a long breathhold (L-HVBH) following the hyperventilation, performed in random order.

Survival prediction in high-grade gliomas using CT perfusion imaging.

Patients with high-grade gliomas usually have heterogeneous response to surgery and chemoirradiation. The objectives of this study were (1) to evaluate serial changes in tumor volume and perfusion imaging parameters and (2) to determine the value of these data in predicting overall survival (OS). Twenty-nine patients with World Health Organization grades III and IV gliomas underwent magnetic resonance (MR) and computed tomography (CT) perfusion examinations before surgery, and 1, 3, 6, 9, and 12 months after radiotherapy. Serial measurements of tumor volumes and perfusion parameters were evalu

Magnetic resonance imaging-estimated placental perfusion in fetal growth assessment.

OBJECTIVE: To evaluate in-vivo placental perfusion fraction, estimated by magnetic resonance imaging (MRI), as a marker of placental function. METHODS: A study population of 35 pregnant women, of whom 13 had pre-eclampsia (PE), were examined at 22-40 weeks' gestation. Within a 24-h period, each woman underwent an MRI diffusion-weighted sequence (from which we calculated the placental perfusion fraction), venous blood sampling and an ultrasound examination including estimation of fetal weight, amniotic fluid index and Doppler velocity measurements. The perfusion fractions in pregnancies with a

Arterial spin labeling magnetic resonance perfusion imaging in cerebral ischemia.

PURPOSE OF REVIEW: Arterial spin labeling (ASL) is a noninvasive magnetic resonance perfusion imaging method for visualizing and quantifying whole-brain perfusion that does not require exogenous contrast agents. The goal of this review article is to explain the principles of ASL perfusion imaging and review the strengths and limitations of different ASL methods. RECENT FINDINGS: There are several different approaches that vary mainly on the basis of the technique that is used to label the inflowing arterial blood. These methods can be used to assess perfusion at brain tissue level or the perf

Improved multislice perfusion imaging with velocity-selective arterial spin labeling.

PURPOSE: To improve the multislice performance of velocity-selective arterial spin labeling (VS-ASL) imaging for cerebral blood flow (CBF) measurement such that it might be routinely applied for clinical applications with whole brain coverage. MATERIALS AND METHODS: VS-ASL was performed with improvements such as timing optimization, stimulated echo removal, and slice profile sharpening. Each improvement was evaluated in volunteers by measuring temporal noise in the CBF measurement. VS-ASL with all these improvements was performed in 20 patients with Moyamoya disease some of whom also underwen

Influence of the arm position in myocardial perfusion imaging acquisition.

OBJECTIVE: despite the technologic advances in myocardial perfusion imaging, we keep using an uncomfortable and sometimes impracticable patient position - supine with arms raised above the head (U). The purpose of this study was to investigate whether perfusion and functional cardiac gated SPECT scan results of acquisition U are equivalent to another position modality: supine with arms down at the sides of the trunk (D). METHODS AND RESULTS: we performed U acquisition and in sequence D acquisition in 120 patients (pts) using a one-day MPI (rest-gated/stress), with 99mTc-sestamibi (370 MBq and

Myocardial perfusion imaging study of CO(2)-induced panic attack.

Chest pain is often seen alongside with panic attacks. Moreover, panic disorder has been suggested as a risk factor for cardiovascular disease and even a trigger for acute coronary syndrome. Patients with coronary artery disease may have myocardial ischemia in response to mental stress, in which panic attack is a strong component, by an increase in coronary vasomotor tone or sympathetic hyperactivity setting off an increase in myocardial oxygen consumption. Indeed, coronary artery spasm was presumed to be present in cases of cardiac ischemia linked to panic disorder. These findings correlating

Large abdominal photopenic area on 99mTc-sestamibi myocardial perfusion imaging.

(99m)Tc-sestamibi myocardial perfusion imaging is frequently performed in conjunction with exercise or pharmacologic stress testing for evaluation of coronary heart disease. Interpretation of these studies includes systematic review of unprocessed rotating projectional images for evaluation of cardiac size as well as the presence of motion or attenuation artifacts. Occasionally, incidental noncardiac findings are detected on review of the projectional images. We report a case of a patient with a history of autosomal dominant polycystic kidney disease who was found to have a large abdominal pho

[Computed tomography perfusion (CTP) imaging in diagnostics of cerebral ischemia].

