Adequate Catchment Area Representation in Cancer Clinical Trials at NCI Designated Cancer Centers: The University of California Irvine Experience.

BACKGROUND: Ensuring that clinical trial enrollment reflects the demographics and disease burden of a cancer center's catchment area is essential for improving trial accrual and the generalizability of research findings. We hypothesized that aligning the clinical trial portfolio with the needs of the catchment area can enhance enrollment and access to novel therapeutics. METHODS: A retrospective cohort study was conducted at the University of California Irvine Chao Family Comprehensive Cancer Center (CFCCC), an National Cancer Institute-designated cancer center serving Orange County (OC), Cal

Flow experience fosters university students' well-being through psychological resilience: A longitudinal design with cross-lagged analysis.

BACKGROUND: Existing research has linked individuals' flow experience - a positive affective and cognitive state of deep immersion and engagement in daily activities - and their well-being, particularly among university students. A growing number of longitudinal studies have further contributed to this understanding. However, limited attention has been given to exploring the dynamic interplay between these two variables and their underlying mechanisms (i.e., the mediator) of psychological resilience, specifically among university students in the context of the COVID-19 pandemic. AIMS: To addr

Guidelines in Designing a Universal Primer Mixture to Probe and Quantify Antibiotic-Resistant Genes Using the Polymerase Chain Reaction (PCR).

Multidrug resistance efflux pumps (MDREPs) in biofilm communities have become an increasingly expensive problem in clinical settings. Polymerase chain reaction (PCR)-based detection can be used to diagnose and characterize these genes, but this requires effective primer design to minimize false positives and negatives in test conclusions. A universal primer approach has previously been used to detect conserved core genes but not for accessory genes such as MDREPs. This study describes a guideline for the design of primers used in the detection of MDREP genes and an optimization approach for cr

The epidemiology of Helicobacter pylori resistance in a university affiliated hospital: a comparison between two time periods-a cross-sectional design.

PURPOSE: During the last few decades, the increased use of various types of antibiotics in the general population caused a significant change in regional Helicobacter pylori (H. pylori) antibiotic resistance. Our aim is to study the changes in H. pylori resistance in patients who had undergone an esophagogastroduodenoscopy (EGD) and susceptibility testing and found positive for H. pylori. The study was conducted in a university affiliated hospital between 2013-2020. METHODS: A cross-sectional study was performed on all consecutive patients who had undergone an EGD and tested positive for H. p

Risk Alert System to Facilitate Universal Suicide Risk Screening in Emergency Care: Protocol for a Prediction Model and Software Development Study Using a Co-Design Approach.

BACKGROUND: Suicide is a leading cause of preventable mortality and a major contributor to years of life lost. Many people who later die by suicide present to emergency departments in the months before death, often for reasons not explicitly related to self-harm and without receiving a risk assessment. Universal suicide risk screening in emergency care can improve detection but is difficult to implement because of time constraints, workflow disruption, and limited capacity to respond to increased identification. OBJECTIVE: The Clinical Risk Alert System for Suicide Risk Screening in Emergency

Rationale and design of the Virginia Commonwealth University-Anakinra Remodeling Trial-3 (VCU-ART3): A randomized, placebo-controlled, double-blinded, multicenter study.

There is clear association between the intensity of the acute inflammatory response during acute myocardial infarction (AMI) and adverse prognosis after AMI. Interleukin-1 (IL-1) is a pro-inflammatory cytokine released during AMI and involved in adverse remodeling and heart failure (HF). We describe a study to evaluate the safety and efficacy of IL-1 blockade using an IL-1 receptor antagonist (anakinra) during the acute phase of ST-segment elevation myocardial infarction (STEMI). Rational Design of a Potent Two-Phage Cocktail Against a Contemporary Acinetobacter baumannii Strain Recovered from a Burned Patient at the Lausanne University Hospital.

Acinetobacter baumannii is a critical public health threat, particularly with the rise in multidrug-resistant (MDR) and extensively drug-resistant (XDR) strains that limit treatment options. Phage therapy, which uses bacteriophages to target bacteria, offers a promising alternative. We isolated an XDR strain (Ab125) from a burn wound infection and screened 34 phages, identifying vB_AbaM_3098 as the only effective candidate. However, resistance rapidly emerged, producing a derivative strain (Ab139). Interestingly, Ab139, though resistant to vB_AbaM_3098, became susceptible to six previously ina

Using a Multicomponent Implementation Strategy to Increase Adoption and Effectiveness of a Universal Mental Health Prevention Program in Australian Primary Schools: a Cluster Randomized Trial Using a Type-3 Hybrid Design.

It has been demonstrated in multiple randomized trials that the PAX Good Behavior Game is an effective school-based program for reducing the severity and progression of childhood emotional and behavioral problems that are prognostic of mental disorders, personality disorders, and self-harm behavior in adolescence and early adulthood. Embedding effective programs into routine practices of frontline settings is a global priority for mental health prevention, however, little is known about what strategies effectively support program implementation in schools. The purpose of this study was to test

Exploring the rhodanine universe: Design and synthesis of fluorescent rhodanine-based derivatives as anti-fibrillar and anti-oligomer agents against α-synuclein and 2N4R tau.

Tau and α-synuclein (α-syn) are prone-to-aggregate proteins that can be responsible for pathological lesions found in the brains of Alzheimer's disease (AD), Lewy body dementia (LBD), and Parkinson's disease (PD) patients. The early-stage oligomers and protofibrils of tau are believed to be strongly linked to human cognitive impairment while the toxic α-syn oligomers are associated with behavioral motor deficits. Therefore, concurrent targeting of both proteinaceous aggregates and oligomers are very challenging. Herein, rhodanine-based compounds were designed and synthesized to target the fibr

Universal versus conditional three-day follow up visit for children with uncomplicated fever at the community level: design of a cluster-randomized, community-based, non-inferiority trial in Tanganyika, Democratic Republic of Congo.

BACKGROUND: The current recommendation within integrated Community Case Management guidelines that all children presenting with uncomplicated fever and no danger signs be followed up after three days may not be necessary. Such fevers often resolve rapidly (usually within 48-96 h), and previous studies suggest that expectant home care for uncomplicated fever can be safely recommended. We aim to determine the non-inferiority of a conditional versus a universal follow-up visit for these children. METHODS: We are conducting a cluster-randomized, community-based, non-inferiority trial enrolling ~4

Open pilot study of a guided digital self-help intervention targeting sleep and the biological clock in university students using a pre-test post-test design.

Insomnia is common among university students and is associated with mental health problems. Students often have irregular day/night rhythms, which contribute to circadian rhythm disruptions. This open pilot study investigated the feasibility, acceptability, and preliminary effectiveness of a 5-week digital guided self-help intervention in university students with self-reported insomnia. The intervention is based on cognitive behavioural therapy for insomnia with specific emphasis on the biological clock ('i-Sleep & BioClock'). We assessed feasibility and acceptability.
