Who needs a tourniquet? And who does not? Lessons learned from a review of tourniquet use in the Russo-Ukrainian war.

BACKGROUND: Extremity tourniquets have proven to be lifesaving in both civilian and military settings and should continue to be used by first responders for trauma patients with life-threatening extremity bleeding. This is especially true in combat scenarios in which both the casualty and the first responder may be confronted by the imminent threat of death from hostile fire as the extremity hemorrhage is being treated. Not every extremity wound, however, needs a tourniquet. One of the most important aspects of controlling life-threatening extremity bleeding with tourniquets is to recognize wh

[Application of extracorporeal membrane oxygenation combined with renal replacement therapy in the treatment of war traumatic sepsis].

Sepsis is an important factor for trauma related death, especially the refractory septic shock, which has a mortality rate exceeding 50%. The treatment of sepsis is a medical problem that needs to be solved urgently. The role and status of extracorporeal membrane oxygenation (ECMO) in the treatment of cardiopulmonary failure has become more and more prominent, and its role in the treatment of sepsis in children and infants has gained remarkable achievements. However, its role in adult sepsis remains controversial, with current reports showing varied outcomes for ECMO in adult sepsis treatment.

Immunotherapeutic options for inflammation in trauma.

BACKGROUND: Surgical management of trauma in the last 20 years has evolved in parallel with the military's experience in the current conflicts. Therapies such as widespread tourniquet use, empiric administration of fresh frozen plasma, and airborne intensive care units had been viewed skeptically but are now common practice. There is an opportunity to expand the envelope of care even further through similarly innovative approaches and varied avenues of research. RESULTS: As the molecular biology of trauma is elucidated, research methodologies must also be developed to capitalize on innovative

The "Top 10" research and development priorities for battlefield surgical care: Results from the Committee on Surgical Combat Casualty Care research gap analysis.

BACKGROUND: The US Military has achieved the highest casualty survival rates in its history. However, there remain multiple areas in combat trauma that present challenges to the delivery of high-quality and effective trauma care. Previous work has identified research priorities for pre-hospital care, but there has been no similar analysis for forward surgical care. METHODS: A list of critical "focus areas" was developed by the Committee on Surgical Combat Casualty Care (CoSCCC). Individual topics were solicited and mapped to appropriate focus areas by group consensus and review of Eastern Ass

Prevention of infections associated with combat-related eye, maxillofacial, and neck injuries.

The percentage of combat wounds involving the eyes, maxillofacial, and neck regions reported in the literature is increasing, representing 36% of all combat-related injuries at the start of the Iraq War. Recent meta-analysis of 21st century eye, maxillofacial, and neck injuries described combat injury incidences of 8% to 20% for the face, 2% to 11% for the neck, and 0.5% to 13% for the eye and periocular structures. This article reviews recent data from military and civilian studies to support evidence-based recommendations for the prevention of infections associated with combat-related eye, m

Noninvasive diagnostics for extremity compartment syndrome following traumatic injury: A state-of-the-art review.

Acute compartment syndrome (ACS) is a serious medical condition that can occur following traumatic injury to an extremity. If left undiagnosed, ACS can eventuate in amputation of the limb or even death. Because of this, fasciotomy to release the pressure within the muscle and restore tissue perfusion is often performed upon suspicion of ACS, as the sequelae to fasciotomy are less severe than those associated with not performing the fasciotomy. Currently, the "gold standard" of diagnosis is based on clinical assessment of such symptoms as pain out of proportion to the injury, obvious high press

Fatal injury epidemiology among the New Zealand military forces in the First World War.

BACKGROUND: Despite the large mortality burden of First World War (WW1) on New Zealand (NZ) military forces, no analysis using modern epidemiological methods has ever been conducted. We therefore aimed to study injury-related mortality amongst NZ military forces in WW1. METHODS: An electronic version of the Roll-of-Honour for NZ Expeditionary Force (NZEF) personnel was supplemented with further coding and analysed statistically. We also performed literature searches to provide context. RESULTS: Out of a total of 16,703 deaths occurring during the war (28 July 1914 to 11 November 1918), injur

Characteristics of combat-related spine injuries: a review of recent literature.

