Is there a role for urodynamics in chronic nonbacterial prostatitis?

The etiology of chronic nonbacterial chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is unclear. That may be why treatment with antibiotics and anti-inflammatory drugs often fail. The use of urodynamic testing in evaluating any patient with both pelvic/perineal pain coexistent with voiding symptoms can help identify voiding dysfunction. If identified, urodynamic voiding disorders, such as bladder neck obstruction and pseudodyssynergia, should be specifically treated to ameliorate symptoms. Through more research of nonbacterial CP/CPPS, we will be able to further define the successfu

[Effects of ureteral stents on rabbit upper urinary tract urodynamics].

OBJECTIVE: To explore the effects of ureteral stent placement on ureteral peristalsis and the time course of renal pelvic pressure changes. METHOD: Forty rabbits were randomized equally into 8 groups, including a control group without placement of ureteral stents. The other 7 groups received ureteral stent placement, which were retained for 0, 1, 2, 3, 4, 7, and 12 weeks respectively. When the rabbits were bred for the specified time periods, ureteral peristalsis was observed and renal pelvic pressure measured. RESULT: The renal pelvic pressure was initially increased after the stent placeme

Long-term urodynamics followup of bladder augmentation for neurogenic bladder.

PURPOSE: Augmentation enterocystoplasty is well tolerated by patients with neurogenic bladder in whom conservative therapy has failed. However, few studies exist on long-term urodynamic evaluation of these patients. We assessed the clinical and urodynamic outcomes of patients with neurogenic bladder treated with augmentation enterocystoplasty with at least 4 years of followup. MATERIALS AND METHODS: A total of 26 patients with neurogenic voiding dysfunction underwent augmentation enterocystoplasty alone or in conjunction with various continence or antireflux techniques. Clinical outcomes rega

Normal urodynamics in patients with bladder exstrophy: are they achievable?

PURPOSE: Urodynamic study was performed in patients with exstrophy to determine the effect of bladder neck reconstruction and the ability to achieve normal urodynamics following surgery. MATERIALS AND METHODS: A total of 30 exstrophy cases at different stages of reconstruction were retrospectively reviewed in terms of bladder capacity, compliance, stability and presence of detrusor contractions following urodynamic study. RESULTS: Bladder capacity increased from a third predicted volume for age to half after reconstruction. Approximately 80% of patients had compliant and stable bladders befo

Urodynamics in women with stress incontinence before and after surgery.

Pad test, cystometry and analysis of micturition were performed in 36 women with stress incontinence before and a year after operation with either retropubic urethrocystopexy (n = 22) or pubococcygeal repair (n = 14). There was no difference in the subjective cure rate between the two groups of women (77% and 79%, respectively). The pad test 1 year after operation showed that 59% of the women in the urethrocystopexy group and 43% of the women in the pubococcygeal repair group had stopped leaking urine. The bladder volume had increased in both groups and the intravesical pressure of the bladder

Are urodynamics necessary in female patients presenting with stress incontinence?

A urological and urodynamic database was used in a prospective study of female patients whose initial symptom was stress incontinence alone. The urodynamic diagnoses in 89 such patients were analysed against any additional symptoms. Fifty-four patients complained of stress incontinence without other symptoms and none of these had detrusor instability. One patient from a group of 25 who had frequency with stress incontinence had low bladder compliance but the others had normal urodynamic studies. Of the 10 patients with urgency in addition to stress incontinence, 5 had unstable bladders. A deta

Role of urodynamics in management of urethral diverticulum in females.

This report deals with 10 female patients with urethral diverticula, 8 of whom were also found to have findings of stress incontinence. This association was proved by urodynamic studies. Three patients were seen with incontinence postdiverticulectomy. Seven patients were evaluated prior to diverticulectomy, and of these 5 had anatomic changes of stress urinary incontinence. Those patients in whom the preoperative evaluation considered them to be at risk for development of stress incontinence postdiverticulectomy were treated with a prophylactic urethropexy. Patients so treated were continent a

Impact of ethnicity on urodynamic outcomes.

