BACKGROUND: Massive retracted rotator cuff tears are disabling in physically active patients. In patients with persistent pain in whom nonoperative treatment has failed, multiple surgical treatment options are available. Lower trapezius tendon transfer is a promising surgical procedure that can decrease pain, improve external rotation strength, and recreate more normal glenohumeral kinematics. In the present video article, we describe the surgical technique for successful arthroscopic ("scopy")-assisted lower trapezius transfer (SALTT) with use of an easily accessible peroneus longus autograft

BACKGROUND: Large and massive rotator cuff tears and tears after failed surgical repair are a challenging clinical problem with different treatment options. The purpose of the study was to evaluate the midterm outcomes after rotator cuff repair (RCR) with autologous hamstring tendon graft bridging (tissue-enhanced autologous rotator cuff repair [TEAR] patch) with the hypothesis that outcomes would be reasonable and complication rates would be low.
METHODS: This is a retrospective case series study of patients who underwent open RCR using a TEAR patch from June 2015 to March 2019. The exclusio

OBJECTIVES: It is well known that alterations in landing mechanics persist for years after anterior cruciate ligament reconstruction (ACL-R). Nevertheless, existing literature is controversial in reporting successful or unsuccessful recovery of prelanding muscle activation timing after ACL-R. The study aimed at comparing myoelectric and kinematic patterns during landing tasks between ACL-R and healthy subjects.
DESIGN: Cross-sectional study.
SETTING: Institutional research laboratory.
PATIENTS AND INTERVENTION: Fifteen male athletes after ACL-R using patellar tendon and 11 using hamstrings

BACKGROUND: Lateral extra-articular tenodesis (LET) can reduce the risk of graft failure after anterior cruciate ligament (ACL) reconstruction in adolescents and young adults. It is unclear whether the use of this procedure increases risks of complications such as arthrofibrosis after ACL reconstruction using hamstring autograft in adolescents.
HYPOTHESIS: Adolescents treated with ACL reconstruction using hamstring autograft with and without modified Lemaire LET have similar rates of arthrofibrosis after surgery.
STUDY DESIGN: Retrospective cohort study; Level of evidence, 3.
METHODS: Cases

INTRODUCTION/OBJECTIVES: Graft failure and secondary meniscal tears remain significant concerns following anterior cruciate ligament reconstruction (ACLR). Lateral extra-articular procedures, including anterolateral ligament (ALL) reconstruction, have demonstrated efficacy in reducing anterior cruciate ligament (ACL) graft failure and meniscal repair rates. However, their impact on untreated stable meniscal tears remains unclear. This study aimed to compare clinical outcomes and reoperation rates between ACLR with and without ALL reconstruction in patients with untreated stable lateral menisca

BACKGROUND: A limb symmetry index (LSI) of >90% for single-leg horizontal hop distance is recommended as a cutoff point for safe return to sports after anterior cruciate ligament reconstruction (ACLR). Despite achieving this threshold, abnormal lower limb biomechanics continue to persist in athletes after ACLR. Symmetry in single-leg vertical hop height appears to be more difficult to achieve and can be a better representation of knee function than single-leg horizontal hop distance.
PURPOSE: To explore whether an LSI of >90% for single-leg vertical hop height can represent normal lower limb

BACKGROUND: Wrestling is a physically demanding sport, with young athletes prone to anterior cruciate ligament (ACL) injuries. There is a paucity of data evaluating the results of ACL reconstruction (ACLR) in this cohort.
PURPOSE: To assess return to sport (RTS), patient-reported outcomes, reoperation rates, and graft survival after ACLR in a large cohort of competitive wrestlers at midterm follow-up.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: We identified all competitive wrestlers (club, high school, collegiate, or professional) with a history of an ACLR at a single institu