Injuries to the spinal column in combat casualties sustained during the conflicts in Iraq and Afghanistan are common, and the highest in reported wartime history. High-energy blast mechanisms from improved explosive devices have resulted in complex polytrauma and injury patterns, which are often markedly different from those injuries encountered in civilian trauma. Herein, we review the most current literature with regard to the distinct types of combat-related spine injuries/concomitant comorbidities sustained in Operations Enduring Freedom, Iraqi Freedom and New Dawn.

Sociodemographic Factors in Combat Compensation Seekers for Delayed-Onset PTSD.

INTRODUCTION: Sociodemographic factors can sometimes be more contributory in relation to war-related stress-induced disorder treatment and compensation-seeking than health-related factors. However, their impact is often overlooked. This study explores a relationship between sociodemographic factors and diagnoses of combat-related stress-induced disorders in combat compensation seekers for delayed-onset PTSD (DOPTSD). MATERIALS AND METHODS: Between June 2002 and August 2004, at the Regional Centre for Psychotrauma Zagreb, University Hospital Dubrava, the expert team evaluated subjects to diagn

Firearm injury in a child: An uncommon case of penile trauma.

Genitourinary system injuries generally occur due to blunt and penetrating trauma. Approximately 10% of all injuries encountered in the emergency department are related to the genitourinary system. Male genital trauma is a urological emergency because of the high risk of infection and the priority of protecting the sexual, endocrine, and reproductive functions. Blunt injuries to the penis are more often seen due to the mobility and localization of the penis than due to penetrating injuries caused by firearms or cutting instruments. We aimed to present and discuss a case involving shrapnel that

Stroke Care in Conflict and War Zones -Peace Saves Lives!

PURPOSE OF REVIEW: Cardiovascular conditions and related risk factors are an important and common group of diseases. Stroke is a disease that requires rapid diagnosis and treatment, and there are significant inequalities in stroke diagnosis, treatment and follow-up between countries, regions, societies and economic levels. This inequality becomes even more evident in regions where war and turmoil continue. What is the goal of your paper? What questions did you seek to answer? This review aims to explore whether war and armed conflicts impact stroke and care and describe potential mechanisms an

Head and neck surgical reconstruction in Operation Iraqi Freedom and Operation Enduring Freedom-Afghanistan: A systematic review.

Historically, head and neck injuries constituted 16 to 20% of all nonfatal combat injuries. However, advances in body and vehicle armor in the context of the use of ambushes and improvised explosive devices by enemy combatants have resulted in fewer fatalities from head and neck wounds, and thus the incidence of nonfatal head and neck injuries has risen to as high as 52%. Despite this increase, data regarding specific injury distributions, surgical cases, and approaches to repair are lacking in the current literature. We conducted a study to systematically review the current literature regardi

Outcomes after long-term follow-up of combat-related extremity injuries in a multidisciplinary limb salvage clinic.

BACKGROUND: Although the incidence of casualties from the Global War on Terror is decreasing, there remains a focus on the long-term sequelae from injuries sustained in the combat. Patients with prior significant limb injuries remain at risk of future complications. This study examines our institution's experience with a multidisciplinary team approach toward this challenging patient population. METHODS: A retrospective review was performed on all patients treated in a single institution Limb Preservation Clinic over a 2-year period. Those patients who sustained a combat-related injury in the

A year into the war: Longitudinal effects of trauma, displacement, and income loss on mental health in conflict zones.

BACKGROUND: Research has linked war-related stressors like traumatic loss, forced displacement, and income disruption to acute mental health symptoms. Little is known about how symptoms evolve longitudinally during prolonged conflict. Extending our 90-day studies, we tracked clinical symptoms and betrayal-based moral injury over one full year following the October 7, 2023, attack and ensuing war in Israel. We hypothesized stressor-exposed individuals would report persistent psychological distress and heightened betrayal perceptions one year later. METHODS: A four-wave longitudinal study follo
