Are three antiseptic paints needed for safe preparation of the surgical field? A prospective cohort study with 239 patients.

BACKGROUND: Preoperative skin antisepsis is an essential component of safe surgery. However, it is unclear how many antiseptic paints are needed to eliminate bacteria prior to incision. This study compared microbial skin counts after two and three antiseptic paints. METHODS: We conducted a prospective cohort study in non-emergency patients receiving a cardiac/abdominal surgery with standardized, preoperative skin antisepsis consisting of an alcoholic compound and either povidone iodine (PI) or chlorhexidine (CHX). We obtained three skin swabs from the participant's thorax/abdomen using a ster

Dimethyl sulfoxide enhances effectiveness of skin antiseptics and reduces contamination rates of blood cultures.

Effective skin antisepsis is of central importance in the prevention of wound infections, colonization of medical devices, and nosocomial transmission of microorganisms. Current antiseptics have a suboptimal efficacy resulting in substantial infectious morbidity, mortality, and increased health care costs. Here, we introduce an in vitro method for antiseptic testing and a novel alcohol-based antiseptic containing 4 to 5% of the polar aprotic solvent dimethyl sulfoxide (DMSO). The DMSO-containing antiseptic resulted in a 1- to 2-log enhanced killing of Staphylococcus epidermidis and other micro

Evaluation of 15-second alcohol-based hand rub efficacy: a multi-laboratory study using a modified EN 1500 protocol.

INTRODUCTION: Hands are a key vector for pathogen transmission in healthcare, making effective hand antisepsis crucial for infection prevention. According to the European standard EN 1500, the reference method for evaluating hand antiseptics, a minimum rub-in time of 30 s is required. However, observations show healthcare workers typically spend less time on hand antisepsis. METHOD: To assess the feasibility of a reduced rub-in time under standardized conditions, the German Association for Applied Hygiene conducted a multi-centre ring trial in 14 laboratories using a modified EN 1500 protocol

Topical proparacaine eye drops to improve the experience of patients undergoing intravitreal injections: A randomized controlled trial.

PURPOSE: We sought to evaluate whether additional topical anesthetic, specifically proparacaine 0.5%, improved patient experience with intravitreal injections without hindering antisepsis. METHODS: A prospective, randomized controlled trial was conducted including 36 eyes of 36 patients undergoing intravitreal injections. Patients were randomized to treatment with additional topical proparacaine 0.5% versus control after undergoing informed consent. All patients prior to intravitreal injection underwent conjunctival culture after one drop of topical proparacaine 0.5% was placed. Half of patie

Reappraising Elements of the Aseptic Technique in Dermatology: A Review.

Dermatologic procedures are performed under varying degrees of antisepsis, and no clear guidelines exist regarding the role of the aseptic technique in dermatology. This review aims to clarify the terminology surrounding surgical asepsis and examines the importance of various components of the aseptic technique in cutaneous surgery. Included are studies examining optimal glove type, surgical instruments, skin antisepsis, and cost-reducing protocols. Our review highlights that most dermatology procedures are not performed under completely sterile conditions due to the lack of environmental and

Infection following soft tissue injury: its role in wound healing.

PURPOSE OF REVIEW: Wound infection has always been an unconquered problem temporarily improved with the discovery of antibiotics but now struggling with an epidemic of resistant organisms. Wound healing has become a popular sub-speciality for the doctor and allied health professional working in the laboratory or at the bedside. It is a field with many new journals and frequent congresses that publish elaborate proceedings. These have been examined for this review. RECENT FINDINGS: Measurement of infection remains elusive. A clear difference between contamination and infection is difficult to

Effect of rectal mucosa cleansing on acute prostatitis during prostate biopsy: A randomized prospective study.

OBJECTIVE: Infectious complications after transrectal ultrasound-guided prostate biopsy (TRUS-PB) can range from asymptomatic bacteriuria and febrile or non-febrile urinary tract infection (UTI) to sepsis. Cleaning of rectal mucosa with topical antiseptics such as povidone iodine or chlorhexidine before the procedure are alternative prophylaxis methods. We aimed to investigate the effects of these two different topical antiseptic agents on infectious complications and their superiority to each other. MATERIAL AND METHODS: The study was conducted with 200 patients. Rectal mucosa cleansings wer

Johann Friedrich Dieffenbach: successful use of leeches in plastic surgery in the 1820s.

