Innocuous "foot lump" in patient with diabetes mellitus: A manifestation of phaeohyphomycosis.

Invasive mycotic infection presenting as "foot lump" in an immunocompetent patient is extremely rare. The case highlights the difficulty in diagnosis of fungal infections of foot including attempts to isolate the fungus and non-response to anti-fungal agents. A 64-year-old lady with T2DM for four years presented with painless, gradually progressive swelling over plantar aspect of left mid-foot for six months. Foot examination revealed soft-to-firm lump over the plantar aspect of left mid-foot obscuring the lateral longitudinal arch. No signs of inflammation or bony deformities were noticed. X-

Severe disseminated phaeohyphomycosis in an immunocompetent patient caused by Veronaea botryosa.

We present a severe case of disseminated phaeohyphomycosis due to Veronaea botryosa. A 32-year-old female, native from Cuautla, Morelos, Mexico, presented a chronic dermatosis which started 10 years earlier with multiple exophytic, multilobulated, soft, and pedunculated or sessile neoformations of diverse sizes from 2 to 10 cm in diameter, which became verrucose and increased in size. The patient was immunocompetent, and no hereditary or familiar precedents of importance were known. No treatment was given, and the dermatosis remained relatively stable until the patient became pregnant in 2001

Phaeohyphomycosis in a snow leopard (Uncia uncia) due to Cladophialophora bantiana.

Phaeohyphomycosis caused by Cladophialophora bantiana was diagnosed in a 5-month-old snow leopard with spastic paralysis of the hind legs and inability to defaecate or urinate. At post-mortem examination, a greenish soft mass resembling an abscess was found on one side of the epidural space at the fourth lumbar vertebral body. Histological examination revealed a purulent meningitis with myelomalacia. Dematiaceous fungal hyphae, present within the inflammatory infiltrate, were identified as C. bantiana by culture and sequence analysis of the 18S ribosomal RNA gene. This neurotropic fungus rarel

[Subcutaneous phaeohyphomycosis due to Exophiala spinifera in a renal transplant recipient].

INTRODUCTION: Among the dematiaceous fungi responsible for human or animal phaeohyphomycosis, the Exophiala genus is a well-known etiologic agent and presently includes nine species considered as opportunist pathogens. To our knowledge, Exophiala spinifera has been reported as causative agent of only thirteen cases of cutaneous or systemic phaeohyphomycosis. We describe some typical phaeohyphomycotic cysts. CASE-REPORT: A 59 year-old female renal transplant recipient, treated with ciclosporine and prednisone, presented with two painless nodular and suppurative lesions of the leg, extending sl

Fatal cerebral phaeohyphomycosis due to Curvularia lunata in an immunocompetent patient.

Curvularia infections in humans are relatively uncommon despite the ubiquitous presence of this soil-dwelling dematiaceous fungus in the environment. Originally thought to be solely a pathogen of plants, Curvularia has been described as a pathogen of humans and animals in the last half-century, causing respiratory tract, cutaneous, and corneal infections. Only three previous cases of central nervous system involvement by Curvularia have been documented in the medical literature. We report a fatal case of cerebral Curvularia infection in which there was no known history of immunocompromise or p

Locally disseminated phaeohyphomycosis caused by Neoscytalidium dimidiatum in an immunosuppressed patient.

Phaeohyphomycosis is considered an uncommon disease, and associated with occupational exposure, being more prevalent in inlands populations from tropical areas of Central and South America. It is caused by dematiaceous fungi, saprophytes that may be found in the soil, plants, as well as in humid and dusty areas, in household environments. They comprise a large group, including more than 150 species and 70 genera implicated in human diseases. The main species involved in phaeohyphomycosis are Alternaria, Cladosporium and Exophiala. Herein, we report an uncommon case of locally disseminated subc

Invasive phaeohyphomycosis due to Parathyridaria percutanea in a liver transplant recipient.

BACKGROUND: Parathyridaria percutanea is a rare melanized fungus causing subcutaneous phaeohyphomycosis, previously reported only in renal transplant recipients. We report the first fatal case in a liver transplant (LT) recipient. CASE PRESENTATION: A 58-year-old Somali woman developed a right foot fungating mass 10 months post-LT during intensive immunosuppression for multiple episodes of T-cell-mediated and antibody-mediated rejection (high-dose corticosteroids, antithymocyte globulin, plasmapheresis). Cultures of drainage and subsequent operative deep-tissue cultures grew Enterococcus faec

Disseminated Phaeohyphomycosis Caused by Curvularia tuberculata in a Previously Healthy Man.

Disseminated phaeohyphomycosis is an extremely rare clinical syndrome, especially in a host without apparent immunological defect. Here, we report a case of disseminated phaeohyphomycosis in a 22-year-old previously healthy man who showed nonmassive hemoptysis from diffuse lung nodules and cavities, together with a hard palate ulcer and generalized subcutaneous nodules. Histopathology, cultures and subsequent molecular assay from two different sites confirmed Curvularia tuberculata infection. The patient was successfully treated with amphotericin B and itraconazole.

Case study: posaconazole treatment of disseminated phaeohyphomycosis due to Exophiala spinifera.

A 41-year-old woman with no known immunosuppression experienced a 12-year period of a relapsing phaeohyphomycosis. Despite administration of multiple courses of therapy with standard antifungals, sustained clinical remission was not achieved. A partial response was seen initially with the combination of itraconazole and flucytosine therapy, but the patient did not respond to subsequent treatment. During the patient's pregnancy, the mycosis became disseminated, with lymphadenopathy and fever, and was considered life threatening. Despite receipt of parenteral amphotericin B therapy, the patient

Additional case of Ramichloridium mackenziei cerebral phaeohyphomycosis from the Middle East.

In this report we describe a case of cerebral phaeohyphomycosis involving a 56-year-old Egyptian male who worked as a mason in Kuwait for 6 years. Computerized tomography scan of the brain revealed presence of a large abscess in the left occipital lobe. Aspirated pus from the abscess showed branched, septate, hyphae with light brown pigmentation. Cultured pus grew the fungus Ramichloridium mackenziei. Despite amphotericin B (1 mg kg(-1) per day) therapy for 2 weeks, the patient expired. The isolate was later found to be resistant to amphotericin B ( > 32 microg ml(-1)). Antifungal susceptibili

Hyalohyphomycosis and phaeohyphomycosis: two global disease entities of public health importance.

Two relatively new opportunistic diseases--hyalohyphomycosis and phaeohyphomycosis--are the byproduct, in most cases, of the progress being made in combating microbial and non-microbial diseases. Immunologically impaired hosts have become vulnerable to infection by fungi that were long considered to be innocuous. The currently known etiologic agents of these two diseases are listed and pertinent literature references are cited. The basic histologic differences between phaeohyphomycosis and chromoblastomycosis was emphasized and illustrated.
