Controversies Surrounding the Pathophysiology of Tics.

Tics are sudden, rapid, recurrent, nonrhythmic motor movements or vocalizations (phonic productions) that are commonly present in children and are required symptoms for the diagnosis of Tourette syndrome. Despite their frequency, the underlying pathophysiology of tics/Tourette syndrome remains unknown. In this review, we discuss a variety of controversies surrounding the pathophysiology of tics, including the following: Are tics voluntary or involuntary? What is the role of the premonitory urge? Are tics due to excess excitatory or deficient inhibition? Is it time to adopt the contemporary ver

[Neuroradiological changes by suppression of tics].

Tourette's syndrome is characterized by involuntary tics. First choice of treatment has been pharmacological, but recently, behavioural therapy teaching patients to suppress their tics has been introduced. Neuroimaging studies have shown an increased activity in the prefrontal cortex, temporal lobes and caudate nucleus, and a decreased activity in globus pallidus and putamen during inhibition of tics. The activity in the frontal lobes changes with age, probably caused by a lack of compensatory hypertrophy. In order to fully understand the mechanism behind behavioural therapy further studies ar

Acute Worsening of Tics on Varenicline.

OBJECTIVE: The aim of this study was to report worsening of Tourette syndrome (TS) in 2 patients treated with varenicline. BACKGROUND: Abnormal dopaminergic signaling is likely involved in the pathophysiology of TS. Varenicline is a partial α4β2 nicotinic acetylcholine agonist that enhances dopamine release. Therefore, the use of varenicline may influence tics in patients with TS. METHOD: We analyzed and described 2 case studies on patients with significant worsening of tics after treatment with varenicline. RESULTS: Patient 1 had motor tics in childhood, which completely resolved by the ag

Late-Onset Psychogenic Chronic Phonic-Tics.

BACKGROUND: Tics beginning in late adulthood often have an identifiable etiology. Psychogenic tics with onset around 60 years of age are rarely described in the literature. CASE REPORT: A 67-year-old female had experienced phonic tics for 8 years. Episodes occurred without premonitory sensations and precipitant factors, and she could not suppress them. She had no history of childhood tic disorder, and secondary causes of tics were excluded. She was diagnosed with psychogenic tics and treated with quetiapine with mild improvement. DISCUSSION: When physicians are faced with no identifiable cau

Therapeutic Developments for Tics and Myoclonus.

Tics and myoclonus are phenomenologically similar given that both are jerk-like movements, but, in contrast to myoclonus, tics are often preceded by premonitory sensations and are typically associated with a variety of behavioral comorbidities, including attention deficit and obsessive-compulsive disorder. There are many other clinical features that help differentiate these two hyperkinetic disorders. Whereas behavioral and antidopaminergic therapies are most effective in the management of tics, clonazepam, other anticonvulsants, and serotonergic drugs are often used to control myoclonic movem

Treatment of tics and tourette syndrome.

Tics come in a variety of types and frequencies; have a waxing and waning course; are exacerbated by stress, anxiety, and fatigue; and often resolve or improve in the teenage or early adult years. Tourette syndrome requires the presence of chronic, fluctuating motor and phonic tics. In addition to tics, individuals with Tourette syndrome often have a variety of comorbid conditions such as attention deficit hyperactivity disorder (ADHD), obsessive-compulsive disorder, depression and anxiety, episodic outbursts, and academic difficulties. These conditions often are a greater source of difficulty

Pimozide for tics in Tourette's syndrome.

BACKGROUND: Neuroleptic drugs with potent D-2 receptor blocking properties have been the traditional treatment for tics caused by Tourette Syndrome. Pimozide is the most studied of these. Use of these medications is declining because of concerns about side effects, and new atypical neuroleptics are now available. The true benefit and risks associated with pimozide compared to other drugs is not known. OBJECTIVES: To evaluate the efficacy and harms of pimozide in comparison to placebo or other medications in the treatment of tics in Tourette Syndrome. SEARCH STRATEGY: We cross-referenced pimo

Adult onset tics after peripheral injury.

We describe a case with adult onset motor tics after peripheral trauma. A 43-year-old man suffered a left shoulder dislocation during a motorcycle accident 21 years ago. Within 2 weeks after the injury, he noticed the gradual onset of involuntary jerking movements of his left shoulder, which was markedly exacerbated after second left shoulder injury 2 years later. The involuntary movements are phenomenologically identical to tics typically associated with Tourette syndrome (TS), but without the involvement of any other body part and without phonic tics or the typical TS co-morbidities, such as

Behavioral problems and severity of tics.