Johann Friedrich Dieffenbach (1792-1847) regularly and successfully utilised leeches in sophisticated plastic surgery in Berlin in the 1820s and 1830s, well before anaesthesia, antisepsis and antibiotics. Inexplicably, it took nearly another 150 years before the use of leeches in this context was revived.

Intranasal povidone-iodine concentration and olfactory recovery after endoscopic endonasal pituitary surgery.

PURPOSE: The endoscopic endonasal approach (EEA) is a widely accepted, safe, and effective technique to treat pituitary lesions. Among postoperative considerations, olfactory function plays a key role in the patient’s quality of life. Although povidone-iodine (PVP-I) is widely used for intranasal antisepsis, its optimal concentration remains unclear. This study evaluated whether the PVP-I concentration during antisepsis influences postoperative olfactory recovery in patients undergoing EEA. METHODS: We retrospectively reviewed patients with sellar tumors who underwent endoscopic endonasal pitu

Preoperative skin antiseptics for preventing surgical wound infections after clean surgery.

BACKGROUND: Approximately 15% of elective surgery patients and 30% of patients receiving contaminated or dirty surgery are estimated to develop post-operative wound infections. The costs of surgical wound infection can be considerable in financial as well as social terms. Preoperative skin antisepsis is performed to reduce the risk of post-operative wound infections by removing soil and transient organisms from the skin. Antiseptics are thought to be both toxic to bacteria and aid their mechanical removal. The effectiveness of preoperative skin preparation is thought to be dependent on both th

Isopropyl alcohol is as efficient as chlorhexidine to prevent contamination of blood cultures.

BACKGROUND: False-positive blood cultures can lead to unnecessary risks and misuse of antibiotics; to reduce rates of false-positives, it would be useful to determine whether use of an antiseptic with a prolonged effect is required. METHODS: Clinical study of efficacy (blinded and randomized) to compare the rate of blood culture contamination when skin antisepsis was performed with 70% isopropyl alcohol or 2% chlorhexidine gluconate in 70% isopropyl alcohol in 2 hospitals. Patients aged 16 years or older with suspected bloodstream infection who were allocated in the emergency room, internal m

Skin Preparation Techniques for Orthopedic Foot and Ankle Surgery: A Current Concepts Review.

Surgical site infection (SSI) following orthopedic foot and ankle surgery is associated with morbidity, mortality, and cost to the healthcare system. The local anatomy and physiology predispose patients undergoing procedures to elevated risk. In particular, sterilizing of the webspaces, nailfolds, and subungual areas presents challenges, and the preferred skin antisepsis technique for foot and ankle procedures has not yet been defined. Skin antiseptic solutions (SASs) consist of 3 main categories: alcohol, chlorhexidine, and iodine-based solutions. This review of the literature supports a comb

Impact of evidence-based interventions on wound complications after cesarean delivery.

BACKGROUND: A number of evidence-based interventions have been proposed to reduce post-cesarean delivery wound complications. Examples of such interventions include appropriate timing of preoperative antibiotics, appropriate choice of skin antisepsis, closure of the subcutaneous layer if subcutaneous depth is ≥2 cm, and subcuticular skin closure with suture rather than staples. However, the collective impact of these measures is unclear. OBJECTIVE: We sought to estimate the impact of a group of evidence-based surgical measures (prophylactic antibiotics administered before skin incision, chlor

Reducing rates of endophthalmitis from intravitreal injections - strategies and areas of controversy.

PURPOSE OF REVIEW: Post-injection endophthalmitis (PIE) is the most concerning complication that accompanies intravitreal injections. This review discusses the recent literature in endophthalmitis prophylaxis including types of antisepsis, the use of topical antibiotics, methods of anesthesia, masking, and office-based versus operating room-based injections. RECENT FINDINGS: Povidone iodine (PI) remains the gold standard for PIE prophylaxis. Chlorhexidine gluconate (CHG) is an alternative antiseptic agent utilized in other areas of medicine with similar broad spectrum antibacterial activity.
