BACKGROUND: There are few reported instances of intraspinal migration of a bullet fragment. The majority of these migrations occur caudally, typically below the level of T10. Even fewer cases demonstrate cephalad migration from the sacral spine to the lumbar spine. We report here for the first time a case of a cephalad migration intradurally from the thoracic spine to cervical spine.
CASE DESCRIPTION: A 31-year old man presented to the emergency department with a suspected spinal cord injury following a GSW sustained to the left shoulder. A penetrating gunshot injury to the thoracic spine at

The Thoracolumbar Injury Classification and Severity Score (TLICS) is a scoring and classification system developed by the Spine Trauma Study Group in response to the recognition that previous classification systems have limited prognostic value and generally do not suggest treatment pathways. The TLICS provides a spine injury severity score based on three components: injury morphology, integrity of the posterior ligamentous complex (PLC), and neurologic status of the patient. A numerical score is calculated for each category, with a lower point value assigned to a less severe or less urgent i

Abusive spinal injuries, although often clinically silent, may require treatment and have medicolegal significance. Abused infants are prone to ligamentous injury of the craniocervical junction and upper cervical spine, which is often associated with brain hypoxic-ischemic injury. Although they are relatively common and highly specific for abuse, thoracolumbar spine injuries (e.g., thoracolumbar compression fractures and spinal extra-axial hemorrhages) are most often clinically occult. Additionally, they are often undetectable on skeletal survey and are frequently found in the absence of cervi

Lifting with a flexed spine, especially near the end range of motion, has been identified as a potential risk factor for low back injury/pain. Therefore, individuals who develop discomfort from repetitive, prolonged and/or loaded flexed or slouched postures may benefit from a greater awareness of how to control and/or modify their spinal posture to avoid irritating their backs in these situations. This study was therefore designed to test the ability of spine-oriented verbal instructions to reduce intersegmental spine flexion during three lifting tasks. The lifts were first performed without a

The aim of the study was to investigate the neurological outcome of spinal cord injuries in the traumatized cervical spine with a stenosis of the spinal medullary canal. From 1992 to 1999 we treated 145 spinal cord injuries and/or injuries with an unstable cervical spine, 138 were treated operatively, in 7 patients we found an injury of the spinal cord with a stable cervical spine and a stenosis of the spinal medullary canal. The radiological diagnostics consisted of anterioposterior X-rays of the cervical spine, an X-ray view of the dens axis and a stress roentgenogramm in extension and flect

BACKGROUND CONTEXT: Significant variability in the management of Adult Spinal Deformity (ASD) has been observed among spine surgeons worldwide. The variability among Canadian spine surgeons, a country with universal public healthcare, remains unknown.
PURPOSE: The study aims to evaluate areas of variability in the perioperative optimization and surgical management of ASD among Canadian spine surgeons.
STUDY DESIGN/SETTING: In this cross-sectional study, 25 Canadian spine surgeons, predominantly orthopedic surgeons (20) and neurosurgeons (5) with varying experience, participated in an online

The thoracolumbar spine is the most commonly afflicted area in vertebral column injuries. Here we bring up a case of a 20-year-old male who presented to our emergency department with a history of a high-velocity road traffic accident with noncontiguous two-level fracture-dislocations of the thoracolumbar spine with blunt trauma to the chest. The patient was managed with posterior reduction and instrumented postero-lateral fusion. Such an injury pattern has been reported only rarely in the literature. This report expects to highlight the unusual fracture pattern of a common injury and the chall

We report the case of a 20-year-old patient with a C5 cervical spine fracture and an undetected ligamentous lesion between C1 and C2. Cervical spine lesion protocols and the rates of lesions that are not diagnosed with standard evaluation protocols are reviewed, with particular emphasis on comatose patients. Dynamic studies during the surgical procedure for fixation of the fracture are recommended to increase the detection of ligamentous lesions.

Metastatic spine cancer (MSC), a common complication of advanced malignancies, poses significant challenges due to pain, neurological deficits, and mechanical instability. While radiation therapy is a cornerstone of treatment, the role of spine surgery is evolving, fueled by advances in surgical techniques and radiation modalities such as stereotactic body radiation therapy (SBRT). This review examines the evolving role of spine surgery in MSC management, focusing on separation surgery, surgical innovations, and future directions. The treatment paradigm for MSC shifted with the advent of SBRT,

The etiology of the adolescent idiopathic scoliosis (AIS) remains unknown. Variations in the sagittal profile of the spine between the early stage scoliotic and non-scoliotic pediatric patients have been shown. However, no quantitative study has shown the link between the sagittal profile and 3D deformity of the spine. 126 right thoracic scoliosis with spinal and 3D reconstructions were included. A 2D finite element model was developed for each of the sagittal curve types without any deformity in the frontal or axial planes. Physiological loadings were determined from the literature and were a

The development of the spine and spinal cord occurs at the earliest weeks of gestation. Their development not only affects each other but also are most likely associated with anomalies in other systems. It is essential to recognize the stages of spine development to understand the cause of congenital spinal deformities and their influences on the postnatal growing spine. A vast majority of congenital spinal problems are not evident clinically. For instance, the presence of neural axis abnormalities, such as spinal dysraphism or syringomyelia, may be so subtle that patients never seek medical c

Spinal alignment changes with age and degeneration. Different compensatory mechanisms of the spine are necessary to preserve spinal balance. The capacity of compensation of the spine decreases with age. Thus, the pelvis and the lower limbs become involved in the compensatory mechanism. Concomitant osteoarthritis of the hip could impair this capacity. The biomechanical principles of compensation are described with respect to planning reconstructive hip and spine surgery.

INTRODUCTION: Involvement of the spinal column in either monostotic or polyostotic form is rare, with fewer than thirty-five cases discussed in the literature. Most of the cases of polyostotic fibrous dysplasia of spine have involvement of the appendicular skeleton.
CASE REPORT: We report a case of a 74-year-old Irish man with a two month history of back pain. Investigations revealed a diagnosis of fibrous dysplasia involving three levels of the thoracic spine in isolation. The patient underwent T2-T9 stabilization and bone grafting.
CONCLUSION: A case of fibrous dysplasia involving three le

The elderly present with progressive degenerative osteoarthritis of the knee and lumbar spine degeneration (LSD). The coexistent conditions when presented concurrently become challenging for the clinicians and surgeons, as well as determining the predominant source of the underlying pain generation factor. The concurrent presentation of a significant knee and low back pain poses challenges in decision-making for treatment with management being normally individualized. The review narrates the different concepts used for the assessment of knee-spine syndrome. The prime factor for the pain needs

INTRODUCTION: There are various surgical interventions to manage osteoporotic vertebral compression fracture. Modular spine block (MSB) is a novel intravertebral fixator that can be assembled. This study aimed to quantitatively investigate the force distribution in vertebrae with the various structural designs and implantation methods by finite element analysis (FEA).
METHODS: A three-dimensional nonlinear FEA of the L3 implanted with MSB was constructed. Different structural designs (solid vs. hollow) and implantation methods (three-layered vs. six-layered and unilateral vs. bilateral) were
