Traumatic brain injury and disorders of consciousness.

Trauma is one of the most common causes of disorders of consciousness (DOC) worldwide. Traumatic brain injury (TBI) leads to heterogeneous, multifocal injury via focal brain damage and diffuse axonal injury, causing an acquired network disorder. Recovery occurs through reemergence of dynamic cortical and subcortical networks. Accurate diagnostic evaluation is essential toward promoting recovery and may be more challenging in traumatic than non-traumatic brain injuries. Standardized neurobehavioral assessment is the cornerstone for assessments in the acute, prolonged, and chronic phases of trau

Review of Ventilation in Traumatic Brain Injury.

Acute brain injury is a prominent admitting diagnosis of critically ill patients, often requiring endotracheal intubation to protect the airway and resulting in respiratory failure and the need for mechanical ventilation. Following brain injury, a primary focus is avoidance of secondary insults including both hypercarbia and hypoxemia. Hyperoxemia may also result in unanticipated neurologic consequences. Brain-lung crosstalk refers to complex relationships that drive iatrogenic injury in both organs, mediated by inflammation, immunosuppression, and autonomic dysfunction. In an effort to furthe

Coagulopathy prediction in traumatic brain injury.

Traumatic brain injury (TBI) represents a significant public health concern. Besides the initial primary injury, a defining point of TBI is causing secondary, delayed damage through inflammatory biochemical processes. Among the complications arising from this inflammatory response, coagulopathy emerges as a critical concern. With an overall prevalence of 32.7 %, TBI-induced coagulopathy significantly contributes to increased mortality rates and unfavorable patient outcomes, through its clinical manifestations, such as progressive hemorrhagic injury (PHI). This chapter investigates biomarkers c

GPCRs in traumatic brain or spinal injury.

G protein-coupled receptors (GPCRs) are the target for nearly one-third of currently marketed drugs. Central nervous system (CNS) impacts, for example traumatic brain injury (TBI), subarachnoid hemorrhage (SAH) or intracerebral hemorrhage (ICH), as well as spinal cord injury (SCI) frequently result in disability or death worldwide. In this review we aimed to discuss the role of GPCRs in traumatic brain and spinal injury. Firstly, we provide evidence that whether the endogenous ligands or the expression of GPCRs are changed upon trauma to the brain or spine. Secondly, we discuss potential benef

Immune Response in Traumatic Brain Injury.

PURPOSE OF REVIEW: This review aims to comprehensively examine the immune response following traumatic brain injury (TBI) and how its disruption can impact healing and recovery. RECENT FINDINGS: The immune response is now considered a key element in the pathophysiology of TBI, with consequences far beyond the acute phase after injury. A delicate equilibrium is crucial for a healthy recovery. When this equilibrium is disrupted, chronic inflammation and immune imbalance can lead to detrimental effects on survival and disability. Globally, traumatic brain injury (TBI) imposes a substantial burde

Neuropsychological Evaluation in Traumatic Brain Injury.

Neuropsychological evaluations can be helpful in the aftermath of traumatic brain injury. Cognitive functioning is assessed using standardized assessment tools and by comparing an individual's scores on testing to normative data. These evaluations examine objective cognitive functioning as well as other factors that have been shown to influence performance on cognitive tests (eg, psychiatric conditions, sleep) in an attempt to answer a specific question from referring providers. Referral questions may focus on the extent of impairment, the trajectory of recovery, or ability to return to work,

Neuroworsening in Moderate Traumatic Brain Injury.

Patients with moderate traumatic brain injury (TBI) are at high risk for developing intracerebral complications and in particular neuroworsening (NW). NW can be unpredictable and may be an important risk factor for poor neurologic outcome and for increased mortality. NW is often a medical and surgical emergency, and it is, therefore, fundamental to identify patients at risk early because they require strict neuromonitoring and repeated neuroimaging. So far, there is no standardized and validated definition of NW. In this review, we aim to discuss the definition, risk factors, and management of

[Science of Mild Traumatic Brain Injury].

Mild traumatic brain injury (TBI) is commonly encountered in daily medical practice. Approximately half of all patients with mild TBI develop postconcussional syndrome, and post-traumatic higher brain dysfunction is a cause of many social problems. This article defines and explains the diagnosis, pathology, symptoms, and sequelae of mild TBI from a scientific perspective, and "mild blast-induced TBI," which has recently become a major problem in military medicine, is also introduced.

Traumatic Brain Injury: Acupuncture Treatment.

Traumatic brain injury (TBI) is a significant cause of morbidity and mortality worldwide. It is estimated that half of the people in the world will experience at least 1 episode of TBI during their lifetimes. While the primary injury to the brain parenchyma is usually irreversible, the secondary effects, which involve cellular dysfunction, derangement of blood flow, and blood-brain barrier changes in ionic flux and elevated levels of free radicals are potentially amenable to treatment. At present, there are no neuroprotective agents in mainstream medicine to interrupt these secondary processes

An Audit of Tracheostomy in Traumatic Brain Injury.

