Semi-degradable biomimetic double-layer small diameter vascular graft for arteriovenous fistula in large animals.

A double layer small diameter vascular graft (SDVG) was fabricated by sequential electrospinning. The inner layer was composed by three kinds of biodegradable poly (ε-caprolactone) (PCL) fibers with three different molecular weights to induce endothelial regeneration and the outer layer comprised non-degradable thermoplastic polyurethane (TPU) fibers and PCL fibers with the numerical molecular weight of 80,000 g/mol to provide long-term mechanical support. The SDVG was further heparinized through "erosion and graft" strategy. The surface heparin content, the clotting time and the mechanical pr

Systemic vascular resistance predicts high-output cardiac failure in patients with high-flow arteriovenous fistula.

AIMS: Patients with high-flow arteriovenous (AV) access are at risk of developing high-output cardiac failure (HOCF) and subsequent hospitalization. However, diagnosing HOCF is challenging and often requires invasive procedures. The role of systemic vascular resistance (SVR) in diagnosing HOCF is underestimated, and its predictive value is limited. Our study aims to identify non-invasive risk factors for HOCF to facilitate early diagnosis and timely surgical interventions. METHODS AND RESULTS: We included 109 patients with high-flow AV access who underwent serial echocardiography. The retrosp

Aorto-bronchial fistula masquerading as haematemesis: a rare late complication of thoracic aortic vascular grafts.

Aorto-bronchial fistula is an exceedingly rare pathology with high mortality. Late vascular graft infection may occur secondary to haematogenous seeding of bacteria from a distant source such as gastrointestinal infection. We present an unusual case of aorto-bronchial fistula masquerading as haematemesis in a patient with sigmoid diverticulitis, and review the pathophysiology, diagnosis, surgical and endovascular management of aorto-bronchial fistulas.

Congenital anomalous/aberrant systemic artery to pulmonary venous fistula: closure with vascular plugs & coil embolization.

A 7-month-old girl with failure to thrive, who, on clinical and diagnostic evaluation [echocardiography & CT angiography] to rule out congenital heart disease, revealed a rare vascular anomaly called systemic artery to pulmonary venous fistula. In our case, there was dual abnormal supply to the entire left lung as(1) anomalous supply by normal systemic artery [internal mammary artery](2) and an aberrant feeder vessel from the abdominal aorta. Left Lung had normal bronchial connections and normal pulmonary vasculature. The fistula drained through the pulmonary veins to the left atrium leading t

Amplatzer vascular plug closure of a subclavian arteriovenous fistula associated with implantable cardioverter-defibrillator implantation.

Subclavian arteriovenous fistula (AVF) is a rare complication of implantable cardioverter-defibrillator (ICD) implantation. This report illustrates the case of a subclavian AVF after ICD implantation in a patient with nonischemic cardiomyopathy and the successful closure of this AVF using an Amplatzer vascular occlusion plug. Given the increasing number of ICD implantations, physicians should be made aware of this uncommon complication. In addition, as more percutaneous endovascular techniques are developed to treat peripheral vascular and structural heart disease, iatrogenic AVFs may occur mo

A novel technique of vascular anastomosis to prevent juxta-anastomotic stenosis following arteriovenous fistula creation.

OBJECTIVES: Juxta-anastomotic stenosis (JAS) is one of the predominant causes of arteriovenous fistula (AVF) failure, with the reported incidence as high as 65%. We hypothesized that technical modification to alter the outflow vein configuration using the novel piggyback Straight Line Onlay Technique (pSLOT) would prevent JAS and improve AVF maturation. METHODS: Intention-to-treat analysis of the outcomes of consecutive distal radiocephalic (RC) fistulas performed by a single operator with three different anastomotic techniques using a prospectively maintained database. Traditional end-to-sid

[Endoscopic treatment of subcutaneous fat necrosis secondary to pancreatic-vascular fistula: apropos of a case].

PATIENT AND METHOD: We report the case of a 68-year-old man who presented a pancreatic tumor with a pancreato-vascular fistula and a Weber-Christian syndrome. Pancreatic enzymes levels at the admission were high: amylasemia 2,470 IU/L (N < 110) and lipasemia 11,700 IU/L (N < 220). The treatment consisted in total parenteral nutrition and somatostatin (100 micrograms x 3/day). Because we noted neither clinical nor biological improvement after 10 days of treatment, we performed an endoscopic retrograde pancreatography. During this examination, we put a 7 French diameter prosthesis through the Wi

[Left subclavian-pulmonary trunk fistula with a vascular prosthesis as palliative treatment in Fallot conditions].

