Beyond biochemical cascades: novel bio-mechanical and epigenetic paradigms of glial SASP in brain aging.

One of the most pressing scientific challenges of the current century is the mounting loss of cognitive function and vulnerability to neurodegenerative disorders associated with brain aging. This requires a paradigm shift beyond traditional neuron-focused models to address the central importance of non-neuronal glial cells, most notably astrocytes, in promoting age-related neuroinflammation. While the pro-inflammatory secretome of senescent cells, known as the senescence-associated secretory phenotype (SASP), is well-characterized in peripheral tissues, its specific role in the central nervous

External Lumbar Drainage to Abort Severe Traumatic IntraCranial Hypertension Phase 1 Randomized Clinical Trial: Scientific Rationale and Methodology: An MTBI2 Study.

BACKGROUND AND OBJECTIVES: There has been renewed interest in the use of external lumbar drainage (ELD) of cerebrospinal fluid (CSF) to reduce intracranial pressure after traumatic brain injury (TBI). Although the evidence is retrospective and thus confounded by selection bias, it is critical that a phase 1 trial systematically identify "select" patients who would gain benefit of intracranial pressure (ICP) reduction from lumbar drainage. The aim of this study is to evaluate safety and feasibility of controlled CSF ELD in reducing ICP burden and improving outcomes of select severe TBI patients

The Function of Sleep and the Treatment of Primary Insomnia.

BACKGROUND: Good sleep is correlated with health. Primary insomnia is a common reason for consulting a primary care physician. It is usually treated with drugs despite clear evidence for the efficacy of behavioral therapy. METHODS: We conducted a selective search of PubMed for German- and English-language reviews of the past decade that deal with the function and regulation of sleep in healthy individuals and/or the mechanism of action of currently used drugs and behavioral therapy. RESULTS: The important functions of sleep include cleaning processes such as the elimination of metabolic prod

Region-specific features of early glial activation and Aquaporin-4 dysregulation in conditional mouse models of TDP-43 proteinopathies.

INTRODUCTION: Aggregation and cytoplasmic mislocalization of TDP-43 are key features of several neurodegenerative diseases, including amyotrophic lateral sclerosis (ALS) and frontotemporal dementia (FTD). Neuroinflammatory processes mediated by glial cells play crucial roles in the pathophysiology of these and other diseases, defined as TDP-43 proteinopathies. Here, we characterized region-specific glial activation in two conditional mouse models: hTDP-43-WT (overexpressing nuclear wild-type human TDP-43) and hTDP-43-ΔNLS (expressing cytoplasmic TDP-43 with altered nuclear localization signal)

The skull bone marrow-meninges-brain axis: A new sentinel for neuroinflammation regulation in ischemic stroke.

The central nervous system (CNS) has long been regarded as relatively immune-privileged, but the discovery of glymphatic transport and meningeal lymphatic vessels has reshaped our understanding of neuroimmune communication. In ischemic stroke, emerging evidence suggests that post-injury inflammation is regulated not only by systemic leukocyte recruitment and blood-brain barrier disruption but also by a spatially organized skull bone marrow-meninges-brain axis. Anatomical studies have identified vascular channels connecting calvarial bone marrow with the dura mater, providing a potential route

Transcranial Photobiomodulation for Neuromodulation of Brain Disorders: A Perspective.

INTRODUCTION: Photobiomodulation (PBM) uses light to modulate biological activity. Because red and near-infrared wavelengths penetrate the skin and skull, PBM can be applied transcranially (tPBM) to noninvasively modulate brain activity, offering therapeutic potential for neurologic, psychiatric, neurodevelopmental, neurodegenerative, and neuroimmunologic disorders, and for cognitive enhancement. MATERIALS AND METHODS: A scoping review is performed integrating literature on tPBM in humans, excluding indications for which only preclinical data exist. RESULTS: tPBM can be delivered through the

Choroid Plexus Volume, Amyloid Burden, and Cognition in the Alzheimer's Disease Continuum.

As a part of the glymphatic system, the choroid plexus (CP) is involved in the clearance of harmful metabolites from the brain. We investigated the association between CP volume (CPV), amyloid-β (Aβ) burden, and cognition in patients on the Alzheimer's disease (AD) continuum. We retrospectively reviewed the records of 203 patients on the AD continuum and 82 healthy controls who underwent brain magnetic resonance imaging and 18F-florbetaben positron emission tomography. Automatic segmentation was performed, and the CPV was calculated. Cognitive function was assessed using detailed neuropsycholo

Optical coherence tomography reveals light-dependent retinal responses in Alzheimer's disease.

Spectral-domain optical coherence tomography (SD-OCT) is an accessible clinical tool for measuring structural changes to the retina, and increasingly as a biomarker for brain-predominant neurodegenerative diseases like Alzheimer's. Information about retinal function can also be extracted from OCT images, but is under-studied, with literature examples often employing challenging protocols or requiring specialized hardware. The first goal of this study was to verify that functional retinal imaging was feasible with a commercially-available SD-OCT device and a clinically practical protocol. Inspi

Baseline glymphatic efficiency is associated with plasma BDNF changes following rTMS: an exploratory biomarker study in mood disorders.

