Early Left Ventricular Involvement Detected by Cardiovascular Magnetic Resonance Feature Tracking in Arrhythmogenic Right Ventricular Cardiomyopathy: The Effects of Left Ventricular Late Gadolinium Enhancement and Right Ventricular Dysfunction.

Background Left ventricular (LV) involvement is common in arrhythmogenic right ventricular cardiomyopathy (ARVC). We aim to evaluate LV involvement in ARVC patients by cardiovascular magnetic resonance feature tracking. Methods and Results Sixty-eight patients with ARVC and 30 controls were prospectively enrolled. ARVC patients were divided into 2 subgroups: the preserved LV ejection fraction (LVEF) group (LVEF ≥55%, n=27) and the reduced LVEF group (LVEF <55%, n=41). Cardiovascular magnetic resonance with late gadolinium enhancement (LGE) and cardiovascular magnetic resonance feature tracking

Invasive Assessment of Right Ventricular Dysfunction Predicts Periprocedural Outcomes in Patients Undergoing Unplanned Ventricular Tachycardia Ablation.

BACKGROUND: Risk stratification in patients undergoing urgent ventricular tachycardia (VT) ablation is limited, particularly regarding right ventricular dysfunction, which is often underrecognized in this setting. OBJECTIVES: We investigated whether the right atrial pressure (RAP) to pulmonary capillary wedge pressure (PCWP) ratio or pulmonary artery pulsatility index (PAPi) predicts short-term outcomes following urgent VT ablation. METHODS: We retrospectively analyzed 102 consecutive patients who underwent unplanned inpatient VT ablation and had right heart catheterization (RHC) within the

Nitric oxide inhalation in the treatment of right ventricular dysfunction following left ventricular assist device implantation.

BACKGROUND: Following left ventricular assist device (LVAD) implantation in end-stage heart failure, the management of right ventricular dysfunction presents a therapeutic problem unresolved by conventional drug therapy (catecholamines, nitrates, and prostacyclin). This study was performed to investigate the effects of supplemental inhalation of nitric oxide (NO), a selective pulmonary vasodilator, postoperatively and prospectively. METHODS AND RESULTS: Intraindividual dose titration of NO was performed (0 to 40 ppm) according to a standardized protocol. Thereafter treatment was continued wit

Association between right ventricular strain and subclinical left ventricular dysfunction in obstructive sleep apnea-hypopnea syndrome.

OBJECTIVE: Using two-dimensional speckle-tracking echocardiography (2D-STE), this study analyzed biventricular strain changes in obstructive sleep apnea-hypopnea syndrome (OSAHS) and explored whether right ventricular (RV) dysfunction statistically mediates OSAHS-related left ventricular (LV) impairment. METHODS: We enrolled 212 OSAHS patients and 52 controls, categorized by apnea-hypopnea index (AHI) into control (n = 52), mild-to-moderate (n = 80), and severe (n = 132) groups. We assessed 2D-STE-derived RV global longitudinal strain (RVGLS), RV free wall strain (RVFWS), and LV global longit

Right ventricular dysfunction in left ventricular assist device candidates: is it time to change our prospective?

The use of left ventricular assist devices (LVAD) has significantly increased in the last years, trying to offer a therapeutic alternative to heart transplantation, in light also to the significant heart donor shortage compared to the growing advanced heart failure population. Despite technological improvements in the devices, LVAD-related mortality is still fairly high, with right heart failure being one of the predominant predictors. Therefore, many efforts have been made toward a thorough right ventricular (RV) evaluation prior to LVAD implant, considering clinical, laboratory, echocardiogr

Late-onset right ventricular dysfunction after mechanical support by a continuous-flow left ventricular assist device.

BACKGROUND: Right heart failure (RHF) is a serious post-operative complication of left ventricular assist device (LVAD) implantation, with significant morbidity and mortality. Many clinical, hemodynamic and laboratory variables have been shown to have prognostic value for appearance of RHF. We sought to investigate the incidence of new-onset right ventricular dysfunction (RVD) complicating the long-term use of LVADs. METHODS: We retrospectively examined all patients supported with a continuous-flow LVAD for >1 year at our center. RESULTS: Twenty patients (mean age 54 ± 10 years, 95% men, 60%

Right ventricular dysfunction in patients undergoing left ventricular assist device implantation: predictors, management, and device utilization.

Nonreversible failure of the right ventricle is seen in 0.04% to 0.1% of postcardiotomy cases. The incidence of right ventricular dysfunction after left ventricular assist device (LVAD) implantation that fails to resolve in the operating room is reported to be as frequent as 20% to 50% and imposes a considerable burden in terms of postoperative morbidity and mortality. Should this syndrome supervene, the mortality of an LVAD operation increases from 19% to 43%. Although most patients can be maintained with prolonged inotropic support, 10% to 15% may require implantation of a separate right ven

Left ventricular conduction delays induced by right ventricular apical pacing: effect of left ventricular dysfunction and bundle branch block.

