This is a case report involving a 71 year old man with generalized tetanus. The patient was initially treated conservatively with sedatives and muscle relaxants, which necessitated intubation and mechanical ventilation. After intrathecal administration of baclofen all cramps and spasms subsided and the patient could be weaned from the respirator and subsequently discharged from the intensive care unit.

Oral baclofen, the most often prescribed antispastic drug, has been shown to be effective in trigeminal neuralgia. Spinal application of baclofen leads to a complete supression of spasticity even in cases in which no previous oral antispastic medication achieved a response. A programmable baclofen pump was implanted in 7 patients for relief of severe spasticity. The patients experienced improvement in bladder and sphincter function. This system would seem to be of value in improving lower urinary tract function in patients with diseases that produce marked spasticity.

1. Baclofen given intraperitoneally (i.p.) to rats caused a dose-dependent decrease in food intake. 2. Bicuculline or picrotoxin (GABAA-antagonist) and methergoline (5-HT antagonist) decreased the anorectic effect of baclofen. 3. Pimozide (dopamine receptor blocker), phenoxybenzamine and propranolol (alpha and beta adrenergic blockers) did not diminish the baclofen effect, but even increased the anorexia induced by the drug. 4. It can be postulated that, at least partially, GABAA receptor mechanism, GABA-5HT receptor complex and/or 5-HT mechanism may be involved in baclofen induced anorexia.

Six patients with long-lasting spasticity resistant to different drug therapies including oral baclofen received a bolus injection of lumbar intrathecal baclofen. OBJECTIVES: To examine dosing patterns in patients receiving baclofen via intrathecal baclofen pumps to assess for common patterns by diagnosis, ambulation ability, and affected limbs distribution.
MATERIALS AND METHODS: This trial study included 25 patients with baclofen pumps selected from the 356 patients enrolled in our center's baclofen pump program. Selection was done by splitting all patients into diagnostic categories of stroke, multiple sclerosis, traumatic/anoxic brain injury, cerebral palsy, and spinal cord injury, and then, five patients were randomly selected from each diagnosis.

Baclofen is a highly used centrally acting GABA agonist that continues to be an effective therapy for spasticity and chronic hiccups. The renally dependent excretion determines the circulating concentrations and guides effective dosing to decrease adverse reactions. Caution should be considered in administering baclofen to patients with decreased renal function. We present a patient with end stage renal disease on hemodialysis with recent baclofen ingestion who presented with toxic encephalopathy that was resolved with additional dialysis sessions.
