Body-based therapies and transcranial magnetic stimulation for posttraumatic stress disorder: a proposed integrative approach.

Posttraumatic stress disorder (PTSD) remains difficult to treat, with 30-50% of patients retaining their diagnosis after first-line trauma-focused psychotherapies. Body-based therapies (BBTs) and transcranial magnetic stimulation (TMS) have each shown preliminary benefit for PTSD but have not yet been studied in combination. To our knowledge, this is the first proposal to integrate these modalities as a combined therapeutic approach. PTSD involves disrupted coordination across multiple levels of neural organization, from cortical regulatory networks to limbic threat circuits and autonomic syst

Using Passive Sensing to Isolate a Biosignature for Craving Among Individuals in Early Alcohol Use Disorder Recovery.

Craving is one of the most robust proximal predictors of both treatment dropout and relapse during early recovery for alcohol use disorder (AUD). Craving can onset rapidly, and the ability to accurately detect or modulate cravings varies significantly within- and between-persons. Because of this, craving management is a central feature of most current AUD treatment models. However, impaired interoceptive awareness, which is common in early AUD recovery, can stymie individuals' capacity to implement craving management strategies. Measurement and detection of craving through passive biosensor mo

Gabapentin may promote language development in a pediatric patient with autism spectrum disorder: a case report.

At this time, there is no report of how gabapentin may promote language development in children with autism spectrum disorder (ASD) experiencing neuropathic pain. A clinically significant increase in expressed vocabulary, around 10 words when gabapentin was prescribed to around 150 words at the 6-month follow-up, was observed in a child with ASD. This was likely due to improved symptoms of neuropathic pain, which could have allowed the patient to more effectively focus on language acquisition. Given that speech therapy had failed for years at that time to improve the patient's vocabulary and h

Autonomic regulation constrains psychotherapeutic workability: a state-dependent translational model for psychotherapy and psychosomatics.

Psychotherapy does not fail only because techniques are poorly chosen or alliances are weak; it may also fail because the patient's momentary state does not support productive use of the task. This matters especially in psychotherapy and psychosomatic care, where bodily distress, interoceptive threat, shame, and autonomic dysregulation can narrow affective, mnemonic, and reflective workability. In this hypothesis-and-theory paper, I propose a state-dependent translational model of psychotherapeutic processing. Operationally, autonomic regulation is treated not as a binary switch but as a const

Movement-Anchored Emotional Memory: An Embodied Neuropsychological Framework.

This article advances a clinically oriented neuropsychological framework and introduces the construct of movement-anchored emotional memory, proposing that bodily movement constitutes a core organizing dimension of emotional memory encoding, reactivation, and transformation. Integrating embodied cognition, affective and social neuroscience, developmental psychology, trauma research, and neuropsychology, emotional memory is conceptualized as emerging through dynamic coordination among sensorimotor, interoceptive, affective, relational, and contextual processes. Movement contributes temporal, sp

Touching the intangible: an exploratory resting-state EEG study of complex bodily hallucinations in the general population.

Experiences such as sensing a presence, feeling as if insects were crawling under the skin and related phenomena have been described in psychiatric, neurological, and toxicological contexts. However, they also occur frequently in the general population. Previous research suggests that stable physiological traits may predispose individuals to complex bodily hallucinations. The present exploratory study therefore examined whether resting-state electrocortical oscillatory activity is associated with proneness to such experiences. Participants from the general population completed the Complex Bodi

Tumour-brain crosstalk restrains cancer immunity via a sensory-sympathetic axis.

Body-brain communication has emerged as a key regulator of tissue homeostasis1-5. Solid tumours are innervated by different branches of the peripheral nervous system and increased tumour innervation is associated with poor cancer outcomes6-8. However, it remains unclear how the brain senses and responds to tumours in peripheral organs, and how tumour-brain communication influences cancer immunity. Here we identify a tumour-brain axis that promotes oncogenesis by establishing an immune-suppressive tumour microenvironment. Combining genetically engineered mouse models with neural tracing, tissue

Big ideas series: avoidance-to-embrace: the death discontinuity continuum between panic disorder and acute suicidal affective disturbance.

BACKGROUND: Acute Suicidal Affective Disturbance (ASAD) is a proposed clinical diagnosis characterized by sudden, intense escalations in suicidal intent, profound social and self-alienation, hopelessness, and heightened physiological arousal. Unlike traditional linear frameworks implying an "ideation-to-action" trajectory, ASAD captures temporally discrete, high-risk states occurring within hours or days. Although accumulating evidence supports the construct validity of ASAD, direct research remains constrained by ethical and practical challenges. OBJECTIVES: This paper proposes Panic Disorde

Female Orgasmic Disorder: Current Understanding and Clinical Management.

Female orgasmic disorder represents a significant, yet underrecognized aspect of women's sexual health that affects 10-28% of women and is characterized by persistent difficulty or absence of orgasm after adequate sexual stimulation. It remains poorly understood and infrequently addressed in clinical practice. The female orgasm involves complex neurobiological, vascular, hormonal, and psychosocial mechanisms, with the clitoris serving as the primary anatomical structure for orgasmic response. The orgasm gap, which refers to disparities in orgasm frequency between men and women, signifies biolo

Focal and Steerable Temporal Interference Stimulation of the Human Left Posterior Insula: A Proof-of-Concept Validation Using Intracranial Recordings in a Human Cadaver.