BACKGROUND: RV apical pacing (RVP) may be deleterious, possibly by simulating LBBB, i.e., prolonging QRS duration (QRSd) and LV activation (LVAT). However, determinants of electrical delays are unknown. HYPOTHESIS: LV dysfunction (LVEF <or= 40%, HF) and pre-existing conduction system abnormalities may modulate RVP's effects, compared to LBBB. METHODS: RVP-induced QRSd and LVAT were compared in normal LV to HF, with normal QRS (<120 ms), RBBB, or LBBB. LVAT was estimated by interval from QRS onset to basal inferolateral LV depolarization. RESULTS: During LBBB and RVP, LVAT/QRSd was >or=85%,

Interventricular septum involvement is related to right ventricular dysfunction in anterior STEMI patients without right ventricular infarction: a cardiovascular magnetic resonance study.

The value of cardiovascular magnetic resonance (CMR) in assessing and predicting acute right ventricular (RV) dysfunction in patients with anterior ST-segment elevation myocardial infarction (STEMI) remains ascertained. Eighty eight patients with anterior STEMI were prospectively recruited and underwent CMR examinations within one week following the coronary intervention. Patients with RV ejection fraction (RVEF) less than 2 standard deviations below the average at the center (RVEF ≤ 45.0%) were defined as having RV dysfunction. The size of infarction, segmental wall motion, and T1 and T2 mapp

Left ventricular dysfunction is related to the presence and extent of a septal flash in patients with right ventricular pacing.

AIMS: Septal flash (SF), a marker of left ventricular (LV) dyssynchrony in the presence of a left bundle branch block (LBBB), has been shown to predict improved ventricular function and outcome when corrected with cardiac resynchronization therapy. We hypothesized that a SF is present in patients receiving right ventricular (RV) pacing and its presence and extent could predict the development of LV dysfunction and remodelling. METHODS AND RESULTS: Seventy-four consecutive patients receiving conventional RV pacing (>6 months, >85% paced) were studied with two-dimensional (2D) echocardiography.

Association of Immunoglobulin E With Right Ventricular Dysfunction in Pulmonary Arterial Hypertension.

Pulmonary arterial hypertension (PAH) is characterized by pulmonary vasoconstriction and vascular remodeling, leading to increased pulmonary vascular resistance, pathological right ventricular (RV) remodeling, and ultimately right heart failure and death. RV function and right ventricle-pulmonary artery (RV-PA) coupling are critical determinants of patient prognosis, and echocardiography remains the preferred tool for assessing RV function and predicting RV-PA coupling. Recent studies have revealed that immunoglobulin E(IgE) plays a regulatory role in cardiac and pulmonary vascular remodeling;

Myeloid Cell Expansion Propels Right Ventricular Dysfunction in HFpEF Through Sterile Inflammation.

BACKGROUND: The role of leukocytes in the pathogenesis of right ventricular (RV) dysfunction (RVD) associated with heart failure with preserved ejection fraction (HFpEF) remains poorly defined, partially due to the lack of suitable small animal models. Here, we followed a translational research approach by establishing a murine HFpEF model developing manifest RVD and analyzed human HFpEF cohorts to study the mechanistic link between leukocytes and RVD in HFpEF. METHODS: Young (<20-week-old) and aged (>80-week-old) male and female C57BL/6J mice were divided into 4 experimental groups: chow, HF

Myocardial work across different etiologies of right ventricular dysfunction and healthy controls.

Evaluating right ventricular (RV) function remains a challenge. Recently, novel echocardiographic assessment of RV myocardial work (RVMW) by non-invasive pressure-strain loops was proposed. This enables evaluation of right ventriculoarterial coupling and quantifies RV dyssynchrony and post-systolic shortening. We aimed to assess RVMW in patients with different etiologies of RV dysfunction and healthy controls. We investigated healthy controls (n=17), patients with severe functional tricuspid regurgitation (FTR; n=22), and patients with precapillary pulmonary hypertension (PCPH; n=20). Echocard

Right ventricular dysfunction in children and adolescents conceived by assisted reproductive technologies.

Assisted reproductive technologies (ART) predispose the offspring to vascular dysfunction, arterial hypertension, and hypoxic pulmonary hypertension. Recently, cardiac remodeling and dysfunction during fetal and early postnatal life have been reported in offspring of ART, but it is not known whether these cardiac alterations persist later in life and whether confounding factors contribute to this problem. We, therefore, assessed cardiac function and pulmonary artery pressure by echocardiography in 54 healthy children conceived by ART (mean age 11.5 ± 2.4 yr) and 54 age-matched (12.2 ± 2.3 yr)

Right ventricular dysfunction and adverse outcome in patients with advanced heart failure.

BACKGROUND: In patients with heart failure (HF) from left ventricular systolic dysfunction, the presence of coexistent right ventricular (RV) dysfunction is associated with poor exercise capacity and reduced survival. We sought to determine whether a simple measure of RV function, the RV myocardial performance index (RV MPI), is associated with adverse outcome in a population of advanced heart failure patients selected to receive cardiac resynchronization therapy (CRT). METHODS AND RESULTS: The RV MPI was measured on 77 consecutive preimplantation echocardiograms. The relationship between RV