INTRODUCTION: The timing of tracheostomy in traumatic brain injury is controversial. The benefits of early tracheostomy are early weaning from ventilation, and reduction in pneumonia. But some studies demonstrated increase in intracranial pressure during tracheostomy. AIM AND METHODS: The aims of the audit are to analyse the timing, benefits, complications, and the results of tracheostomy in patients with traumatic brain injury in the hospital in the period of 2012-2021. RESULTS: The number of patients was 34. The maximum number of tracheostomies(24) were between 6th to 14th days of admissio

Mobilization phases in traumatic brain injury.

Mobilization in traumatic brain injury (TBI) have shown the improvement of length of stay, infection, long term weakness, and disability. Primary damage as a result of trauma's direct effect (skull fracture, hematoma, contusion, laceration, and nerve damage) and secondary damage caused by trauma's indirect effect (microvasculature damage and pro-inflammatory cytokine) result in reduced tissue perfusion & edema. These can be facilitated through mobilization, but several precautions must be recognized as mobilization itself may further deteriorate patient's condition. Very few studies have discu

[Care for patients with traumatic brain injury].

Traumatic brain injury (TBI) as well as Attention Deficit Disorder with or without hyperactivity (ADHD) are very common problems that affect children. It is known that patients who suffer a traumatic brain injury may present symptoms of ADHD, which often go unnoticed in the acute period, especially when there are more serious injuries that hide them and are only evident when the patient returns to their regular cognitive activity after discharge. Symptoms can vary depending on the mechanism of injury, the location in the brain where the trauma or its effects occur, complications, and the sever

Antifibrinolytics in the treatment of traumatic brain injury.

PURPOSE OF REVIEW: Traumatic brain injury (TBI) is a leading cause of trauma-related deaths, and pharmacologic interventions to limit intracranial bleeding should improve outcomes. Tranexamic acid reduces mortality in injured patients with major systemic bleeding, but the effects of antifibrinolytic drugs on outcomes after TBI are less clear. We therefore summarize recent evidence to guide clinicians on when (not) to use antifibrinolytic drugs in TBI patients. RECENT FINDINGS: Tranexamic acid is the only antifibrinolytic drug that has been studied in patients with TBI. Several recent studies

Sleep Disorders in Traumatic Brain Injury.

Sleep disorders are common after traumatic brain injury (TBI). This study will review the spectrum and proposed mechanisms of TBI-associated sleep disorders and discuss the clinical approach to diagnosis and management of them. Disordered and fragmented sleep with insomnia and daytime sleepiness is very common after TBI. Sleep disruption contributes to morbidity and neurocognitive and neurobehavioral deficits and prolongs the recovery phase after injury. Early recognition and correction of these problems may limit the secondary effects of TBI and improve patient outcomes. Evaluating sleep diso

Traumatic Brain Injury in Child Burn.

BACKGROUND: Burns are one of the most important health problems in communities. Traumatic injuries, especially Traumatic Brain Injury (TBI) associated with burns, may increase disability and mortality. In addition to preventing burns, any action for a better treatment approach and early detection of concomitant traumatic injuries can reduce complications, disability, and treatment costs. We aimed to investigate the outcome of children with burn injury with and without TBI. METHODS: In this cross-sectional study, 392 children with burn injuries treated at Motahari Hospital in Tehran, Iran from

Advances in Traumatic Brain Injury Biomarkers.

Traumatic brain injury (TBI) is increasingly a major cause of disability across the globe. The current methods of diagnosis are inadequate at classifying patients and prognosis. TBI is a diagnostic and therapeutic challenge. There is no Food and Drug Administration (FDA)-approved treatment for TBI yet. It took about 16 years of preclinical research to develop accurate and objective diagnostic measures for TBI. Two brain-specific protein biomarkers, namely, ubiquitin C-terminal hydrolase-L1 and glial fibrillary acidic protein, have been extensively characterized. Recently, the two biomarkers we

Mild Traumatic Brain Injury in Children.

Mild traumatic brain injury accounts for an estimated 4.8 million cases of pediatric traumatic brain injuries worldwide every year. In the United States, 70% of mild traumatic brain injury cases are due to sports and recreational injuries. Early diagnosis, especially in active children, is critical to preventing recurrent injuries. Management is guided by graded protocols for returning to school and activity. Ninety percent of children recover within 1 month of injury. Promising research has shown that early referral to specialty concussion care and multidisciplinary treatment with physical an

Neurobehavior and Mild Traumatic Brain Injury.

Neurobehavioral sequelae after mild traumatic brain injury are multifactorial, often necessitating a multidisciplinary approach. Neurobehavioral sequelae generally resolve within 3 months; when more persistent, a search for contributing factors beyond a brain injury should be done. To accomplish this, a systematic and comprehensive evaluation is recommended to place the complaint in context of the patient's premorbid state. The treatment of neurobehavioral sequelae cannot be accomplished without a clear understanding of the underlying cause, and the treatment must be placed within a patient's
