Psychophysical correlates in children with sensory modulation disorder (SMD).

Sensory modulation disorder (SMD), affecting approximately 5% of children, is characterized by sensory over or under-responsiveness to a range of stimuli in several modalities. Children with over-responsiveness (SOR) demonstrate increased aversion to certain natural stimuli that manifests as increased distress and avoidance behaviors to common stimuli, accompanied by abnormal electrodermal responses and brain evoked potentials to various stimuli. This study is the first to use quantitative sensory testing to characterize the somatosensory sub-modalities of children with SMD. Seventy eight chil

Neurophysiologic parameters and symptoms in chronic renal failure.

We studied: (1) the sensitivity of various neurophysiologic parameters in the diagnosis of uremic polyneuropathy, (2) the relationship between subjective symptoms and neurophysiologic parameters, and (3) the effect of a single hemodialysis on the neurophysiologic parameters in 21 patients undergoing hemodialysis. The following parameters were studied: sensory and motor nerve conduction, including F-wave parameters; vibration detection thresholds; and thermal thresholds. The clinical findings and subjective symptoms were studied using a standardized questionnaire. The most sensitive parameters

Sensory impairment: a feature of chronic venous insufficiency.

OBJECTIVE: Clinical and microscopic evidence suggests the existence of sensory neuropathy in patients with severe chronic venous insufficiency (CVI). A clinical evaluation was conducted to determine whether a sensory neuropathy was present and, if so, to determine its extent and distribution. METHODS: The study was performed in a university-affiliated Veterans Affairs Medical Center. Twenty-three limbs were studied in 14 male veterans with mild or moderate CVI. The exclusions included diabetes, previous ipsilateral extremity surgery, or other diseases associated with neuropathy. Sensory thres

Effect of Dexmedetomidine on Perception of Paresthesia during Subthalamic Nucleus Deep Brain Stimulation Surgery for Parkinson's Disease.

BACKGROUND: Deep brain stimulation (DBS) has become a well-established treatment for the management of Parkinson's disease (PD). The most common method of lead targeting utilizes microelectrode recording (MER) and intraoperative macrostimulation to confirm accurate placement of the lead. This has been significantly aided by the use of dexmedetomidine (DEX) sedation during the procedure. Despite the frequent use of DEX, it has been theorized that DEX may have some effects on the MER during intraoperative testing. The effect on the perception of sensory thresholds during macrostimulation in the

Altered thermal sensitivity in facial skin in chronic whiplash-associated disorders.

There is a close functional relationship between the jaw and neck regions and it has been suggested that trigeminal sensory impairment can follow whiplash injury. Inclusion of manageable routines for valid assessment of the facial sensory capacity is thus needed for comprehensive evaluations of patients exposed to such trauma. The present study investigated facial thermal thresholds in patients with chronic whiplash-associated disorders (WADs) with both a qualitative method and quantitative sensory testing (QST). Ten women with pain and dysfunction following a whiplash injury were compared to

Whiplash (grade II) and cervical radiculopathy share a similar sensory presentation: an investigation using quantitative sensory testing.

OBJECTIVES: Recent research has identified the coexistence of generalized sensory hypersensitivity and hypoesthetic changes suggestive of a neuropathic component to chronic whiplash associated disorders (WAD). This study aimed to compare chronic whiplash with a cervical neuropathic condition-cervical radiculopathy, using Quantitative Sensory Testing. METHODS: Fifty participants with chronic grade II WAD (>3 mo), 38 participants with radiculopathy, and 31 controls who were age and sex matched to participants with WAD participated in the study. Hypoaesthesia occurs with sensory hypersensitivity in chronic whiplash--further evidence of a neuropathic condition.

Hypersensitivity to a variety of stimuli has been shown in whiplash associated disorders and may be indicative of peripheral nerve involvement. This cross-sectional study utilised Quantitative sensory testing (QST) including vibration, thermal, electrical detection thresholds as an indirect measure of primary afferents that mediate innocuous and painful sensation. Pain thresholds and psychological distress (SCL-90-R) were also measured. Thirty-one subjects with chronic whiplash (>3 months, NDI: 49+/-17) and 31 controls participated. The whiplash group demonstrated elevated vibration, heat and

Subthalamic deep brain stimulation modulates conscious perception of sensory function in Parkinson's disease.

Subthalamic deep brain stimulation (STN-DBS) is used to treat refractory motor complications in Parkinson disease (PD), but its effects on nonmotor symptoms remain uncertain. Up to 80% of patients with PD may have pain relief after STN-DBS, but it is unknown whether its analgesic properties are related to potential effects on sensory thresholds or secondary to motor improvement. We have previously reported significant and long-lasting pain relief after DBS, which did not correlate with motor symptomatic control. Here we present secondary data exploring the effects of DBS on sensory thresholds

Self-reported sensory descriptors are associated with quantitative sensory testing parameters in patients with cervical radiculopathy, but not in patients with fibromyalgia.

