OBJECTIVE: The purpose of this study is to demonstrate that pseudotumor cerebri, also known as benign intracranial hypertension, can be an overlooked cause of spontaneous, nontraumatic cerebrospinal fluid (CSF) rhinorrhea.
STUDY DESIGN AND METHODS: This study presents a literature review and 2 case reports. The medical records of 2 patients who had nontraumatic CSF rhinorrhea were reviewed.
RESULTS: The patients were diagnosed as having spontaneous, nontraumatic CSF rhinorrhea, believed to have been caused by pseudotumor cerebri, and were surgically treated. The patients are obese, middle-ag

Cerebrospinal fluid (CSF) rhinorrhea is a rare condition characterized by the leakage of CSF through the nose. The diagnosis is established through comprehensive history taking, brain imaging, and nasal endoscopy. Surgical intervention is considered a secondary option for CSF leakage when conservative treatments, including behavioral therapy, pharmacotherapy, or lumbar puncture, fail to elicit a response. In recent years, endoscopic intranasal surgery has been favored over craniotomy for such surgical treatment. When repairing CSF leakage defects via endoscopic intranasal surgery, autologous f

Cerebrospinal fluid (CSF) rhinorrhea is a rare complication of macroprolactinomas that, in the vast majority of cases, is subsequent to either medical or surgical intervention. Here, we present the successful management of a rare case of spontaneous, noniatrogenic CSF rhinorrhea in a patient with an untreated macroprolactinoma. A 27-year-old man with no significant medical history presented with six months of persistent CSF rhinorrhea, which was confirmed by testing for beta-2-transferrin. He had had decreased libido since adolescence and impaired growth of secondary sexual characteristics. Wo

Seven interesting and instructive cases of cerebrospinal fluid rhinorrhea evaluated by metrizamide computed tomographic cisternography are presented. The rhinorrhea was spontaneous in three patients and was related to previous head trauma or surgical procedures in four patients. The anatomical site and the extent of the fistula were demonstrated precisely by directly showing metrizamide passing through the bony defect. A combination of bone dehiscence and metrizamide within the adjacent paranasal sinuses or the nasal cavity is also useful in localization. Distortion of the interhemispheric fis

BACKGROUND: Chordomas are rare neoplasms derived from the notochord, primarily affecting the skull base and sacral regions. While cranial nerve deficits are common, CSF rhinorrhea as an initial symptom is rare. Ecchordosis physaliphora (EP), another notochord-derived lesion, is typically asymptomatic but may also be present with CSF leakage. Differentiating between these entities remains challenging due to overlapping histopathological and imaging features.
OBSERVATIONS: The authors report the case of a woman in her 50s with persistent CSF rhinorrhea for 4 months. Imaging revealed a central c

Choroid plexus papilloma (CPP) is a rare brain tumour occurring mostly in infants and children. Most CPPs are intraventricular and present with symptoms and signs of increased intracranial pressure (ICP). This case report describes a middle-aged female who presented with spontaneous cerebrospinal fluid (CSF) rhinorrhea from a tumour located in the cerebellopontine angle (CPA). She underwent craniotomy with subtotal tumour resection and remained progression and rhinorrhea-free for several years. Upon clinical progression, the patient was treated with Cyberknife stereotactic radiosurgery. The pa

BACKGROUND: Spontaneous cerebrospinal fluid (CSF) leaks are caused by intracranial hypertension. Given this underlying etiology, patients may be at risk for developing multiple skull base defects.
OBJECTIVE: The purpose of our study is to present the prevalence of multiple simultaneous skull base defects in patients with spontaneous CSF rhinorrhea.
METHODS: We performed a retrospective chart review in a tertiary care practice of 44 consecutive patients with spontaneous CSF rhinorrhea who underwent endoscopic repair by the senior author (R.R.C.) to determine the prevalence of having multiple

This study describes a case of a patient with traumatic rupture of a maxillary sinus retention cyst, which had an interesting clinical presentation of unilateral rhinorrhea, mimicking a CSF leak. The diagnosis was made fortuitously by comparison of a posttraumatic CT brain examination with a CT sinus study performed 1 day earlier.

A supra-ethmoidal approach was used to treat cerebrospinal fluid rhinorrhea secondary to anterior fractures of the base of the skull. After an internal orbital incision, the anterior wall of the frontal sinus is removed and the posterior wall of this sinus trephined in the internal region of its horizontal portion in order to expose the meninges covering the roof of the first ethmoidal space. Dissection is continued within the space towards the anterior extremity of the apophysis of the crista galli against which the fibrous band issuing from the anterior ethmoidal opening is sectioned. OBJECTIVE: Spontaneous cerebrospinal fluid (CSF) rhinorrhea is commonly misdiagnosed due to overlapping symptomatology with other sinonasal diseases, thus leading to delay in diagnosis and increased risk of meningitis. The objective of this study was to examine the risk factors for the initial misdiagnosis of spontaneous CSF rhinorrhea.
METHODS: A single-institutional retrospective chart review of patients with spontaneous CSF rhinorrhea was performed. Patient characteristics, initial diagnosing provider specialty, and accuracy of radiology reports were compared between subjects initially dia

INTRODUCTION: Spontaneous CSF leak is a known complication of idiopathic intracranial hypertension (IIH). Patients with CSF rhinorrhea present a unique challenge within the IIH population, as the occurrence of a leak can mask the typical IIH symptoms and signs, complicating the diagnosis. Treatment of leaks in this population can also be challenging, with the risk of rhinorrhea recurrence if intracranial hypertension is not adequately treated.
OBJECTIVE: The aim of this narrative review was to examine current literature on the association between spontaneous CSF rhinorrhea leaks and IIH, focu
