Effects of YM-14673, a thyrotropin-releasing hormone analogue, injected into the shell and the core of the nucleus accumbens on production of repetitive jaw movements in rats: comparison with the effects of a dopamine D1 and D2 receptor agonist combination.

The present study examined whether the shell and the core of the nucleus accumbens play a differential role in the display of YM-14673-induced jaw movements in rats. For that purpose the effects of YM-14673 were compared to those of a SKF 82958 and quinpirole combination, a dopamine D1 and a D2 receptor agonist respectively, that is known to functionally differentiate these two subregions of the nucleus. Consistent with the previous report, bilateral injections of a mixture of SKF 82958 (5 micrograms) and quinpirole (10 micrograms) into the shell of the nucleus accumbens produced repetitive ja

Cardiac strain in patients on Janus Kinase inhibitors for rheumatic diseases: a 1-year echocardiographic study.

OBJECTIVES: Janus kinase inhibitors (JAKi) are increasingly used to treat autoimmune rheumatic diseases (ARDs), despite concerns regarding their potential cardiovascular risks. Cardiac strain, a sensitive marker for subclinical myocardial dysfunction, can predict the risk of heart failure. This study aims to evaluate the effect of JAKi on cardiac strain and function in patients with ARDs in routine clinical practice. METHODS: This prospective Greek cohort study enrolled patients diagnosed with RA, PsA, or axSpA initiating treatment with a JAKi (baricitinib, tofacitinib, or upadacitinib). Comp

Validation of dabrafenib-trametinib prognostic groups in patients treated with vemurafenib and cobimetinib for advanced BRAF-mutated melanoma.

Prognostic groups defined by lactate dehydrogenase concentration and number of organ sites containing metastases have been reported for patients treated with dabrafenib and trametinib for advanced melanoma. We aimed to validate these prognostic groups for patients treated with vemurafenib and cobimetinib in the coBRIM and BRIM-3 clinical studies. Eight hundred nine patients were included, 240 treated with vemurafenib plus cobimetinib and 569 with vemurafenib. For patients treated with vemurafenib and cobimetinib, both overall survival (P < 0.001, c-statistic = 0.72) and progression-free surviv

Points to consider for the treatment of immune-mediated inflammatory diseases with Janus kinase inhibitors: a consensus statement.

OBJECTIVES: Janus kinase inhibitors (JAKi) have been approved for use in various immune-mediated inflammatory diseases. With five agents licensed, it was timely to summarise the current understanding of JAKi use based on a systematic literature review (SLR) on efficacy and safety. METHODS: Existing data were evaluated by a steering committee and subsequently reviewed by a 29 person expert committee leading to the formulation of a consensus statement that may assist the clinicians, patients and other stakeholders once the decision is made to commence a JAKi. The committee included patients, rh

Impact of adding ezetimibe to statin to achieve low-density lipoprotein cholesterol goal (from the Clinical Outcomes Utilizing Revascularization and Aggressive Drug Evaluation [COURAGE] trial).

In the Clinical Outcomes Utilizing Revascularization and Aggressive Drug Evaluation (COURAGE) study, a revascularization strategy trial with optimal medical therapy in both arms, the low-density lipoprotein (LDL) cholesterol goal was 60 to 85 mg/dl; this was revised to <70 mg/dl in 2004. COURAGE patients (n = 2,287) were titrated with increasing statin doses to achieve the initial LDL cholesterol goal using a prespecified protocol. Ezetimibe was not available when study enrollment began in 1999 but became available after approval in 2003. After maximizing statin dose, ezetimibe was added to re

Novel synergistic therapeutic approach in idiopathic pulmonary fibrosis: Combining the antifibrotic nintedanib with the anti-inflammatory baricitinib.

BACKGROUND: Baricitinib and nintedanib can target inflammation and fibrosis respectively, which are the two most important processes in idiopathic pulmonary fibrosis (IPF). However, it is still unknown whether targeting these two processes simultaneously can synergistically improve the therapeutic effect of IPF. Therefore, it is necessary to predict the possible translational potential through preclinical studies. METHODS: We evaluated both the in vitro and in vivo efficacy of a drug combination, nintedanib with baricitinb, a JAK1/JAK2 inhibitor. We first examined the fibroblast proliferation

Differences in immunogenicity to zoster recombinant vaccine in IMID patients undergoing treatment with non-selective or selective JAK-i.

OBJECTIVES: Patients with immune-mediated inflammatory diseases (IMIDs) treated with immunosuppressive therapies are at higher risk for infections, including those preventable by vaccines. Among these therapies, Janus kinase inhibitors (JAK-is) are increasingly used, though their association with varicella zoster virus (VZV) reactivation raises concerns. The recombinant zoster vaccine (Shingrix™) is recommended for immunocompromised individuals and has shown strong safety and efficacy; however, data on its immunogenicity in IMID patients-especially those receiving JAK-is, anti-TNF agents or MT
