Hyperthyroidism in pregnancy: evidence and hypothesis in fetal programming and development.

The management of hyperthyroidism in pregnant patients has been a topic of raised clinical awareness for decades. It is a strong recommendation that overt hyperthyroidism of Graves' disease in pregnant women should be treated to prevent complications. The consequences of hyperthyroidism in pregnancy are less studied than hypothyroidism, and a literature review illustrates that the main burden of evidence to support current clinical guidance emerges from early observations of severe complications in Graves' disease patients suffering from untreated hyperthyroidism in the pregnancy. On the other

Developing a Spinal CSF Leak Program in a Multihospital Network.

OBJECTIVE: Spontaneous spinal cerebrospinal fluid (CSF) leaks are rare and challenging to diagnose and treat. Patients may present to a variety of physicians, and many patients are often referred to a specialized center with a dedicated spinal CSF leak program and expertise in this condition. To our knowledge, there are no reported publications on how to create such a program. CONCLUSION: In this article, we describe the specific steps we took to develop a spinal CSF leak program, which we have implemented over a multihospital network.

Development of the Initial Surveys for the All of Us Research Program.

BACKGROUND: The All of Us Research Program is building a national longitudinal cohort and collecting data from multiple information sources (e.g., biospecimens, electronic health records, and mobile/wearable technologies) to advance precision medicine. Participant-provided information, collected via surveys, will complement and augment these information sources. We report the process used to develop and refine the initial three surveys for this program. METHODS: The All of Us survey development process included: (1) prioritization of domains for scientific needs, (2) examination of existing v

Developing intervention programs for children with stuttering and concomitant impairments.

School-aged children who stutter often present concomitant impairments in articulation and language that can complicate treatment. In this article, a framework is offered for designing intervention programs for such children. It is stressed that clinicians must first identify clinical priorities by determining the severity of the impairments, their impact on daily activities, others' reactions to the impairments, and the likelihood of unassisted recovery. Several potential treatment models are presented, as are general treatment principles and specific treatment strategies for three profiles o

Perinatal programming, renal development, and adult renal function.

Substantial data indicate that maternal diet during gestation, as well as perinatal events, affect organogenesis and, furthermore, may affect organ function at maturity, thus determining whether an individual is prone or resistant to a variety of health problems such as hypertension, cardiovascular disease, or renal dysfunction. This concept is called perinatal programming, or the fetal origins of adult disease (FOAD), a concept that arose after the publication of epidemiologic observations by Barker and colleagues that birth weight among apparently normal individuals studied in midlife was in

Role of the teaching hospital in the development of a laparoscopic cholecystectomy program.

Laparoscopic cholecystectomy was introduced at McGill University-affiliated hospitals in a planned manner to evaluate the safety and results of this new procedure while training attending and resident surgeons. Laparoscopy was performed with the intent of carrying out cholecystectomy in 500 consecutive patients (70% female, 30% male), whose age averaged 48 years (range from 7 to 93 years). Thirty-seven percent had undergone intra-abdominal surgery previously, and 9.1% had had acute cholecystitis. There were two common-bile-duct injuries and one major small-bowel injury. The procedure had to be

Development of trachoma control programs and the involvement of national resources.

Trachoma, one of the commonest eye diseases in developing countries, is associated with adverse living conditions and low socioeconomic status. The control of trachoma as a blinding disease has been the target of many national campaigns. The strategy of such campaigns has usually been based on intermittent topical treatment on a mass or selective basis, together with services for trichiasis surgery and health education. National campaigns against trachoma have often been successful on a short-term basis but have not always achieved their long-term goals. Sustained efforts are needed to maintai

An Online-Based Transition Care Program for Adolescents with Spina Bifida Using Intervention Mapping: A Protocol for Program Development.

Adolescents with spina bifida (SB) face challenges in their transition to adulthood due to intensive medical regimens and delayed development of independence. Despite an increasing interest in the transition of adolescents with SB to adulthood, the clinical evidence of transition care remains limited, and existing studies have focused on the effects of intervention programs. This study aims to describe the process of systematically developing an online-based transition care program for adolescents with SB using the intervention mapping (IM) protocol. IM consists of six steps: (1) logic model o

Using the Stop Transmission of Polio (STOP) Program to Develop a South Sudan Expanded Program on Immunization Workforce.

