Airway Obstruction in Primary Care Patients: Need for Implementing Spirometry Use.

(1) Background: To detect early airway obstruction in an adult primary care setting. (2) Methods: Seventeen general practitioners (GP) were involved. A total of 912 patients consulting their GPs over 40 years were recruited: 583 of them (323M) agreed to perform/undergo all the procedures: respiratory questionnaire, mMRC questionnaire, and spirometry. We identified four subgroups: physician COPD patients; physician asthma patients; asthma-COPD overlap syndrome patients; and no respiratory diagnosis subjects, on the basis of physician diagnosis. For screening purposes, an FEV1/FVC < 70% was cons

Incentive spirometry to prevent acute pulmonary complications in sickle cell diseases.

BACKGROUND: This study was designed to determine the incidence of thoracic bone infarction in patients with sickle cell diseases who were hospitalized with acute chest or back pain above the diaphragm and to test the hypothesis that incentive spirometry can decrease the incidence of atelectasis and pulmonary infiltrates. METHODS: We conducted a prospective, randomized trial in 29 patients between 8 and 21 years of age with sickle cell diseases who had 38 episodes of acute chest or back pain above the diaphragm and were hospitalized. Each episode of pain was considered to be an independent eve

Risk Factors, Morbidity, and Mortality in Association With Preserved Ratio Impaired Spirometry and Restrictive Spirometric Pattern: Clinical Relevance of Preserved Ratio Impaired Spirometry and Restrictive Spirometric Pattern.

BACKGROUND: Preserved ratio impaired spirometry (PRISm) and restrictive spirometric pattern (RSP) are often considered interchangeable in identifying restrictive impairment in spirometry. RESEARCH QUESTION: Do PRISm and RSP have different individual associations with risk factors, morbidity, and mortality? STUDY DESIGN AND METHODS: In a cross-sectional and longitudinal study, including 26,091 Norwegian general population men (30 to 46 years of age), we explored the association of PRISm and RSP with smoking habits, BMI, education, respiratory symptoms, self-reported cardiopulmonary disease, a

Spirometry and impulse oscillometry in the diagnosis of cough variant asthma in children.

OBJECTIVE: This study aimed to assess the diagnostic utility of spirometry and impulse oscillometry, focusing on their correlation and diagnostic advantages, in children with cough variant asthma (CVA). METHODS: This study included children aged 5-12 years with a diagnosis of CVA who underwent sequential IOS and spirometry pulmonary function with bronchodilation (BD) tests. A control group of healthy children was matched. Receiver operating characteristic (ROC) curves were plotted, and the area under the curve (AUC) was calculated to assess the discriminatory potential of these spirometry and

Assessing diagnostic accuracy for asthma with home spirometry in primary care.

The aim was to evaluate the diagnostic accuracy of lung function measurements for asthma in primary care, including trial treatment. Undiagnosed patients seeking care for asthma-like symptoms were assessed at primary healthcare centers in Sweden. Participants underwent remote or in-clinic spirometry with bronchodilator responsiveness testing (BDR), remote diurnal variability testing of forced expiratory volume in 1 s (FEV1) and peak expiratory flow (PEF) over 2-4 weeks using a home spirometry system; and if necessary, three-months trial treatment with inhaled corticosteroids. Overall, 71/123 (

Oscillometry findings in altered spirometry patterns and relationship to respiratory symptoms.

BACKGROUND: Preserved ratio impaired spirometry (PRISm) and restrictive spirometry pattern (RSP) are associated with increased morbidity and mortality, but the underlying mechanisms are unclear. Impulse oscillometry (IOS) measures small airway function more sensitively than spirometry. We aim to study the prevalence of abnormal IOS findings in relation to three spirometry patterns (obstructive, PRISm and RSP), and their associations with respiratory symptoms. METHODS: In the Swedish CArdioPulmonary bioImage Study (SCAPIS), 10 360 participants aged 50-64 years underwent IOS and spirometry, and

Postexercise Airway Responses by Spirometry and Oscillometry in Nonathlete and Athlete Adolescents.

BACKGROUND: Exercise-induced bronchoconstriction (EIB) is common among adolescents and athletes. While typically assessed with spirometry, oscillometry may offer complementary insights. This cross-sectional study examined how fractional exhaled nitric oxide (FeNO) and symptoms relate to postexercise airway responses assessed via spirometry and oscillometry, and whether these associations differ between nonathletes and athletes. METHODS: Subsamples from two adolescent cohorts (N = 241; 143 nonathletes, 98 athletes) completed questionnaires, FeNO measurements, and an EIB test using spirometry,

Adherence, reliability, and variability of home spirometry telemonitoring in cystic fibrosis.