IMPORTANCE: Graft choice for anterior cruciate ligament reconstruction (ACLR) remains a subject of interest among orthopaedic surgeons because no ideal graft has yet been found. Peroneus longus tendon (PLT) has emerged as an alternative autograft for reconstruction in kneeling populations and in simultaneous anterior cruciate ligament (ACL) and medial collateral ligament (MCL) injuries.
OBJECTIVE: To evaluate the current evidence on the outcome of primary ACLR with PLT autograft in adults and donor ankle morbidity, in addition to determining the average PLT graft dimensions from published stu

PURPOSE: To identify predictors of inferior patient-reported outcomes (PROs) 1 year after anterior cruciate ligament reconstruction (ACLR) and to examine whether these predictors differ by sex. We hypothesised that older age, female sex, and meniscectomy are associated with inferior PROs.
METHODS: We conducted a prospective cohort study involving 733 patients (396 males and 337 females) who underwent ACLR using autologous hamstring tendons. Participants were enroled in the Hiroshima Clinical ACL Research Project. Pre-operative assessments included demographic data, injury history, and scores

PURPOSE: To investigate whether the surgical technique of single-bundle anterior cruciate ligament (ACL) reconstruction, the visualization of anatomic surgical factors and the presence or absence of concomitant injuries at primary ACL reconstruction are able to predict patient-reported success and failure. The hypothesis of this study was that anatomic single-bundle surgical procedures would be predictive of patient-reported success.
METHODS: This cohort study was based on data from the Swedish National Knee Ligament Register during the period of 1 January 2005 through 31 December 2014. Patie

BACKGROUND: Lateral extra-articular procedures are increasingly used to improve rotational stability during anterior cruciate ligament reconstruction (ACLR), particularly in high-grade pivot-shift knees. However, whether concomitant anterolateral ligament reconstruction (ALLR) provides different intraoperative (time-zero) rotational stability when combined with single-bundle (SB) versus double-bundle (DB) hamstring ACLR remains unclear. We compared stepwise intraoperative laxity in pivot-shift grade 2-3 knees under anesthesia.
METHODS: Consecutive primary hamstrings ACLR with concomitant ALLR

BACKGROUND: There is no consensus regarding the appropriate indications for the addition of lateral extra-articular tenodesis (LET) to anterior cruciate ligament (ACL) reconstruction (ACLR).
PURPOSE: To determine if incomplete correction of the pivot shift during ACLR is an appropriate intraoperative indication to add the LET, in terms of clinical outcome.
STUDY DESIGN: Cohort study; Level of evidence, 3.
METHODS: From January 2018, incomplete correction of the pivot shift relative to the contralateral knee was used as the indication to add LET to ACLR. Patients presenting before this date

Anterior cruciate ligament reconstruction (ACLR) is commonly performed in the younger or active population, but failure rates have been disappointing in high-risk patients. Recently, lateral extra-articular procedures such as the modified Lemaire extra-articular tenodesis (LET) have been proposed to decrease failure rates, but knowledge on short-term rehabilitation, stiffness, and isokinetic strength is limited. This study aimed to assess the short-term patient-reported outcome measures (PROMs) and physical performance outcomes following ACLR with and without LET. A prospective study was perfo

PURPOSE: The aim of this study was to compare anatomic double-bundle anterior cruciate ligament reconstruction with non-anatomic single-bundle reconstruction.
METHODS: In a prospective consecutive series, 94 unselected patients [45 anatomic double-bundle (ADB) and 49 non-anatomic single-bundle (SB)] underwent ACL reconstruction involving hamstring tendon autograft, interference screw fixation on both the femoral and tibial side and drilling the femoral tunnel(s) through the antero-medial portal in both groups. In the ADB group, the remnants of the ACL were identified and the grafts were place

Forty-three patients who had undergone an anterior cruciate ligament (ACL) reconstruction using a doubled semitendinosus and gracilis graft were prospectively reviewed at 5-year follow-up. All had suffered subacute or chronic tears of the ligament. At surgery, the femoral tunnel was drilled first through the antero-medial portal. The correct position of the femoral and tibial guide wire was checked fluoroscopically. A cortical fixation to the bone was achieved in the femur with a Mitek anchor, directly passing the two tendons in the slot of the anchor, and in the tibia with an RCI screw, suppl
