A computer simulation model for Doppler ultrasound signals from pulsatile blood flow in stenosed vessels.

A computer simulation model based on an analytic flow velocity distribution is proposed to generate Doppler ultrasound signals from pulsatile blood flow in the vessels with various stenosis degrees. The model takes into account the velocity field from pulsatile blood flow in the stenosed vessels, sample volume shape and acoustic factors that affect the Doppler signals. By analytically solving the Navier-Stokes equations, the velocity distributions of pulsatile blood flow in the vessels with various stenosis degrees are firstly calculated according to the velocity at the axis of the circular tu

Numerical simulation of opening process in a bileaflet mechanical heart valve under pulsatile flow condition.

BACKGROUND AND AIM OF THE STUDY: Most previous computational fluid dynamics (CFD) studies of blood flow in mechanical heart valves (MHVs) have not efficiently addressed the important features of moving leaflet and blood-leaflet interaction. Herein, computationally efficient approaches were developed to study these features and to obtain better insight into the pulsatile flow field in bileaflet MHVs. METHODS: A simple and effective method to track the moving boundary was proposed, and an efficient method for calculating the blood-leaflet interaction applied. In this way, a CFD code was develop

In vitro investigation of the impact of pulsatile blood flow on the vascular architecture of decellularized porcine kidneys.

A method was established using a scaffold-bioreactor system to examine the impact pulsatile blood flow has on the decellularized porcine kidney vascular architecture and functionality. These scaffolds were subjected to continuous arterial perfusion of whole blood at normal physiological (650 ml/min and 500 ml/min) and pathophysiological (200 ml/min) rates to examine dynamic changes in venous outflow and micro-/macrovascular structure and patency. Scaffolds subjected to normal arterial perfusion rates observed drops in venous outflow over 24 h. These reductions rose from roughly 40% after 12 h

Multiphysics coupling numerical simulation of flow-diverting stents in the treatment of patients with pulsatile tinnitus.

Patients with pulsatile tinnitus (PT) have unstable treatment effects after resurfacing surgery. Flow-diverting stents (FDS) are proposed as a potential method for the treatment of PT, but the therapeutic effect is not clear. This study aimed to investigate the efficacy of FDS in the treatment of patients with PT induced by sigmoid sinus diverticulum (SSD) and sigmoid sinus wall dehiscence (SSWD). In addition, we aimed to explore the treatment mechanism of PT. Transient-state multiphysics coupling numerical simulation method based on computed tomography angiography of five patients was used to

Non-Invasive Assessment of Damping of Blood Flow Velocity Pulsatility in Cerebral Arteries With MRI.

BACKGROUND: Damping of heartbeat-induced pressure pulsations occurs in large arteries such as the aorta and extends to the small arteries and microcirculation. Since recently, 7 T MRI enables investigation of damping in the small cerebral arteries. PURPOSE: To investigate flow pulsatility damping between the first segment of the middle cerebral artery (M1) and the small perforating arteries using magnetic resonance imaging. STUDY TYPE: Retrospective. SUBJECTS: Thirty-eight participants (45% female) aged above 50 without history of heart failure, carotid occlusive disease, or cognitive impai

The combined role of sinuses of Valsalva and flow pulsatility improves energy loss of the aortic valve.

OBJECTIVES: Normal aortic valve opening and closing movement is a complex mechanism mainly regulated by the blood flow characteristics and the cyclic modifications of the aortic root. Our previous in vitro observations demonstrated that the presence of the Valsalva sinuses, independently from root compliance, is important in reducing systolic pressure drop across the aortic valve. This in vitro study was designed to ascertain if this effect is dependent on the flow characteristics. METHODS: Stentless 21, 23 and 25 mm aortic prostheses were sutured inside Dacron graft with and without sinuses.

Effect of pulsatile and nonpulsatile flow on cerebral perfusion in patients with left ventricular assist devices.

BACKGROUND: Insertion of a left ventricular assist device (LVAD) is an accepted therapy for advanced heart failure patients. However, the effects on end-organ perfusion, including cerebral autoregulation, are unclear in the presence of reduced pulsatility. Therefore, the objective of this study was to determine whether cerebral autoregulation is impaired in patients with continuous-flow (CF) LVADs. METHODS: Dynamic cerebral autoregulation was assessed in both time-domain (autoregulatory index) and frequency-domain (transfer function analysis) analyses in 9 CF-LVAD subjects, 5 pulsatile LVAD s

3D pulsatile flow in a curved tube with coexisting model of aortic stenosis and coarctation of the aorta.

Coarctation of the aorta is a congenital heart disease defined as an obstruction of the aorta distal to the left subclavian artery (between the aortic arch and descending aorta). It is usually associated with other diseases such as bicuspid and tricuspid aortic stenosis. If the coarctation remains uncorrected it can lead to hypertension, left ventricular failure and aortic dissection. Numerous investigations pointed out that there is a relationship between the genesis and the progression of cardiovascular disease and the locally irregular flow occurring at the diseased zone. Therefore, to exam

Experimental and numerical study of pulsatile flows through stenosis: wall shear stress analysis.