INTRODUCTION: Forced spirometry is the gold standard to assess lung function, but its accessibility may be limited. By contrast, home spirometry telemonitoring allows a multi-weekly lung function follow-up but its real-life adherence, reliability, and variability according to age have been poorly studied in patients with CF (PwCF). We aimed to compare real-life adherence, reliability and variability of home spirometry between children, teenagers and adults with CF. METHODS: This real-life observational study included PwCF followed for six months in whom lung function (i.e, forced expiratory v

Airway inflammation and hyperresponsiveness in subjects with respiratory symptoms and normal spirometry.

BACKGROUND: Subjects without a previous history of asthma, presenting with unexplained respiratory symptoms and normal spirometry, may exhibit airway hyperresponsiveness (AHR) in association with underlying eosinophilic (type 2 (T2)) inflammation, consistent with undiagnosed asthma. However, the prevalence of undiagnosed asthma in these subjects is unknown. METHODS: In this observational study, inhaled corticosteroid-naïve adults without previously diagnosed lung disease reporting current respiratory symptoms and showing normal pre- and post-bronchodilator spirometry underwent fractional exha

Prognostic significance of chronic respiratory symptoms in individuals with normal spirometry.

Normal spirometry is often used to preclude airway disease in individuals with unspecific respiratory symptoms. We tested the hypothesis that chronic respiratory symptoms are associated with respiratory hospitalisations and death in individuals with normal spirometry without known airway disease. We included 108 246 randomly chosen individuals aged 20-100 years from a Danish population-based cohort study. Normal spirometry was defined as a pre-bronchodilator forced expiratory volume in 1 s/forced vital capacity ratio ≥0.70. Chronic respiratory symptoms included dyspnoea, chronic mucus hypersecr

Utility of Routine Spirometry Measures for Surveillance of Idiopathic Subglottic Stenosis.

IMPORTANCE: Because of the recurrent nature of idiopathic subglottic stenosis, routine follow-up is necessary for monitoring progression of stenosis. However, no easily accessible, standardized objective measure exists to monitor disease progression. OBJECTIVE: To determine whether peak expiratory flow (PEF) can be used as a reliable and easily accessible biometric indicator of disease progression relative to other validated spirometry measures in patients with idiopathic subglottic stenosis. DESIGN, SETTING, AND PARTICIPANTS: Prospectively collected data on PEF, expiratory disproportion ind

Predictive value of prebronchodilator and postbronchodilator spirometry for COPD features and outcomes.

INTRODUCTION: We compared the predictive value of prebronchodilator and postbronchodilator spirometry for chronic obstructive pulmonary disease (COPD) features and outcomes. METHODS: We analysed COPDGene data of 10 192 subjects with smoking history. We created regressions models with the following dependent variables: clinical, functional and radiographic features, and the following independent variables: prebronchodilator airflow obstruction (PREO) and postbronchodilator airflow obstruction (POSTO), prebronchodilator and postbronchodilator FEV1% predicted. We compared the model performance u

Peripheral airway dysfunction and relationship with symptoms in smokers with preserved spirometry.

BACKGROUND AND OBJECTIVE: Smokers develop respiratory symptoms and peripheral airway dysfunction even when spirometry is preserved. Multiple breath nitrogen washout (MBNW) and impulse oscillometry system (IOS) are potentially useful measures of peripheral airway function but they have not been compared in such subjects. We hypothesized that MBNW and IOS are jointly abnormal in smokers with normal spirometry and that these abnormalities relate to respiratory symptoms. METHODS: Eighty smokers with normal spirometry completed a symptom questionnaire, had ventilation heterogeneity in diffusion (S

Inflammatory Responses, Spirometry, and Quality of Life in Subjects With Bronchiectasis Exacerbations.

BACKGROUND: Bronchiectasis exacerbations are critical events characterized by worsened symptoms and signs (ie, cough frequency, sputum volume, malaise). OBJECTIVES: Our goal was to examine variations in airway and systemic inflammation, spirometry, and quality of life during steady state, bronchiectasis exacerbations, and convalescence (1 week following a 2-week antibiotic treatment) to determine whether potentially pathogenic microorganisms, including Pseudomonas aeruginosa, were associated with poorer conditions during bronchiectasis exacerbations. The efficacy of spirometry as a screening tool in detection of air flow obstruction.

BACKGROUND/OBJECTIVES: In developing countries, spirometry has not been considered a part of routine medical check-up. The purpose of the study was to establish the usefulness of spirometry as a primary screening tool in detecting air flow obstruction (AFO) during routine medical check-up (RMC). METHODS: This was a hospital based, retrospective, non-randomized case series study of 3696 participants, who presented to hospital for routine medical check-up. All subjects were assisted at the Pulmonary Medicine Department, from January 2003 till December 2008 who, having met other inclusion criter
