[Charcot and Spinal Cord Disease].

This paper outlines the contributions of Jean-Martin Charcot (1825-1893) to the study of spinal cord diseases. Charcot pioneered the anatomoclinical method, which correlated clinical symptoms with pathological findings through detailed observations and autopsies. He elucidated the dual structure of the motor system-gray and white matter-and established the clinical and pathological framework of amyotrophic lateral sclerosis, distinguishing it from other disorders. He also contributed to the understanding of tabes dorsalis by linking sensory ataxia to lesions of the dorsal columns and roots, an

Primary spinal hydatid disease.

Primary spinal hydatid disease is rare. Spinal hydatid disease should be considered in the differential diagnosis of spinal cord compression syndrome in endemic countries and evaluated with imaging and serology. Our case was a 34- year-old man. The patient presented with progressive back pain for 8 months and lower extremity weakness for 3 months. Neurological examination was suggestive of upper motor neuron type of paraperesis. Magnetic resonance images of the thoracal region showed an intradural multicystic lesion. The mass was explored with T 10-11 laminectomy. It had displaced the cord to

Spinal decompensation in neuromuscular disease.

STUDY DESIGN: In this retrospective radiography study, we analyzed curve shape and direction in scoliosis secondary to neuromuscular disease. OBJECTIVE: To determine if in different types of neuromuscular scoliosis a predominant curve pattern can be found and if similarities with idiopathic scoliosis exist. SUMMARY OF BACKGROUND DATA: To the authors' knowledge, systematic analysis of curve patterns in patients with neuromuscular scoliosis has not been performed in a group of this size and composition. METHODS: Spinal full-length radiographs of 198 patients with neuromuscular scoliosis were

Spinal involvement in Pyle's disease.

BACKGROUND: Pyle's disease is a rare skeletal dysplasia involving the metaphyses of long bones. To date, spinal involvement has been only rarely described in the literature. OBJECTIVE: To show that spinal changes, which are an expression of the same growth defect of the long bones, are an important and constant sign of the disease in the families studied. MATERIALS AND METHOD: The radiographic skeletal changes in five patients have been observed. RESULTS: The pathognomonic metaphyseal widening of tubular bones (known as the 'Erlenmeyer flask sign') was associated with the spinal changes in

Approach to the patient with spinal disease.

The approach to the patient suspected of having spinal disease should be methodical to assure an accurate diagnosis that is cost-effective with minimal risk to the patient. Using a problem-oriented approach will facilitate attaining this goal. A neurologic examination should enable the veterinarian to localize to one of four spinal cord segments or the cauda equina.

Spinal empyema in Crohn's disease.

A 19-year-old man with a 1-year history of Crohn's disease of the ileum and rectosigmoid developed back and leg pain with neurological deficits. He proved to have an epidural and subdural spinal empyema originating from a rectal fistula. Drainage of the empyema, a diverting sigmoid colostomy, and appropriate antibiotics allowed full recovery of neurologic function and resolution of infection. We review the applicable literature.

Spinal arachnoiditis: disease or coincidence?

In 86% of 63 patients with spinal arachnoiditis the localization was lumbosacral, and in 14% it was cervical or thoracic. The most important aetiological factor was the combination of one or more myelographies with one or more operations. It is remarkable that in most patients with lumbosacral arachnoiditis the clinical picture did not show new or typical signs of arachnoiditis, but was the same as before the arachnoiditis had developed. In only 20% of patients with lumbosacral arachnoiditis, and in all patients with cervical or thoracic arachnoiditis, did new signs and symptoms occur concomit

[Frequency of 5q spinal muscular atrophy in adults with unspecified neuromuscular diseases].

OBJECTIVE: To assess the prevalence of 5q spinal muscular atrophy (SMA) among adult patients with undifferentiated neuromuscular disorders. MATERIAL AND METHODS: Prospective study of 50 patients (19-78 years) presenting ≥1 feature of 5q SMA: areflexia, proximal weakness, fasciculations, neurogenic EMG changes, atrophy, calf hypertrophy, or elevated creatine kinase (CK). Molecular testing (MLPA/melting curve analysis of SMN1/SMN2) was performed. RESULTS: 5q SMA was confirmed in one female patient (2% [95% CI 0.05-10.6]), who was found to have a homozygous deletion of exons 7-8 in the SMN1 gen

[Importance of neuro-urological care in patients with spinal cord injuries/diseases].

Integrity, control and regulation of the urinary tract are subject to a complex neuronal regulation, in which portions of the sympathetic, parasympathetic and somatic nervous system are involved. The spinal cord plays a central role in regulation and serves as a transmitter for the motor and sensory pathways. Bladder dysfunction followed by renal dysfunction was the most frequent cause of death in patients with spinal cord injuries/diseases (paraplegia) as recently as half a decade ago. Thanks mainly to diagnostic and therapeutic advances made in neuro-urology, urological problems are no longe

Disruption of Neuromuscular Junction Following Spinal Cord Injury and Motor Neuron Diseases.

The neuromuscular junction (NMJ) is a crucial structure that connects the cholinergic motor neurons to the muscle fibers and allows for muscle contraction and movement. Despite the interruption of the supraspinal pathways that occurs in spinal cord injury (SCI), the NMJ, innervated by motor neurons below the injury site, has been found to remain intact. This highlights the importance of studying the NMJ in rodent models of various nervous system disorders, such as amyotrophic lateral sclerosis (ALS), Charcot-Marie-Tooth disease (CMT), spinal muscular atrophy (SMA), and spinal and bulbar muscul

Advances in Platelet-Rich Plasma Treatment for Spinal Diseases: A Systematic Review.

Spinal diseases are commonly associated with pain and neurological symptoms, which negatively impact patients' quality of life. Platelet-rich plasma (PRP) is an autologous source of multiple growth factors and cytokines, with the potential to promote tissue regeneration. Recently, PRP has been widely used for the treatment of musculoskeletal diseases, including spinal diseases, in clinics. Given the increasing popularity of PRP therapy, this article examines the current literature for basic research and emerging clinical applications of this therapy for treating spinal diseases. First, we revi

C5 palsy following anterior decompression and spinal fusion for cervical degenerative diseases.

Postoperative C5 palsy is a common complication after cervical spine decompression surgery. However, the incidence, prognosis, and etiology of C5 palsy after anterior decompression with spinal fusion (ASF) have not yet been fully established. In the present study, we analyzed the clinical and radiological characteristics of patients who developed C5 palsy after ASF for cervical degenerative diseases. The cases of 199 consecutive patients who underwent ASF were analyzed to clarify the incidence of postoperative C5 palsy. We also evaluated the onset and prognosis of C5 palsy. The presence of hig

Brain and/or Spinal Cord Tumors Accompanied with Other Diseases or Syndromes.

Several medical conditions that interest both the brain and the spinal cord have been described throughout the history of medicine. Formerly grouped under the term Phacomatosis because lesions of the eye were frequently encountered or genodermatosis when typical skin lesions were present, these terms have been progressively discarded. Although originally reported centuries ago, they still represent a challenge for their complexity of cure. Nowadays, with the introduction of advanced genetics and the consequent opportunity of whole-genome sequencing, new single cancer susceptibility genes have
