Blood pressure measurement and monitoring.

Recent developments in blood pressure measurement have emphasized recordings taken outside the physician's office. Such readings can recognize the spuriously high levels recorded in "white coat" hypertension. Nocturnal and early morning readings can detect silent complications and impending cardiovascular accidents.

[Blood pressure variability and morbidity].

In a previous study (resting blood pressure profile, Dinamap) we have confirmed the correlations between blood pressure and left ventricular mass (myocardial hypertrophy being one of the morbidity criteria in Hypertension) and we have demonstrated the absence of significant difference (Fisher's z Test) with the results of ambulatory recordings found in literature. Furthermore, we have showed a weaker correlation between absolute variability (AV) and left ventricular mass (LVM). If indeed there exists a cause-effect relationship between the AV increase and the LVM increase, a relative independe

Calcium--osteoporosis and blood pressure.

Calcium is the main essential mineral for building strong bones and sound teeth. Bone remodeling occurs throughout life; new bone is deposited and existing bone is replaced. Peak bone mass development is obtained up to 35 yr of age after which bone resorption is increased. It is therefore imperative that Ca intake meet the recommended dietary allowances not only during the growing period but throughout life. Milk and dairy products are the major food source of Ca in the US diet, supplying about 61% of our intake while other food groups supply the rest. It is almost impossible to supply the rec

Dietary salt and blood pressure.

Research evidence on the role of dietary sodium in the etiology and pathogenesis of hypertension is briefly reviewed. This matter is assuming new importance at present, given new data on the efficacy of normalization of blood pressure for adults with so-called "mild" hypertension (average diastolic 90-104 mm Hg), hence the need for safe nutritional-hygienic alternatives to years-long drug treatment for millions of people with such hypertension. Two trials by the authors deal with some unresolved questions in this area. The first, a preliminary study, involved 21 lacto-ovo-vegetarian high schoo

Blood pressure trends with aging.

In industrialized societies, blood pressure increases with age, and blood pressure at one age is related to blood pressure at an earlier age. Blood pressure is also related to weight, weight change, and maturation. This paper reviews the association of growth and maturation with blood pressure and the evidence for blood pressure "tracking" with age. Additional longitudinal studies are required to determine if blood pressures before puberty are related to blood pressures of sexually mature young adults. Adolescents with "gestational" hypertension also have relatively high blood pressures at lon

Suboptimal Levothyroxine Treatment is Associated with Increases in Mean Blood Pressure and Blood Pressure Variability.

CONTEXT: Thyroid hormone homeostasis is critical to the metabolic function of cardiovascular tissues. The degree to which the suboptimal treatment of hypothyroidism with levothyroxine (LT4) impacts blood pressure (BP) and blood pressure variability (BPV) is not known. OBJECTIVES: Determine the relationship between LT4 time above range (TAR) (TSH level >4.5 mIU/L), time below range (TBR) (TSH <0.4 mIU/L), and changes in annual mean systolic (SBP) and diastolic blood pressure (DBP) and visit-to-visit BPV. METHODS: Using longitudinal data from 2203 adult patients treated with LT4, we modeled wi

Attention to blood pressure cuff sizes is important for home and in-clinic blood pressure measurement.

Arm circumference determines appropriate blood pressure cuff size, which is critical for accurate measurements. This cross-sectional analysis aimed to assess cuff size needs according to mid-arm circumferences of Australian adults. Based on typical in-clinic cuff sizes, most Australians would require a medium (51.7%) or large (44.5%) cuff, which means that the cuff would need to be changed for almost every second patient. Most home devices are supplied with a standard (22-32 cm) or wide-range (22-42 cm) cuff, these were found to be unsuitable for 9 million adults (48.3%) and over 701,995 adult

Office blood pressure measurements with oscillometric devices in adolescents: a comparison with home blood pressure.

PURPOSE: Compare multiple in office BP measurements in adolescents using an oscillometric device with out-of-office blood pressure measurements (home blood pressure monitoring - HBPM). MATERIALS AND METHODS: Office measurements were performed with validated semi-automatic devices twice (3 minutes interval) in two different moments (1 week apart), with a total of four readings. These BP readings were named R1, R2, R3 and R4 (following the sequence they were performed), FDM (mean of two readings on first day) and SDM (mean of two readings on second day) and SRM (R2-R4 means). The HBPM protocol

Home blood pressure in children and adolescents: a comparison with office and ambulatory blood pressure measurements.