In this paper, our experience with a new systemic-to-pulmonary artery shunt: subclavian-pulmonary artery trunk shunt with PTFE (central shunt) is presented. Between November 1985 and March 1990 this central shunt was employed in 8 children with ages ranged between 4 days and 3 years, and weights between 2 and 12 kg. Diagnosis were Fallot's tetralogy in three; pulmonary atresia with intact septum in three; complete AV canal and Fallot's tetralogy in one, and univentricular heart and pulmonary atresia in 1 patient. There were no surgical deaths. Hospital mortality was present in 1 case (AV canal

Multidisciplinary management of bronchoesophageal fistula using adipose-derived stromal vascular fraction and platelet-rich plasma.

BACKGROUND AND AIMS: Bronchoesophageal fistula (BEF) is a rare but life-threatening adverse event following surgery. Fistulas result in respiratory adverse events, often requiring surgical repair, which is a high-risk procedure. Therefore, minimally invasive alternatives are needed. We aimed to use adipose-derived stromal vascular fraction (SVF) to treat BEFs. METHODS: We present 2 BEF cases after minimally invasive esophagectomy and gastric conduit reconstruction. During the procedure, SVF was isolated and platelet-rich plasma (PRP) obtained. In 1 case with a tracheaesophageal fistula, simul

Impact of Previous Tunneled Vascular Catheters and their Location on Upper Limb Arteriovenous Fistula Function.

BACKGROUND: Long-term arteriovenous fistula (AVF) survival has been shown to be adversely affected by the presence of previous tunneled vascular catheters (TVC). We analyzed the effect of previous TVCs and their location (ipsilateral versus contralateral) on the successful function of upper-limb AVFs in the first 12 months after creation. METHODS: We retrospectively reviewed clinical data on patients' first upper-limb AVFs, created between January 2013 and December 2017. We analyzed the rates of successful AVF function (successful cannulation using two needles for ≥50% sessions over a 2-week

Amplatzer vascular plug IV in the treatment of high flow renal arteriovenous fistula: Case considerations.

Renal arteriovenous fistula is classified into idiopathic, acquired and congenital. Endovascular therapy has become the gold standard compared to surgery. We present the embolization procedure of a renal fistula postbiopsy in a 57-year-old female patient through the use of St. Jude Medical Amplatzer vascular Plug IV. Although numerous embolizing agents are available, plug type IV has numerous advantages in terms of procedure times, speed of embolization and precision of the occlusion, but with a relative increase in costs.

Late-presenting left internal mammary to great cardiac vein fistula treated with a vascular plug.

A 73-year-old patient, who underwent triple bypass surgery in another hospital in 1991 after suffering an acute myocardial infarction, was admitted to our institution after an episode of unstable angina. Evidence of a left internal mammary artery grafted to the great cardiac anterior interventricular vein (GCV), with Qp:Qs > 1.5:1, was demonstrated by angiography. We report the closure of said fistula using a percutaneous left radial artery access and the deployment of an Amplatzer vascular occlusion device via the antegrade approach.

The Role of High-Frequency Wall Vibrations in Adverse Vascular Remodeling of Arteriovenous Fistula for Hemodialysis.

KEY POINTS: Flow-induced high-frequency vascular vibrations, computed through fluid-structure interaction simulations, are associated with fistula complications. The association between vibrations and arteriovenous fistula complications suggests a potential new mechanism responsible for vascular remodeling. The frequency and amplitude of vibrations are potential biomarkers for arteriovenous fistula surveillance. BACKGROUND: Despite progress in research, the mechanobiological mechanisms behind adverse vascular remodeling and failure in arteriovenous fistula (AVF) for hemodialysis remain unclea

Transcatheter closure of a coronary fistula with an Amplatzer vascular plug: should a retrograde approach be standard?

Coronary artery fistulae are rare disorders. Only 2 reports in the medical literature discuss the use of vascular plugs to occlude coronary fistulae, and the same device-the Amplatzer Vascular Plug-was deployed via different techniques to treat those patients. The safety, the feasibility, and the standard approach to deployment have yet to be established. Herein, we describe the case of a 15-year-old boy who presented with a continuous murmur at the left sternal border. The patient was diagnosed with a large coronary fistula that originated from the right coronary artery and emptied into the r

Vascular access surgery managed by renal physicians: the choice of native arteriovenous fistulas for hemodialysis.

BACKGROUND: After decades of success in dialysis research and treatment, prompt availability of a well-functioning vascular access for dialysis remains a disturbing problem. On the basis of a single-center experience in which nephrologists are responsible for access surgery, we sought to identify predictors of catheter use at the start of hemodialysis (HD) therapy and risk factors affecting first permanent access survival. METHODS: Demographics, comorbid conditions, predialysis follow-up, and access-related procedures of the 197 consecutive patients beginning extracorporeal treatment between