In 2009, the international Stop Transmission of Polio (STOP) program began supporting the Global Polio Eradication Initiative in the Republic of South Sudan to address shortages of human resources and strengthen acute flaccid paralysis surveillance. Workforce capacity support is provided to the South Sudan Expanded Program on Immunization by STOP volunteers, implementing partners, and non-governmental organizations. In 2013, the Polio Technical Advisory Group recommended that South Sudan transition key technical support from external partners to national staff as part of the Polio Eradication

Development of program requirements for obesity medicine fellowship training in the U.S.

BACKGROUND: Subspecialty fellowship training programs typically adhere to program requirements for graduate medical education for their field, which are established by accreditation organizations. However, US-based obesity medicine fellowships currently lack this guidance. Therefore, our objective was to develop and validate subspecialty fellowship program requirements for both adult and pediatric obesity medicine in the US. METHODS: A work group from the Obesity Medicine Fellowship Council (OMFC) was convened to define program requirements for obesity medicine (OM) fellowships, including sta

Co-option of a conserved lateral-root development program by symbiotic signals.

Nod factors (NFs) are microbial signals originally identified for their key role in the nitrogen-fixing root nodule symbiosis in legumes. Beyond symbiosis, NFs also possess a conserved capacity to induce lateral-root formation across diverse plant species, including non-legumes. It is now well established that the nodule organogenesis program has co-opted several molecular mechanisms involved in root development, which raises the question of the developmental pathway controlled by NFs to trigger lateral-root formation and how it overlaps with nodule organogenesis in legumes. In Medicago trunca

Specialist paediatric palliative care program development in the standard-of-care era.

OBJECTIVES: The number of Canadian children living with serious illness is increasing. Access to specialist paediatric palliative care is recognized as essential for these children, their families, and their care providers, and yet programs remain under-resourced or non-existent in much of Canada. Health services planning requires current data. This study examined the initial experience of a specialist program established in 2015 at a tertiary paediatric centre in Canada. METHODS: A prospective database study of referred patients was conducted from 2015 to 2023 (prenatal referrals were first

Development of a new competency-based comprehensive training program for vision screeners.

OBJECTIVE: School-based vision screening programs identify children with ocular abnormalities and refer them to eye care professionals, allowing access to better vision and improved learning potential. While non-health care professionals often conduct these screenings, effective training methods are not well defined. This study describes a standardized, competency-based training program to prepare non-health care personnel to perform reliable vision screenings for children. DESIGN: Competency-based and experiential learning. PARTICIPANTS: 170 students from McMaster University. METHODS: Trai

Advancing vascularized composite allotransplantation: essential factors for upper extremity transplant program development.

Vascularized Composite Allotransplantation (VCA) offers a unique option to restore form and function after limb loss or facial trauma that cannot be satisfactorily accomplished through traditional prosthetics or reconstructions. Establishing a successful Upper Extremity Transplantation (UET) program requires strong leadership and a structured surgical team, and extensive interdisciplinary collaboration. We conducted a qualitative study among 12 health care professionals and patients. Informed consent was obtained per protocol, and semi-structured interviews were conducted online and recorded.

RET enhancer haplotype-dependent remodeling of the human fetal gut development program.

Hirschsprung disease (HSCR) is associated with deficiency of the receptor tyrosine kinase RET, resulting in loss of cells of the enteric nervous system (ENS) during fetal gut development. The major contribution to HSCR risk is from common sequence variants in RET enhancers with additional risk from rare coding variants in many genes. Here, we demonstrate that these RET enhancer variants specifically alter the human fetal gut development program through significant decreases in gene expression of RET, members of the RET-EDNRB gene regulatory network (GRN), other HSCR genes, with an altered tran

Development and Introduction of Fexinidazole into the Global Human African Trypanosomiasis Program.

In this article, the authors show the strategy used to streamline the introduction of fexinidazole, the first all oral treatment of human African trypanosomiasis (HAT) caused by Trypanosoma brucei gambiense. The dose range was determined in phase 1 studies and a significant food effect was observed, which was tested with field-adapted meals. The pharmacokinetic profile required definition of a higher loading dosage for the first 4 days and administration of the daily dose together with a typical local meal to optimize product absorption and rapidly achieve drug steady state. This allowed for a
