Intravenous sedation combined with local anesthesia versus spinal anesthesia for hemorrhoidectomy with rubber band ligation: A retrospective cohort study.

BACKGROUND: Evidence comparing intravenous sedation combined with local anesthesia (IV + LA) versus spinal anesthesia (SA) for hemorrhoidectomy with concomitant rubber band ligation (RBL) is limited, particularly in the context of ambulatory-oriented pathways. METHODS: We conducted a single-center retrospective cohort study including consecutive adults undergoing hemorrhoidectomy with RBL between January 2024 and January 2026. Patients were grouped by anesthetic technique (IV + LA vs SA). The primary outcome was postoperative pain at 24 h measured by the numerical rating scale (NRS). Secondar

Gender differences in propofol requirements during total intravenous anesthesia administered by closed-loop anesthesia delivery system: an observational study.

BACKGROUND: Propofol consumption during total intravenous anesthesia (TIVA) is influenced by patient's gender and females consume greater amount than males. As gender difference on propofol consumption during TIVA has only been assessed with manually titrated or target-controlled infusion, its validation with automated delivery systems, is desirable. This study evaluated effects of gender differences on propofol consumption during automated propofol TIVA administered by closed-loop anesthesia delivery system (CLADS). METHODS: The enrolled patients (N.=80) were allocated to two groups based on

Cortical, subcortical, brainstem and autonomic responses to nociception under total intravenous anesthesia.

BACKGROUND: Physiological responses to nociception are complex and involve intricate associations between the central, peripheral, and autonomic nervous systems. To optimize intraoperative analgesic titration, several monitoring devices have been developed, each targeting specific physiologic variables. However, existing devices primarily focus on isolated components of the nociceptive response, such as autonomic or cortical activity, without integrating these perspectives comprehensively. Our aim was to compare the performance of different nociception monitors in response to standardized teta

Parosmia and dysgeusia after intravenous propofol-based general anesthesia: A case report.

Various drugs, including anesthetic agents, can cause parosmia in the perioperative period. There are reported cases of patients with alterations of smell and taste due to local anesthetics, nerve damage, or as a side effect of general anesthesia. We present a case of a 58-year-old male who developed parosmia and dysgeusia in the postoperative period after radical nephrectomy and inferior vena cava thrombectomy. The anesthetics used were fentanyl and propofol for general anesthesia and ropivacaine for epidural analgesia. Clinical examination did not reveal any pathology.

Effect of intravenous magnesium sulfate on bupivacaine spinal anesthesia in preeclamptic patients.

BACKGROUND: Magnesium sulfate (MgSO4) is the standard therapy for the treatment of preeclampsia and prevention of eclamptic seizures in labor. This study aimed to determine the effect of the magnesium administered intravenously as a bolus prior to spinal anesthesia on the speed of onset and duration of the spinal anesthesia in preeclamptic women. METHODS: Sixty women undergoing caesarean section were randomly divided into the MgSO4 group and the control group. The MgSO4 group received 50 mg/kg MgSO4 (total 50 ml) infused intravenously within 15 min and the control group received the same volu

Outcomes of cancer surgery after inhalational and intravenous anesthesia: A systematic review.

Perioperative factors are probably essential for different oncological outcomes. This systematic review investigates the literature concerning overall mortality and postoperative complications after cancer surgery with inhalational (INHA) and intravenous anesthesia (TIVA). A search was conducted according to the PRISMA guidelines, including studies with patients undergoing surgery for cancer and where TIVA was compared with INHA. Two investigators identified relevant papers in the databases: PubMed, Scopus, EMBASE and the Cochrane Library. Risks of bias assessment tools from the Cochrane Colla

[Anesthetic management of a patient with mitochondrial encephalomyopathy under total intravenous anesthesia].

This report describes a patient with mitochondrial encephalomyopathy who underwent tracheostomy under total intravenous anesthesia. This 15-year-old girl had been suffering from aspiration pneumonia repeatedly. Anesthesia was induced with propofol (30 mg) and fentanyl (50 microg), and the trachea was intubated without a muscle relaxant. The patient was mechanically ventilated also without a relaxant, and anesthesia was maintained with a continuous infusion of propofol 4-10 mg x kg(-1) x hr(-1) and a bolus injection of fentanyl 25 microg. Bispectral index (BIS) was monitored and maintained at 1

Total intravenous anesthesia for repair of congenitral diaphragmatic hernia: a case report.

Congenital diaphragmatic hernia is a cardiopulmonary anomaly that causes severe respiratory disorder. Traditionally, inhalational anesthetics with mechanical hyperventilation, opioids, and muscle relaxants are used in anesthesia for repair surgery. In this case, we used total intravenous anesthesia combined with high-frequency oscillatory ventilation and inhaled nitric oxide for surgical repair of the diaphragm. After surgery, the patient recovered well and was discharged from hospital 1 month later.

Total intravenous anesthesia with propofol and remifentanil for elective non-cardiac surgery.

BACKGROUND: Remifentanil is a highly effective mu opioid agonist with predictable pharmacokinetics and a close concentration-effect relationship. Moreover, studies on anesthetic drugs interactions show that optimal propofol concentrations decrease more significantly with remifentanil as compared with other opioids and recovery appears to be much faster than when propofol is combined with other opioids combinations. This intervention study was designed to evaluate the efficacy of propofol combined with remifentanil in elective non cardiac inpatient surgery. METHODS: N. 405 patients undergoing

[Anxiolysis vs. intravenous conscious sedation during maxillary fracture splinting in local anesthesia].

For the conservative treatment of mandibular fractures with arch bars under local anaesthesia we used two forms of premedication in 140 patients: 1. an analgetic sedation with pethidine 1 mg/kg body weight and promethazine 0.5 mg/kg body weight i.m. (n = 70 cases) 2. an anxiolytic sedation with midazolam 0.05-0.1 mg/kg body weight i.v. (n = 70 cases). The effect of the premedication was measured retrospectively by the mean amount of local anaesthetics used during splinting. It could be noted that the mean consumption of local anaesthetics was significantly lower in the midazolam-group than in

[A case of apnea during spinal anesthesia after intravenous injection of sedatives].

During spinal anesthesia for appendectomy (sensory level: -Th7), apnea occurred for about 20 minutes by intravenous administration of diazepam. Since the patient showed hyperventilation sign before the incident, this apnea might be caused by low PaCO2 and stopping respiratory stimulation from psychological unrest when diazepam, pentazocine and droperidol were administered. In the postoperative period, this patient showed normal neurologic and psychosomatic functions. In such a case as this, long duration of apnea might damage brain by severe hypoxia.