This study examined the incidence of behavioral symptoms in youngsters with tic disorders. A total of 186 patients were studied using the Child Behavior Checklist and a Motor/Vocal Tic Severity Scale. The relationship between tic severity and the frequency of behavioral symptoms was found to be not simply linear, but influenced by the presence of patients without significant behavioral problems in spite of severe tics (resilient patients) and patients with very negative adjustment although their tic disorder was mild (vulnerable patients). Behavioral problems were not related to age. Patients

The coexistence of tics and dystonia.

We studied nine patients with motor and phonic tics and other features of Tourette's syndrome, who developed persistent dystonia in addition to their tics. All, except one, were males (mean age, 35.8 years; range, 8 to 59 years), and had onset of tics prior to age 18 years (mean age, 9 years; range, 1.5 to 17 years). None of the patients were treated with neuroleptic drugs prior to the onset of dystonia. Torticollis and blepharospasm were the most common forms of dystonia. Seven patients had a history of tics in first degree relatives. While these patients were seen in a specialized movement d

Tourette's syndrome. Tics, jerks, and quirks.

Over the past 20 years, public and physician awareness of tic disorders, including Tourette's syndrome, has increased remarkably. In fact, Tourette's syndrome is now "diagnosed" by many families before they consult a physician. Patients seek help not only for the involuntary movements and noises that characterize the illness but also for the accompanying learning, behavioral, and social problems. The authors of this article define the various tic disorders and discuss diagnosis and treatment of the most severe type, Tourette's syndrome.

[From movements to tics: neurophysiologic aspects].

The definition and classification of tics is still based on clinical data. Furthermore, in Gilles de la Tourette syndrome, organogenetic and psychogenetic determinants are still opposed and the true automatic involuntary nature of tics is still debated. Neurophysiological assessments can provide objective data, useful in analysing the physiopathology of tics. Simple tics consist of short bursts of EMG activity (lasting usually less than 100 ms) whereas complex tics consist of prolonged EMG activity. In both cases, the reciprocal activation of antagonist muscles may be present or absent. EEG re

[Tics: from Itard to the neuroleptics].

When the six DSM-III (1980) diagnostic criteria are applied to the nine cases reported by Gilles de la Tourette in 1885, six of them are found to be in accordance with the diagnosis of Gilles de la Tourette's syndrome (cases nos 4, 5 and 7 do not involve vocal tics). Gilles de la Tourette deserves credit, not only for having regrouped fragmented observations into one remarkably well described clinical entity which held over time (such as Itard's observations nos 9 and 10 in 1825; the latter is the famous Marquise of D ... seen several times by Charcot and the only one which, along with no 1, a

Secondary tics: Etiology, presentation, and management.

Tics are involuntary or semivoluntary, abrupt, brief, nonrhythmic, recurrent movements or sounds. Tourette syndrome (TS) is the most common cause of tics; however, several other disorders have been associated with tics and tic-like movements. Etiologies of secondary tic syndromes and disorders (STS) include hereditary, drug-induced (including tardive), toxins, traumatic, cerebrovascular, infectious, parainfectious, autoimmune, functional disorders, and others. Age at onset after 18 years, lack of comorbid attention-deficit/hyperactivity disorder, obsessive-compulsive disorder, and no family hi

Oromandibular tics associated with Tourette syndrome.

BACKGROUND: Tourette syndrome (TS) is the most common cause of chronic tics. Patients with TS frequently manifest motor tics involving the eyes and face but oromandibular (OM) tics have been rarely studied. MATERIALS AND METHODS: We reviewed the medical records and video-recordings of 155 consecutive patients with TS in our movement disorders clinic. In addition, we studied 35 patients with classic tardive dyskinesia (TD) and compared their clinical and demographic features with those with TS. RESULTS: We identified 41 patients with OM tics (26.5%). Although patients with OM tics had a great

Neurodevelopmental Disorders: Sensing Tourette's Tics Away.

Though still shrouded in mystery, Gilles de la Tourette's syndrome is widely regarded as an archetypal neurodevelopmental disorder of central, motor control. New evidence that its cardinal manifestation - prominent tics - may be ameliorated by a peripheral, sensory intervention compels us to revise not only our conception of the syndrome, but of the motor system itself.

[Tics and Tourette's syndrome in children].

The article represents a review of modern literature on tics and Tourette's syndrome in paediatric population and describes recent advances in neurogenetics, neuroanatomy and neurotransmitter heterogeneity of the disease pathogenesis. The analysis of the literature supports the plausibility of transformation of tics and Tourette's syndrome from psychiatric disorders into neurological disorders (according to ICD-11) based on recent data on neurophysiology and functional neuroanatomy. Neuroanatomy of motor behaviour is described as the most complex self-regulating neural network of thalamo-corti
