This report describes a case of central serous chorioretinopathy (CSCR) occurring following cessation of terbinafine treatment. A 51-year-old man presented for a routine ophthalmic examination. He was treated with oral terbinafine for onychomycosis up to 3 months before the presentation. Spectral-domain optical coherence tomography (OCT) showed extrafoveal subretinal fluid in both eyes with small underlying pigment epithelial detachments. Terbinafine is a commonly used antifungal medication. Its side effects, while widely known, are rarely described and can be missed by the medical community. We present a 55-year-old woman who visited her primary care physician with onychomycosis. She started treatment with terbinafine, and 1 week later developed a rash in the left flank that extended to the chest, back, and upper part of lower extremities. BACKGROUND: In recent years, therapy-refractory courses of dermatophytoses have increasingly become the focus of attention. The most frequent pathogens are Trichophyton (T.) rubrum and T. mentagrophytes. In addition to local therapy, first-line treatment includes terbinafine, an allylamine antifungal agent that acts by inhibiting squalene epoxidase and thus interfering with ergosterol synthesis. In refractory cases, terbinafine resistance due to point mutation in the squalene epoxidase gene has been frequently detected.

The authors report an unusual case of multiple onychocryptosis, which developed following treatment of onychomycosis with oral terbinafine. With new growth of the healthy nail plate, the distal aspect of multiple toenails became ingrown with periungual inflammation. This required several minor surgical procedures to alleviate the onychocryptosis. The authors present this case report as a potential complication of oral antifungal therapy.

This article reports on a patient suffering from terbinafine-resistant tinea corporis acquired in Asia. Trichophyton mentagrophytes type VIII was first isolated in India. In the past few years, it has gained clinical relevance by causing terbinafine-resistant tinea corporis and cruris. Therefore, in cases of recalcitrant tinea in persons returning from Asia, systemic itraconazole should be started. Specially formulated itraconazole enables high bioavailability despite lower dosage.

Oral terbinafine is a widely used antifungal agent. Adverse effects occur in about 11% of patients taking terbinafine. A rare case of the exacerbation of pre-existing psoriasis after oral terbinafine treatment is reported. A 63-year-old male patient with stable psoriasis vulgaris developed extensive tinea cruris. For this reason, he was treated with oral terbinafine 250 mg/day and topical 1% terbinafine cream. After 6 days of this therapy, a widespread flare-up of psoriasis developed. Oral terbinafine was discontinued, and the psoriasis was treated with topical corticosteroids and tacalcitol.

The medical literature reveals numerous reports of transient taste disturbance associated with the use of oral terbinafine. A review of these reports, however, fails to confirm taste disturbances by formal taste testing. In this article, a case of long-standing taste dysfunction in a patient who exhibited normal formal taste thresholds is described.

A 23-year-old Swedish Caucasian man presented with typical clinical signs of black piedra of his scalp after his return from 4-months' stay in India. There were black nodules around the hair shafts, and the crushed nodules revealed numerous asci and ascospores on microscopy. Piedraia hortae was isolated from the concretions. He was treated with oral terbinafine 250 mg daily for 6 weeks. At the end of treatment no nodules were visible, but 16 days later a few 'new' black concretions appeared. Microscopy of these nodules revealed markedly degenerated fungal elements, and cultures were negative.
