Diabetic neuropathy (DNP) is a severe complication of diabetes mellitus. In this study, we examined the potential of hesperidin (HES) to attenuate DNP and the involvement of the TRPM2 channel in this process. The rats were given a single dose of 45 mg/kg of streptozotocin (STZ) intraperitoneally to induce diabetic neuropathic pain. On the third day, we confirmed the development of diabetes in the DNP and DNP + HES groups. The HES groups were treated with 100 mg/kg and intragastric gavage daily for 14 days. The results showed that treatment with HES in diabetic rats decreased STZ-induced hyperg

BACKGROUND: Lumbosacral pain is commonly seen in daily clinical practice. In fact, entrapment of the part of the sciatic nerve after the sacral foramen causes some of these pains, which should not be overlooked. The sciatic nerve may be compressed during its course after the sacral foramen for a variety of reasons. We aimed in this article to review extra-spinal compressive sciatic neuropathy reasons and abnormal magnetic resonance imaging (MRI) by presenting a sciatic neuralgia case of an entrapment neuropathy mimicking piriformis syndrome due to soft tissue hemangioma.

We report here the case of a 18-year-old young man with Behçet's disease who had suffered deep venous thrombosis of the right femoral and popliteal veins. Consequently, right sciatic nerve injury, drop foot and tightness of the achilles tendon also ensued. The clinical scenario was further challenged by demonstration of the agenetic inferior vena cava and epidural vein dilatations compressing the lumbar nerve roots. To the best notice of the authors, this is the first patient encompassing all these complications in the literature concerning Behçet's disease.

OBJECTIVE: Core outcome sets (COSs) are needed to promote data consistency across studies as well as data synthesis and comparability. The goal of the current study was to utilize a modified Delphi process to develop a COS-sciatic injury and neuropathy evaluation (COS-SINE).
METHODS: A five-stage approach was utilized to develop the COS-SINE: stage 1, consortium development; stage 2, literature review to identify potential outcome measures; stage 3, Delphi survey to develop consensus on outcomes for inclusion; stage 4, Delphi survey to develop definitions; and stage 5, consensus meeting to fi

Neurologic complications following neuraxial anesthesia for cesarean delivery are rare. We present a 33-year-old parturient who developed prolonged lower extremity weakness following a single-shot subarachnoid block for cesarean delivery. After neurologic evaluation, she was diagnosed with bilateral sciatic neuropathies due to prolonged positioning for the anesthetic. We review the incidence of nerve injury associated with neuraxial anesthesia and risk factors for developing peripheral nerve injury in this context. We offer a solution to prevent this complication from occurring.

We report three patients in whom carpal tunnel syndrome was due to neurapraxis as a consequence of unusual sleep postures enabled by their benign joint hypermobility syndrome (BJHS). A fourth patient also had bilateral sensory sciatic neuropathy and a common peroneal palsy, similarly due to regularly adopted unusual postures. Recognition of such cause-effect mechanisms and their simple solution in BJHS should obviate needless investigation and unnecessary surgery.

BACKGROUND: Increased oxidative stress has been linked to local anesthetic-induced nerve injury in a diabetic neuropathy (DN) rat model. The current study explores the effects of diphenyleneiodonium (DPI) chloride, an NADPH oxidase (NOX) inhibitor, on bupivacaine-induced sciatic nerve injury in DN rats.
METHODS: A rat DN model was established through high-fat diet feeding and streptozotocin injection. The model was confirmed via testing (i) blood glucose, (ii) hindpaw allodynia responses to von Frey (VF) monofilaments, (iii) paw withdrawal thermal latency (PWTL), and (iv) nerve conduction vel

We hypothesized that microglia in the ventral posterolateral (VPL) nucleus of the thalamus are reactive following peripheral nerve injury, and that inhibition of microglia by minocycline injection in the VPL attenuates thermal hyperalgesia. Our results show increased expression of OX-42 co-localized with phosphorylated p38MAPK (P-p38) in the VPL seven days after chronic constriction injury (CCI) of the sciatic nerve. However, astrocytic GFAP expression in the VPL is unchanged 7 and 14 days after CCI. Microinjection of minocycline into the VPL contralateral to CCI reverses thermal hyperalgesia,

Paclitaxel (PTX) is a potent taxane widely used in the treatment of solid tumors and can cause dose-limiting peripheral neuropathy. This study evaluated the therapeutic potential of selenium in a paclitaxel-induced peripheral neuropathy model. A total of 30 male Sprague-Dawley rats were divided into five groups (n = 6): Control, SE1, PTX, PTX+SE0.5, and PTX+SE1. PTX (2 mg/kg, i.p., days 1-5) was administered followed by SE (0.5 or 1 mg/kg, i.g., days 6-15); sciatic nerve tissues were analyzed on day 16. In addition to molecular and histopathological analyses, behavioral assessments were perfor