The routine diagnostic tool of acute ischemic stroke is noncontrast computed tomography (CT). Modern multi-slice CT scanners permit also functional imaging with brain perfusion and CT angiography. Wider adoption of thrombolytic therapy in acute stroke have advanced their application. CTP is fast and widely available. It allows verification of cerebral ischemia, and may potentially assist in determining the extent of the ischemic tissue that still is salvageable with thrombolytic therapy. Major cerebral arteries can also be visualized to detect occlusions or stenosis, which also assists in clin

Myocardial perfusion imaging versus biochemical markers in acute coronary syndromes.

The assessment and appropriate clinical management of patients with acute chest pain and non-diagnostic electrocardiograms remain a continuing clinical problem. Accordingly, there is considerable interest in evaluating new strategies to improve early diagnostic accuracy in patients with possible acute myocardial ischaemia. Cardiac troponins (T and I) and acute rest myocardial perfusion imaging have similar sensitivities for detecting acute myocardial infarction. Whereas cardiac markers require 6-12 h to become positive, acute rest myocardial perfusion imaging immediately reflects the status of

Cerebral perfusion imaging evaluates pharmacologic treatment of unilateral moyamoya disease.

Unilateral Moyamoya disease presents as unilateral stenosis or obstruction of the supraclinoid internal carotid artery, which causes cerebral hypoperfusion resulting in seizures or TIA-like attacks. In severe cases, surgical treatment is performed with superficial temporal artery-middle cerebral artery anastomosis. In mild cases, conservative management is the treatment of choice. Flunarizine is a calcium ion anti-blocking agent, whose primary effect is that the cerebral vessels have been used for the treatment of postcerebrovascular disorders. Recently, it has been suggested that flunarizine

SPECT Myocardial Perfusion Imaging in the Era of PET and Multimodality Imaging: Challenges and Opportunities.

Single photon emission computed tomography (SPECT) remains the most widely used modality for the assessment of coronary artery disease (CAD) owing to its diagnostic and prognostic value, cost-effectiveness, broad availability, and ability to be performed with exercise testing. However, major cardiology guidelines recommend positron emission tomography (PET) over SPECT when available, largely due to its superior accuracy and ability to provide absolute myocardial blood flow quantification. A key limitation of SPECT is its reliance on relative perfusion imaging, which may overlook diffuse flow r

[Quantitative Assessment for Brain Lesion by Perfusion and Metabolic Imaging].

Advances in neuroimaging have improved the diagnosis and management of structural brain diseases, including cerebrovascular disorders and brain tumors. Although conventional computed tomography and magnetic resonance imaging (MRI) provide essential morphological information, functional and metabolic imaging enables the qualitative assessment of tissue physiology. Integrating morphological and qualitative imaging has become increasingly important for optimizing diagnostic strategies, treatment selection, and therapeutic response evaluation. However, excessive imaging may increase cost and compl

Independent predictors of true positivity of positive myocardial perfusion imaging.

UNLABELLED: OBJECTIVE: The primary aim of the present study was to determine the success of single-photon emission computed tomography myocardial perfusion imaging (SPECT-MPI) in detecting critical coronary artery disease (CAD) as identified by invasive coronary angiography (ICA), as well as to compare the positive predictive values (PPV) of different stress methods. Our secondary aim was to investigate demographic, laboratory, electrocardiographic, and echocardiographic variables that could predict true positive results. PATIENTS AND METHODS: The study was conducted with 317 consecutive patie

Imaging Photoplethysmography for Noninvasive Anastomotic Perfusion Assessment in Intestinal Surgery.

INTRODUCTION: Anastomotic leakage after gastrointestinal surgery has a high impact on patient's quality of life and its origin is associated with inadequate perfusion. Imaging photoplethysmography (iPPG) is a noninvasive imaging technique that measures blood-volume changes in the microvascular tissue bed and detects changes in tissue perfusion. MATERIALS AND METHODS: Intraoperative iPPG imaging was performed in 29 patients undergoing an open segment resection of the small intestine or colon. During each surgery, imaging was performed on fully perfused (true positives) and ischemic intestines