The insular cortex, especially its posterior subdivision, is a key hub for interoceptive, somatosensory, and nociceptive processing, making it a compelling target for neuromodulation. However, its depth within the lateral sulcus makes it difficult to reach selectively with conventional noninvasive brain stimulation. Temporal interference stimulation (TIS) can, in theory, focus low-frequency modulation onto deep structures, but experimental validation for the human insula is lacking. We tested whether a simple, simulation-guided TIS approach can focally and steerably modulate electric fields wi

Food addiction as a transdiagnostic feature associated with binge-eating symptoms in eating disorders: prevalence and rehabilitation outcomes in an Italian inpatient population.

Food addiction (FA) is an emerging construct describing a compulsive and dysregulated pattern of food consumption that parallels mechanisms observed in substance-related addictions. This study aimed to investigate the prevalence and clinical correlates of FA in a sample of 141 Italian adult inpatients diagnosed with eating disorders (ED). Participants completed standardized self-report instruments assessing FA (YFAS 2.0), binge-eating behaviours (BES), ED-related psychopathology (EDI-3), body image distress (BUT), and general psychological symptoms (SCL-90) at admission (T0) and discharge (T1)

Simultaneous stomach-brain electrophysiology reveals dynamic coupling in human sleep.

Sleep involves continuous communication between the brain and body, yet the dynamics of peripheral signals during human sleep remain poorly understood. Here we tested whether gastric electrophysiology exhibits infraslow structure and coordinated fluctuations with cortical rhythms indicative of sleep physiology. Simultaneous high-density electroencephalography (EEG) and electrogastrography (EGG) were recorded across sixty participants and three nights. Gastric power was consistently higher during NREM than REM sleep and declined across successive cycles, consistent with stage-dependent autonomi

Addressing body dysmorphic disorder through mindfulness: A new therapeutic frontier.

Body dysmorphic disorder presents in up to 2% of the general population, yet less than half seek psychological support. Some treatment barriers include poor illness insight, low body dysmorphic disorder awareness among mental health professionals and a lack of accessibility to body dysmorphic disorder-specific treatments. Currently available treatments for body dysmorphic disorder, including psychopharmacology and psychotherapy, show modest results in improving body dysmorphic disorder symptoms. With the reduced quality of life and increased risks of suicide among individuals with body dysmorp

Decoding the impact of the Covid-19 pandemic on anorexia nervosa psychopathology: A network comparison of pre- and post-pandemic onset.

The COVID-19 pandemic has profoundly impacted anorexia nervosa (AN), increasing pressure on healthcare services and causing specific psychopathological changes. However, the characterization of changes in presentation patterns across the pandemic remains unclear. This study aimed to address this gap by comparing symptom networks in patients with pre- and post-pandemic AN onset. Data from 189 patients diagnosed with AN from 2016 to 2022 were analyzed. Patients were divided into pre-COVID (n = 101) and post-COVID (n = 88) onset groups. Network analysis was employed to examine interconnections am

What the Eye Sees, the Mind Rejects: Implicit Visual Processing of Food Images in Anorexia Nervosa.

OBJECTIVE: This study aims to explore the role of implicit visual processing in reinforcing maladaptive eating behaviours in Anorexia Nervosa-restricting subtype (AN-R), focussing on how high- and low-calorie food stimuli are processed at different stages. METHOD: Thirty-two AN-R females and 36 healthy controls participated. Using a combination of novel paradigms in the field, the study employed: Breaking Continuous Flash Suppression (bCFS) for unconscious detection, Binocular Rivalry (BR) for perceptual dominance, and the Food Preference Approach-Avoidance Task (FP-AAT) for subconscious food

Sensing, feeling and regulating: investigating the association of focal brain damage with voluntary respiratory and motor control.

Breathing is a complex, vital function that can be modulated to influence physical and mental well-being. However, the role of cortical and subcortical brain regions in voluntary control of human respiration is underexplored. Here we investigated the influence of damage to human frontal, temporal or limbic regions on the sensation and regulation of breathing patterns. Participants performed a respiratory regulation task across regular and irregular frequencies ranging from 6 to 60 breaths per minute (bpm), with a counterbalanced hand motor control task. Interoceptive and affective states induc

Cognitive simulation along with neural adaptation explain effects of suggestions: a novel theoretical framework.

Hypnosis is an effective intervention with proven efficacy that is employed in clinical settings and for investigating various cognitive processes. Despite their practical success, no consensus exists regarding the mechanisms underlying well-established hypnotic phenomena. Here, we suggest a new framework called the Simulation-Adaptation Theory of Hypnosis (SATH). SATH expands the predictive coding framework by focusing on (a) redundancy elimination in generative models using intrinsically generated prediction errors, (b) adaptation due to amplified or prolonged neural activity, and (c) using

Expectation of pain and relief: A dynamical model of the neural basis for pain-trauma co-morbidity.

Posttraumatic Stress Disorder (PTSD) is highly co-morbid with chronic pain conditions. When present, PTSD significantly worsens chronic pain outcomes. Likewise, pain contributes to a more severe PTSD as evidenced by greater disability, more frequent use of harmful opioid analgesics and increased pain severity. The biomechanism behind this comorbidity is incompletely understood, however recent work strongly supports the widely-accepted role of expectation, in the entanglement of chronic pain and trauma symptoms. This work has shown that those with trauma have a maladaptive brain response while