AIM: To compare BP measurements of children and adolescents using different methods office BP (OBP), ambulatory BP monitoring (ABPM) and home BP measurement (HBPM) and to study their correlations. METHOD: Individuals were evaluated between 5 and 15 years of age who had been referred because of a previous high BP. OBP was measured with the OMRON-705CP. Three measurements were carried out at 5-min intervals. HBPM were taken using the same device, two measurements at 5-min intervals in the morning and in the evening during 7 days. ABPM was performed using the SpaceLabs 90207 monitors. RESULTS:

Blood pressure in acute ischemic stroke and mortality: a study with noninvasive blood pressure monitoring.

BACKGROUND: In acute ischemic stroke, a transient elevation of blood pressure is common; its significance and its relationship with the neurological outcome are still unclear. METHODS: In 71 consecutive patients with acute ischemic stroke, aged 25-94 years, admitted to our unit, we investigated the relationships between the blood pressure variation during the first 24 h and mortality at 3 months (study endpoint). Neurological status was assessed with the National Institutes of Health Stroke Scale. Blood pressure was measured with bedside noninvasive blood pressure monitoring every 15 min. The

Hypertensive complications and home blood pressure: comparison with blood pressure measured in the doctor's office.

We have compared hypertensive target organ damage with home blood pressure readings (HBPs) and with office blood pressure readings (OBPs) in 100 patients with mild to moderate essential hypertension. The correlation between blood pressure levels and hypertensive target organ damage in HBPs and OBPs were similar (r = .42, p less than 0.001 for systolic HBPs; r = .33, p less than 0.001 for diastolic HBPs; r = .42, p less than 0.001 for systolic OBPs; r = .34, p less than 0.001 for diastolic OBPs). In most instances, HBPs were lower than corresponding OBPs. Among individual patients whose OBPs we

Disparities in Blood Pressure Measurement.

PURPOSE OF REVIEW: We aimed to examine how inaccuracies and inequities in blood pressure (BP) measurement across office, ambulatory, home, and emerging cuffless modalities contribute to disparities in hypertension diagnosis and control, and to identify strategies to improve measurement equity. RECENT FINDINGS: Disparities arise from nonstandardized office technique, inappropriate cuff sizing, limited access to validated home and ambulatory devices, reimbursement and implementation barriers, and underrecognition of masked and nocturnal hypertension in populations at higher risk. Cuffless techn

Blood pressure variability: a review.

Blood pressure variability (BPV) predicts cardiovascular events independent of mean blood pressure. BPV is defined as short-term (24-h), medium or long- term (weeks, months or years). Standard deviation, coefficient of variation and variation independent of the mean have been used to quantify BPV. High BPV is associated with increasing age, diabetes, smoking and vascular disease and is a consequence of premature ageing of the vasculature. Long-term BPV has been incorporated into cardiovascular risk models (QRISK) and elevated BPV confers an increased risk of cardiovascular outcomes even in sub

Blood pressure, frailty and dementia.

High blood pressure (BP) affects 75% of people aged over 70. Ageing alters BP homeostasis, resulting in postural hypotension and increased BP variability. Co-morbidity and frailty add complexity to understanding BP changes in later life. Longitudinal BP declines are likely driven by accumulating co-morbidity and are accelerated in both frailty and dementia. This narrative review summarises what is known about the association between BP and frailty, the clinical management of BP in frailty and the association between BP, cognitive decline and dementia.

Blood Pressure Management in Stroke.

Hypertension is a well-established and modifiable risk factor for stroke and other cardiovascular diseases. Notably, stroke is the second leading cause of death worldwide and the second most common cause of disability-adjusted life-years. As such, we provide a viewpoint on blood pressure management in stroke and emphasize blood pressure control or management for first and recurrent stroke prevention, acute stroke treatment, and for prevention of cognitive impairment or dementia.

Machine learning and blood pressure.

Machine learning (ML) is a type of artificial intelligence (AI) based on pattern recognition. There are different forms of supervised and unsupervised learning algorithms that are being used to identify and predict blood pressure (BP) and other measures of cardiovascular risk. Since 1999, starting with neural network methods, ML has been used to gauge the relationship between BP and pulse wave forms. Since then, the scope of the research has expanded to using different cardiometabolic risk factors like BMI, waist circumference, waist-to-hip ratio in concert with BP and its various pharmaceutic
