Apnea detection using wrist actigraphy in patients with heterogeneous sleep disorders.

Obstructive sleep apnea (OSA) and related hypoxia are well-established cardiovascular and neurocognitive risk factors. Current multi-sensor diagnostic approaches are intrusive and prone to misdiagnosis when simplified. This study introduces an enhanced single-sensor-based OSA screening method, leveraging novel signal processing and machine learning to ensure accurate detection across diverse populations. Wrist actigraphy is a widely-used and energy-efficient tool for respiratory rate estimation. The main challenge in OSA pattern recognition is handling various disturbances in real-world applic

Enzyme-converted O kidneys allow ABO-incompatible transplantation without hyperacute rejection in a human decedent model.

ABO-incompatible kidney transplantation is widely used to meet the escalating need for organs. Current recipient-centric desensitization protocols involving antibody depletion through plasmapheresis increase the risk of infections, perioperative bleeding events and costs. Here we present a donor-centric desensitization protocol, converting type-A kidneys into enzyme-converted O kidneys during hypothermic perfusion to remove the A antigen from the kidneys. An ex vivo model resulted in no antibody-mediated injury. Encouraged by this, an enzyme-converted O kidney was transplanted into a type-O br

The relations between the stress in temporomandibular joints and the deviated distances for mandibular asymmetric patients.

The study aimed to compare the difference of stress distributions in temporomandibular joints (TMJs) between the patients with mandibular asymmetry and asymptomatic subjects and find the relations between deviated distance and biomechanical stress using three-dimensional finite element method, to give guidance to dentists for correction of mandibular asymmetry. Ten facial symmetric subjects without symptoms of temporomandibular disorders (TMD) and 10 mandibular asymmetric patients were recruited and assigned as the Control and Case group respectively. The FE models of the mandible and maxilla

Inflammatory Papillary Hyperplasia: A Diagnostic Challenge to Oral Physicians.

Inflammatory papillary hyperplasia is a benign, non-neoplastic proliferation of epithelial lining the oral cavity that affects the palatal mucosa with one or more granular, pebbly, nodular, grape, or leaf-like growths. It usually occurs as a soft tissue overgrowth on the hard palate mucosa. It commonly occurs among prolonged or ill-fitting denture wearers, occlusal disharmony resulting from defective occlusal contacts or imbalance between centric relation and occlusion, chronic use of steroid inhalers among asthmatics, immunosuppression like uncontrolled diabetes and AIDS in HIV, poor oral and

Design Requirements for Cardiac Telerehabilitation Technologies Supporting Athlete Values: Qualitative Interview Study.

BACKGROUND: Cardiac telerehabilitation (CTR) interventions can provide accessible and affordable remote rehabilitation services. However, as cardiac rehabilitation (CR) primarily targets inactive patients, little is known about the experiences with CR of highly active patients (ie, recreational athletes or, simply, athletes) with established coronary artery disease. Consequently, existing CTR interventions do not address the specific needs of the athletic subpopulation. Understanding the needs and values of athletes is crucial for designing meaningful CTR interventions that enhance user accept

Weight-centric pharmacological management of type 2 diabetes mellitus - An essential component of cardiovascular disease prevention.

Obesity and overweight are contributing factors for diseases such as type 2 diabetes mellitus (T2DM), hypertension, hyperlipidemia, and ultimately, cardiovascular (CV) disease. Obesity is imposing an increasing health burden in rich and poor nations, with almost 30% of people globally now either obese or overweight - a staggering 2.1 billion. The link between obesity and T2DM is widely held to involve two adverse effects: obesity-induced insulin resistance and β-cell failure. This "unified field theory" raises questions about whether defects favoring progressive weight gain and metabolic impai

Evaluation of the condylar position in younger and older adults with or without temporomandibular symptoms by using cone beam computed tomography.