Neurologic complications after spinal anesthesia are uncommon but remain a major concern because of their potential severity. Distinguishing anesthesia-related neurologic injury from preexisting spinal pathology is often challenging. A 70-year-old man with hypertension, hyperlipidemia, and known lumbar degenerative disease underwent elective bilateral inguinal hernia repair under spinal anesthesia. Spinal anesthesia was performed via a paramedian approach at the L3-4 interspace using a 25-gauge Quincke needle and 2.6 mL (13 mg) of 0.5% hyperbaric bupivacaine without paresthesia or technical di

This study evaluates whether diffusion tensor imaging magnetic resonance neurography (DTI-MRN), T2 relaxation time, and proton spin density can detect and grade neuropathic abnormalities in patients with type 1 diabetes. Patients with type 1 diabetes (n = 49) were included-11 with severe polyneuropathy (sDPN), 13 with mild polyneuropathy (mDPN), and 25 without polyneuropathy (nDPN)-along with 30 healthy control subjects (HCs). Clinical examinations, nerve conduction studies, and vibratory perception thresholds determined the presence and severity of DPN. DTI-MRN covered proximal (sciatic nerve

CASE: Three patients with sciatica were referred to our outpatient clinic between 2007 and 2012. Magnetic resonance imaging (MRI) of the hip showed compression of the sciatic nerve by a perineural cyst arising from a paralabral cyst. All 3 patients underwent arthroscopic decompression. Upon follow-up, the mean visual analog scale scores for pain had improved from 7.3 to 0.3. MRI showed no evidence of recurrence of the cyst in any of the patients.
CONCLUSION: Arthroscopic treatment for entrapment neuropathy of the sciatic nerve caused by a paralabral cyst was effective for improving symptoms;

Lower-extremity trauma is an uncommon but reported cause of sciatic nerve injury in children and adolescents. Failure to identify sciatic neuropathy after traumatic injury to the lower extremity may lead to the delayed institution of neuropathic pharmacotherapy, electrodiagnostic testing, physical therapy, and increased risk for the development of complex regional pain syndrome. This article presents a case of an adolescent male with neuropathic pain and weakness in the right lower extremity after traumatic injury. Spontaneous recovery of the injured nerve occurred with early institution of ph

This study was conducted to evaluate the capability of ultrasonography to visualize extremity nerves. Fifty healthy women and men and 10 patients suffering with hereditary motor and sensory neuropathy (HMSN) were examined. The radial nerve lateral to the humerus, ulnar nerve distal to the cubital tunnel, median nerve in the middle of the forearm and proximal to the palmar crease, sciatic nerve in the middle of the thigh and tibial and common peroneal nerves just distal to their bifurcation, were investigated, employing a 7.5-MHz electronic linear-array transducer. In healthy subjects, the medi

An 82-year-old man underwent bilateral internal iliac artery (IIA) embolization for IIA aneurysms and endovascular repair for an abdominal aortic aneurysm. The bilateral IIA branches were embolized with coils and Amplatzer Vascular Plugs, and N-butyl cyanoacrylate (NBCA) was used for the left superior lateral sacral artery. Postprocedure, the patient developed numbness, pain, and walking difficulty. He was diagnosed with ischemic sciatic neuropathy, and magnetic resonance imaging revealed pelvic bone infarction. NBCA injection and IIA embolization were considered the causes. These complication

BACKGROUND: Nerve injury is a common complication of intramuscular (im) injection and the sciatic nerve (SN) is the most frequently affected nerve. Pain management in neuropathic pain due to sciatic injury is challenging.
CASE REPORT: A 66-year-old man admitted with neuropathic pain in his right leg. The pain commenced 6 years ago after an im injection into the right hip.
CONCLUSIONS: Interventional treatment methods are not favored in the treatment of SN neuropathy subsequent to injection injury. However, bipolar pulsed radiofrequency treatment to L4-L5-S1 dorsal root ganglia may be an effe

This report describes a case of sciatic neuropathy caused by an obstruction of the nutrient artery of the nerve. The patient was a 76-year-old man who had an aneurysm in his right thigh and suddenly complained of pain and weakness in his right leg. Computed tomography scan and ultrasonography of the right thigh showed an aneurysm with a thrombus in the lumen of the profunda femoral artery. His symptoms did not immediately improve after the aneurysm had been resected, and were consistent with right sciatic neuropathy. Recovery of strength of the right tibialis anterior muscle from 0/5 to 3/5 re

Spontaneous arterial rupture is an uncommon but potentially life-threatening event. In this case spontaneous bleeding from a gluteal artery resulted in a gluteal compartment syndrome and sciatic neuropathy. Bleeding is best managed by arteriography and embolization. Similar in pathophysiology to the more common leg and forearm compartment syndromes, gluteal compartment syndromes are treated with fasciotomy. Spontaneous arterial bleeding in an extremity may be the initial symptom in patients with a rare form of Ehlers-Danlos syndrome. Additional evaluation for the presence of significant abdomi