Different shapes of pulsatile flows through a model of stenosis are experimentally and numerically modeled to validate both methods and to determine the wall shear stress temporal evolution downstream from the stenosis. Two-dimensional velocity measurements are performed in a 75% severity stenosis using a pulsed Doppler ultrasonic velocimeter. Finite element package is employed for the transient numerical simulations. Polynomial method, based on the experimental velocity values, is proposed to determine the wall shear stress temporal evolution. There is a good agreement between the numerical a

Changes in middle cerebral artery flow velocity and pulsatility index after carotid endarterectomy.

Transcranial Doppler sonography (TCD) was used in 33 patients undergoing carotid endarterectomy (CEA). Mean flow velocity (MCA MV) and the pulsatility index in the middle cerebral artery (MCA PI) were measured pre- and on six occasions postoperatively. The MCA MV was reduced by anaesthesia but was increased postoperatively (+43%, p less than 0.001) compared to the preoperative value and was still increased at late follow-up after several months. The MCA PI, which was lower preoperatively than normally reported in this age group, was not changed by anaesthesia but then rose and remained elevate

Cerebral blood flow pulsatility and cerebral artery stiffness acutely decrease during hemodialysis.

End-stage kidney disease (ESKD) is associated with increased arterial stiffness and cognitive impairment. Cognitive decline is accelerated in ESKD patients on hemodialysis and may result from repeatedly inappropriate cerebral blood flow (CBF). The aim of this study was to examine the acute effect of hemodialysis on pulsatile components of CBF and their relation to acute changes in arterial stiffness. In eight participants (age: 63 ± 18 years, men: 5), CBF was estimated using middle cerebral artery blood velocity (MCAv) assessed with transcranial Doppler ultrasound before, during, and after a s

Clinical outcome and bridge to transplant rate of left ventricular assist device recipient patients: comparison between continuous-flow and pulsatile-flow devices.

BACKGROUND: Long-term implantable continuous axial-flow pumps are increasingly used in bridging heart failure patients to heart transplant. Compared to pulsatile left ventricular assist devices (LVADs), they offer smaller dimensions, less surgical trauma and less thromboembolisms. However concerns still remain about the long-term effects of continuous-flow on patients' outcome. The aim of this study was to review our mechanical bridge to transplant experience to compare pre- and post-transplant outcomes between pulsatile and continuous-flow LVAD recipients. METHODS: Thirty-six patients with a

Experimental analysis of the effects of idealized surface groove amplitudes on post-stenotic pulsatile flow dynamics.

Surface irregularities can substantially modify local hemodynamics within coronary arteries. This study experimentally investigated the influence of controlled grooved surface geometries on pulsatile flow characteristics using idealized coronary-type stenosis models. Four transparent models were fabricated, consisting of a smooth control and three grooved surfaces with increasing groove amplitudes. Time-resolved particle image velocimetry was employed to quantify phase-averaged velocity fields, recirculation structures, and turbulent kinetic energy (TKE). The results showed that groove-induced

Hemodynamic Mechanisms in Venous Pulsatile Tinnitus: A 4D Flow MRI Analysis of Transverse-Sigmoid Sinus Abnormalities.

BACKGROUND: Venous pulsatile tinnitus (VPT) is associated with transverse-sigmoid sinus (TSS) anomalies, bone dehiscence (BD), and hemodynamic disturbances. 4D Flow MRI enables comprehensive TSS evaluation, but causal relationships among TSS morphology, hemodynamics, and BD in VPT onset and progression remain unquantified. Reliable imaging predictors for VPT progression and BD's mediating role are unestablished. PURPOSE: To construct a directed acyclic graph (DAG) testing whether stenosis-induced hemodynamic abnormalities and bone changes predict VPT occurrence and 6-month progression. STUDY

Developing a reduced order model for pulsatile blood flow simulations using minimal three-dimensional simulation data.

BACKGROUND AND OBJECTIVE: Pulsatile blood flow simulations are essential for understanding cardiovascular physiology but are often constrained by the computational cost of full 3D modeling. While reduced-order models (ROMs) offer efficiency, many depend on empirical parameters, limiting their accuracy in complex subject-specific geometries. This study introduces a 1D ROM that minimizes empirical assumptions by deriving parameters directly from 3D simulation data. METHODS: The proposed ROM estimates pressure-flow relationships by fitting parameters of a simplified 1D blood flow equation to res

Vascular function in patients with advanced heart failure and continuous-flow or pulsatile ventricular assist devices.

BACKGROUND: A significant proportion of patients with heart failure (HF) progress to an advanced stage, which is associated with a substantial increase in morbidity and mortality. These patients may be eligible for advanced treatment strategies such as mechanical circulatory support with ventricular assist devices (VAD). Vascular dysfunction is a hallmark of heart failure pathophysiology and prognosis. However, whether and to what degree the hemodynamic benefits of VADs influence vascular function remain unknown. METHODS AND RESULTS: In this study, we evaluated endothelial vascular function w

Computational modeling of blood clot lysis considering the effect of vessel wall and pulsatile blood flow.

Stroke is one of the major causes of global death, which can occur due to blockage in a blood vessel by a clot. The immediate dissolving of the clot is essential to restore the blood flow and prevent tissue necrosis. Clot dissolution can be achieved via thrombolytic therapy using plasminogen activators. In this study, a clot dissolution model is developed for a three-dimensional patient-specific carotid artery that investigates the effect of different vessel wall models on clot dissolution. The lysis pattern of the clot and hemodynamics of blood flow are evaluated using three different models