STATEMENT OF PROBLEM: Recent studies in young adults have questioned the influence of discrepancies between central and maximal intercuspal positions in temporomandibular disorders (TMDs). However, whether this finding was because this type of etiologic agent requires time for the development of changes in the condylar position is unclear. PURPOSE: The purpose of this clinical study was to use cone beam computed tomography (CBCT) to evaluate the presence or absence of discrepancies between the centric relation (CR) and maximum intercuspation (MI) in younger and older adults in populations wit

Dental malocclusion is not related to temporomandibular joint clicking: a logistic regression analysis in a patient population.

OBJECTIVES: To assess the association of several dental malocclusion features with temporomandibular joint (TMJ) click sounds in a population of temporomandibular disorder (TMD) patients. MATERIALS AND METHODS: Four hundred forty-two TMD patients (72% female; 32.2 ± 5.7 years, range 25-44 years) were divided into a TMJ clicking and a no-TMJ clicking group, based on the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) assessment. Seven occlusal features were recorded for each patient: (1) posterior crossbite, (2) overbite, (3) open bite, (4) overjet, (5) mediotrusive and

Midline shift and lateral guidance angle in adults with unilateral posterior crossbite.

INTRODUCTION: Unilateral posterior crossbite is a malocclusion that, if not corrected during infancy, typically causes permanent asymmetry. Our aims in this study were to evaluate various occlusal parameters in a group of adults with uncorrected unilateral posterior crossbite and to compare findings with those obtained in a group of normal subjects. METHODS: Midline shift at maximum intercuspation, midline shift at maximum aperture, and lateral guidance angle in the frontal plane were assessed in 25 adults (ages, 17-26 years; mean, 19.6 years) with crossbites. RESULTS: Midline shift at maxim

Evolution in the treatment of gastroesophageal reflux disease over the last century: from a crural-centered to a lower esophageal sphincter-centered approach and back.

The surgical management of gastroesophageal reflux disease (GERD) has evolved significantly over the past century, driven by increased understanding of the physiology of the reflux barrier, its anatomic components, and surgical innovation. Initially, emphasis was on reduction of hiatal hernias and crural closure as the etiology behind GERD was felt to be solely related to the anatomic alterations caused by hiatal hernias. With persistence of reflux-related changes in some patients despite crural closure, along with the development of what is now modern manometry and the discovery of a high-pre

Empowering Patients in Clinical Research: A Narrative Review.

This narrative review focuses on the evolving shift toward patient centricity in Indian clinical research, highlighting persistent gaps in structured patient engagement. The review synthesizes existing literature on patient-centered clinical research and contextualizes these principles through two India-based initiatives: Patient Advocates for Clinical Research (PACER) and Patient Recommendations for Ethical Decentralized and Inclusive Clinical Trials (PREDICT). A targeted literature search was conducted across Medline (via PubMed), Google Scholar, and the Cochrane Library to identify publishe

Chromosomes of Damaliscus (Artiodactyla, Bovidae): simple and complex centric fusion rearrangements.

G- and C-banded karyotypes of Damaliscus hunteri, D. lunatus and D. pygargus were compared using the standard karyotype of Bos taurus. Chromosomal complements were 2n = 36 in D. lunatus jimela, 2n = 38 in D. pygargus phillipsi and D. p. pygargus, and 2n = 44 in D. hunteri. The fundamental number in all karyotypes was 60. Among the three species of Damaliscus, seven autosomal pairs and the X chromosomes were conserved. Y-chromosome differences were attributed to heterochromatic additions or deletions. Banded karyotypes of the two subspecies of D. pygargus exhibited complete homology. Chromosoma

Recognizing the Role of Insulin Resistance in Polycystic Ovary Syndrome: A Paradigm Shift from a Glucose-Centric Approach to an Insulin-Centric Model.

Polycystic ovary syndrome (PCOS) is a common metabolic-endocrine disorder affecting women of reproductive age, and insulin resistance (IR) is a key pathophysiological feature. Current medical education and clinical practice emphasize glucose-centric approaches in PCOS management, and IR testing is often overlooked due to limited emphasis in current clinical guidelines or the lack of standardized protocols. Additionally, the glucose-focused paradigm has been the standard of care for decades. However, this approach has led to delayed diagnosis of progressive metabolic and reproductive consequenc
