Corticosteroid-responsive optic neuritis associated with acute retinal necrosis due to Herpes simplex virus type 2 in a pediatric patient.

PURPOSE: Optic neuritis (ON) associated with acute retinal necrosis (ARN) has traditionally been considered a complication resulting from direct viral infiltration of the optic nerve. Although it is currently treated with intravenous acyclovir, the appropriate dosage and duration of treatment in the pediatric population remain unclear, and no standardized effective treatment has been established in clinical guidelines. METHODS: A 14-year-old boy was referred for panuveitis in the right eye (RE), with a visual acuity (VA) of hand motions. Fundus examination revealed intense vitritis, and PCR t

Real-world evidence of treatment for relapse of noninfectious uveitis in tertiary centers in Japan: A multicenter study.

Noninfectious uveitis (NIU), which pathogenesis is often autoimmune nature, occurs as a symptom of systemic syndromes or only in the eye. The standard treatment of NIU is local, topical, and oral administration of corticosteroids (CS) in combination with immunomodulatory therapy (IMT). However, additional therapeutic strategies involving topical and systemic administration of CS or others to treat relapse or exacerbation of ocular inflammation in NIU which present as various ocular manifestations have not been established. The aim of this study was to investigate therapeutic strategies used fo

Controlled release of corticosteroid with biodegradable nanoparticles for treating experimental autoimmune uveitis.

Noninfectious uveitis is a potentially blinding ocular condition that often requires treatment with corticosteroids to prevent inflammation-related ocular complications. Severe forms of uveitis such as panuveitis that affects the whole eye often require a combination of topical and either regional or systemic corticosteroid. Regional corticosteroids are currently delivered inside the eye by intravitreal injection (e.g. Ozurdex®, an intravitreal dexamethasone implant). Intravitreal injection is associated with rare but potentially serious side effects, including endophthalmitis, retinal and vit

[Tolerability and short-term efficacy of the Ozurdex® dexamethasone intravitreal implant for treatment of uveitic cystoid macular edema: A retrospective study of 52 injections performed at the Clermont-Ferrand teaching hospital].

Macular edema is the main cause of decreased visual acuity in uveitis and determines the visual prognosis. We retrospectively analyzed the tolerability and efficacy of the dexamethasone intravitreal implant in the management of uveitis. Twenty-five patients with uveitic cystoid macular edema were all treated with the Ozurdex(®) dexamethasone intravitreal implant from January 2012 to August 2014. The main outcome measures were visual acuity recovery, changes in macular thickness and resolution of the vitritis. Thirty-two eyes of 25 patients received 52 injections of Ozurdex(®). Both complete re

Primary Vitreoretinal Lymphomas: A Diagnostic Challenge - Report of Two Recent Cases with Retinal Biopsies and Molecular Investigations in Halifax, Nova Scotia.

INTRODUCTION: Primary vitreoretinal lymphomas (PVRL) are a type of central nervous system lymphoma that arise in the retina, vitreous, or optic nerve without initial brain involvement. The clinical diagnosis can be a challenge since the disease mimics uveoretinitis in its presentation and initial treatment response to steroids. Diagnostic confirmation with vitreous cytology has been the gold standard for diagnosis. However, there are limitations of vitreous cytology, such as low volume of sample, low number of lymphoma cells within the sample, and poor preservation of cells due to shearing for

Sympathetic Ophthalmia - a Contribution to Immunology, Clinic and Current Imaging.

BACKGROUND: Sympathetic ophthalmia (SO) is a rare inflammation of an operated or injured eye that spreads to the fellow eye. It is typically a bilateral granulomatous panuveitis. The traumatized eye is referred to as inciting eye and the fellow eye as sympathizing eye. The pathophysiology of the disease is not entirely understood, but there is strong evidence of an autoimmune genesis. PATIENTS/MATERIAL AND METHODS: A selective literature search on epidemiology, immunology, clinical features and risk factors of SO was carried out. In addition, our own experience using multimodal imaging for th

The prevalence and potential associations of anti-drug antibodies against adalimumab in patients with non-infectious uveitis: a cross-sectional study.

BACKGROUND: Adalimumab is effective in treating non-infectious uveitis (NIU), but some patients develop anti-drug antibodies against adalimumab (ADA-A), which can reduce its effectiveness, resulting in active inflammation and vision loss. We sought to determine the prevalence of ADA-A in adults receiving adalimumab for NIU, and investigate factors associated with the presence of ADA-A. BODY: In this cross-sectional study, eighty-one (46 female) consecutive adult patients receiving adalimumab for NIU at The Royal Victorian Eye and Ear Hospital, and Eye Surgery Associates in Melbourne, Australia