The aim of this study was to assess the impact of ethnicity on urodynamic outcomes. The urodynamic studies (UDS) of 100 women were analysed; 41 were of white Caucasian origin, 35 of Asian origin and 24 of African origin. The presence of detrusor overactivity (DOA) in the three ethnic groups was compared and the nature of DOA in terms of cystometric capacity, volume of strong desire, maximum flow rate (Qmax), volume at which DOA occurred, mean amplitude of contraction and number of contractions, were compared. Asian (p < 0.003) and African (p < 0.008) women were more likely to have a diagnosis

[An urodynamic study on neobladder function].

OBJECTIVE: To analyze the urodynamic characteristics of neobladders, we conducted a pressure-flow study in patients with orthotopic urinary reservoirs. PATIENTS AND METHODS: From 1986 to 1996, 90 patients underwent bladder replacement following cystectomy, using a right colonic, ileocolic, ileal, or sigmoid colonic segment. The subjects were 38 patients (31 men and 7 women) with stable urination and no evidence of cancer recurrence, urethral stricture, urinary tract infection or vesicoureteral reflux. Their mean age was 60.5 years, with a range of 38 to 77 years. Information on neobladder fun

Urodynamic evaluation of hypospadias repair.

PURPOSE: We performed a cross-sectional evaluation of voiding in a population undergoing hypospadias repair to determine whether patients had urinary obstruction at various intervals of followup after the last operation. MATERIALS AND METHODS: Of approximately 600 patients undergoing hypospadias repair at our department during a 30-year interval 175, 40 months to 66 years old were evaluated. Therefore, we created a cross-sectional study group for evaluation of voiding function. All patients had undergone the final operation for hypospadias at least 1 year previously and were toilet trained. S

Urodynamic evaluation of stress incontinence.

A careful history points the urodynamic examination in the right direction and enables the examiner to ask the correct questions. The individual who does the test is the only reliable interpreter of the results of that study. No urodynamic technique is as sensitive or specific as a blood glucose, or even an electrocardiogram. A history of urgency and urge incontinence suggests uninhibited contractility and is a better index of that condition than a cystometrogram. Leakage occurring shortly after a previous operative procedure for stress incontinence suggests type III stress incontinence. A pas

Urodynamic evaluation of female incontinence.

Several diseases of the female bladder are accompanied by stress incontinence or some other form of incontinence. By using micturition cystourethrography, visualizing the bladder in lateral views, with simultaneous recording of micturition pressure and flow, a distinction may be made between the individual diseases, as they often exhibit characteristic radiological configurations and urodynamic patterns. It is also possible, by comparing with measurements in control material, to decide the best physiological method of abolishing the incontinence. This method of examination has proved particula

Antibiotic prophylaxis in urodynamics: A systematic review of secondary studies.

Urodynamics is the functional study of the urinary tract and an invasive diagnostic test that involves inserting a catheter into the bladder to assess bladder-related symptoms. One major risk is the development of urinary tract infections (UTI) following the procedure. Currently, there is no global consensus on the use of antibiotic prophylaxis. This study aims to identify key recommendations regarding prophylactic antibiotic therapy associated with urodynamic procedures. A systematic review of clinical practice guidelines (CPG), systematic reviews, and meta-analyses was conducted with subsequ

From Clinical Trials to Routine Practice: Are Urodynamics Still Useful?

The UPSTREAM and FUTURE trials found no added benefit from routine urodynamics (UDS) in men with presumed benign prostatic obstruction or women with overactive bladder symptoms unresponsive to initial therapy. However, these population-based studies were not powered to evaluate specific subgroups for which a higher degree of diagnostic certainty could have led to better outcomes. This consensus paper argues that UDS is essential in selected cases for which comprehensive clinical assessment alone may be insufficient. A selective approach to UDS will ensure that patients receive the most accurat

Prediction of autonomic dysreflexia during urodynamics: a prospective cohort study.

BACKGROUND: Autonomic dysreflexia is a severe and potentially life-threatening condition in patients with spinal cord injury, as it can lead to myocardial ischemia, brain hemorrhage, or even death. Urodynamic investigation is the gold standard to assess neurogenic lower urinary tract dysfunction due to spinal cord injury and reveal crucial pathological findings, such as neurogenic detrusor overactivity. However, neurogenic detrusor overactivity and urodynamic investigation are known to be leading triggers of autonomic dysreflexia. Therefore, we aimed to determine predictors of autonomic dysref