BACKGROUND: The painDETECT questionnaire (PD-Q) has been used as a tool to characterize sensory abnormalities in patients with persistent pain. This study investigated whether the self-reported sensory descriptors of patients with painful cervical radiculopathy (CxRAD) and patients with fibromyalgia (FM), as characterized by responses to verbal sensory descriptors from PD-Q (sensitivity to light touch, cold, heat, slight pressure, feeling of numbness in the main area of pain), were associated with the corresponding sensory parameters as demonstrated by quantitative sensory testing (QST). Assessment techniques to evaluate tactual aids for hearing-impaired subjects.

In order to optimize the use of tactual aids for the deaf, it is important to have a battery of assessments to determine the potential contribution of the aids to acoustic perception and speech identification. We have designed such a battery to be used with young hearing-impaired children. The tasks were developed so that they could be implemented with standard audiometric equipment and applied to subjects of varying age and to different types of tactual aids. Illustrative results from four profoundly hearing-impaired children showed that tactual vocoders allowed detection of high frequencies

Development and Validation of a Wearable Device to Provide Rich Somatosensory Stimulation for Rehabilitation After Sensorimotor Impairment.

Training sensory discrimination of the skin has the potential to reduce chronic pain due to sensorimotor impairments and increase sensorimotor function. Currently, there is no such device that can systematically provide rich skin stimulation suitable for a training protocol for individuals with amputation or major sensory impairment. This study describes the development and validation of a non-invasive wearable device meant to repeatedly and safely deliver somatosensory stimulations. The development was guided by a structured design control process to ensure the verifiability and validity of t

On the mechanics of monofilaments used in touch sensory perception.

The buckling of a slender monofilament is a standard clinical method used to assess touch sensory perception, with specific applications to somatosensory impairment in patients after a stroke, detecting carpal tunnel syndrome, and as a prognosis tool for diabetic peripheral neuropathy. The basis of this approach is the Semmes-Weinstein monofilament, which comprises a calibrated set of thin, polycarbonate rods with different diameters. Application of a monofilament onto the surface of the skin with increasing pressure causes the rod to buckle. That is, at a specific force of pushing down on the

The Association Between Olfaction and Depression: A Systematic Review.

Previous studies on the relationship between olfaction and depression have revealed mixed results. In addition, few have focused on the reciprocity of this association. The aim of this study is to combine depression and olfactory data in two separate patient populations to further understand their association. A systematic literature review was conducted using 3 online databases to identify studies correlating olfaction and depression in patients presenting with either primary depression or primary olfactory dysfunction. For the depressed population, weighted means and standard deviations for

Optimal colors can predict luminosity thresholds in natural scenes.

Luminosity thresholds define the luminance boundary at which a surface color shifts in appearance from being perceived as an illuminated surface to appearing self-luminous. Previous research suggests that the human visual system infers these thresholds based on internal references of physically realizable surface colors under a given illumination, referred to as the physical gamut. A surface is perceived as self-luminous when its luminance exceeds the upper limit of this empirically internalized gamut. However, the precise structure and boundaries of these gamuts remain uncertain. Optimal colo

The psychophysical limits of agency: Biological motion percepts are dramatically weakened in slow-moving displays.

The most important stimuli we perceive may be other agents, given their effects on our fitness. Accordingly, perception may be specialized for processing agents, and one of the best-studied examples is biological motion: displays of surprisingly few moving dots ("point-light walkers"; PLWs) nevertheless give rise to rich percepts of locomoting agents. Is this a complex instance of general motion perception, such that PLWs simply inherit the lower thresholds of general motion perception? Or might biological motion have a distinct lower "speed limit"? Observers viewed PLWs moving in place, alway

Characterizing amblyopic perception under non-rivalrous viewing conditions.

Current assessments of interocular interactions in amblyopia often use rivalrous stimuli, with conflicting stimuli in each eye, which does not reflect vision under typical circumstances. Here we measure interocular interactions in observers with amblyopia, strabismus with equal vision, and controls using a non-rivalrous stimulus. Observers used a joystick to continuously report the perceived binocular contrast of dichoptic grating stimuli, identical except that the stimulus was contrast-modulated independently in each eye over time. Consistent with previous studies, a model predicting the time

Testing olfaction in a clinical setting.

Assessment of olfactory functioning at the CCCRC entails a threshold test and an odor identification test that contains eight everyday items. A performance average on the two tests yields a composite score on a scale from 0 (anosmia) to 7 (normosmia). The performance of normal volunteers is stable over most of an individual's life span, but decreases for persons over the age of 65 years. Approximately half the patients with olfactory complaints display anosmia, and the other half have hyposmia. The distribution of scores, however, varies with etiologic category, emplified by the fact that pati