Brain-derived neurotrophic factor (BDNF) has been proposed as a potential biological correlate of repetitive transcranial magnetic stimulation (rTMS). However, its relationship with clinical and cognitive outcomes in mood disorders remains unclear. In this prospective exploratory pre-post study, 28 adults with major depressive disorder or bipolar disorder underwent 12 sessions of high-frequency left dorsolateral prefrontal cortex (DLPFC) rTMS while continuing pharmacological treatment. Assessments included Hamilton Rating Scale for Depression (HDRS-24), Wisconsin Card Sorting Test (WCST), plas

Choroid plexus enlargement contributes to CSVD progression and white matter microstructural deterioration via brain-fluid dysregulation.

BACKGROUND: The choroid plexus (ChP) plays a pivotal role in cerebrospinal fluid (CSF) production, blood-CSF barrier maintenance, and immune surveillance at the brain-periphery interface. While ChP enlargement has been implicated in several neurodegenerative and neuroinflammatory disorders, its role in the pathogenesis of cerebral small vessel disease (CSVD) and frequently cooccurring white matter (WM) microstructural damage has not been systematically investigated, particularly in large-scale, longitudinal, population-based settings. We aimed to determine whether ChP enlargement contributes t

Decreased brain interstitial fluid dynamics is associated with risk of Alzheimer's disease-related cognitive decline.

BACKGROUND: Diffusion-tensor image analysis along the perivascular space (ALPS) index that has the potential to reflect brain interstitial fluid (ISF) dynamics may predict the development of Alzheimer's Disease (AD). We aimed to study whether brain ISF dynamics indicated by the ALPS index relate to AD dementia diagnosis and AD-related changes. METHODS: This study included a discovery cohort (n = 180) and a validation cohort (n = 127), which were composed of cognitively normal, subjective memory concern, mild cognitive impairment, and AD dementia subjects. Multimodal MRI reveals impaired glymphatic function with choroid plexus enlargement and cerebrospinal fluid expansion in alzheimer's disease.

There is increasing evidence that cerebrospinal fluid (CSF) circulation plays a key role in the pathophysiology of Alzheimer's disease (AD), although the underlying mechanisms remain poorly understood. This study evaluated CSF circulation in patients with AD using magnetic resonance imaging (MRI)-derived measures, including choroid plexus (CP) volume, CSF volume, and diffusion tensor imaging along the perivascular space (DTI-ALPS) index. The study included 188 participants: 28 with AD, 40 with mild cognitive impairment (MCI), 66 with subjective cognitive decline (SCD), and 54 normal controls (

Enhanced Meningeal Lymphatic Drainage Ameliorates Neuroinflammation and Hepatic Encephalopathy in Cirrhotic Rats.

BACKGROUND & AIMS: Hepatic encephalopathy (HE) is a serious neurologic complication in patients with liver cirrhosis. Very little is known about the role of the meningeal lymphatic system in HE. We tested our hypothesis that enhancement of meningeal lymphatic drainage could decrease neuroinflammation and ameliorate HE. METHODS: A 4-week bile duct ligation model was used to develop cirrhosis with HE in rats. Brain inflammation in patients with HE was evaluated by using archived GSE41919. The motor function of rats was assessed by the rotarod test. Adeno-associated virus 8-vascular endothelial

Peripheral alpha-2 antagonist vatinoxan improves dexmedetomidine-induced perivascular cerebrospinal fluid flow without affecting electroencephalogram activity in female rats.

BACKGROUND: The glymphatic concept represents a brain-wide perivascular fluid network contributing to brain metabolite clearance. Dexmedetomidine, a sedative α2-adrenergic receptor agonist, enhances perivascular cerebrospinal fluid (CSF) flow by reducing central noradrenergic tone and inducing sleep-like electroencephalogram (EEG) slow-wave activity. Concurrently, α2-adrenergic agonists modulate peripheral physiological functions, possibly influencing the central glymphatic dynamics. Utilizing peripherally restricted α2-adrenergic antagonist vatinoxan, we evaluated the role of physiological pa

Blood Pressure Mediates the Association Between Carotid-Femoral Pulse Wave Velocity and Basal Ganglia Enlarged Perivascular Spaces: A Community-Based Cross-Sectional Study.

Although carotid-femoral pulse wave velocity (cf-PWV) is the non-invasive gold standard for assessing aortic stiffness, evidence linking cf-PWV to basal ganglia enlarged perivascular spaces (BG-EPVS) in the general population is limited. We aimed to evaluate this association and quantify the mediating role of blood pressure (BP) in community-based adults. This cross-sectional study included 485 participants from the Kailuan cohort. Cf-PWV was measured by ultrasound, and BG-EPVS were graded on 3.0-Tesla MRI. Logistic regression, restricted cubic spline, and mediation analyses were applied to qu
