The superficial temporal artery trunk as a donor vessel in cerebral revascularization: benefits and pitfalls.

OBJECT: The superficial temporal artery (STA) is the mainstay of donor vessels for extracranial-intracranial bypass in cerebral revascularization. However, the typically used STA anterior or posterior branch is not always adequate in its flow-carrying capacity. In this report the authors describe the use of the STA trunk at the level of the zygoma as an alternative donor and highlight the benefits and pitfalls of this revascularization option. METHODS: The authors reviewed the cases of 4 patients in whom the STA trunk was used as a donor site for anastomosis of a short interposition vein graf

Changes in cerebral blood flow in the postoperative chronic phase after combined cerebral revascularization for moyamoya disease with ischaemic onset.

The purpose of this study was to examine the effects of combined revascularization for ischaemic-onset moyamoya disease (MMD) on cerebral haemodynamics by comparing cerebral blood flow (CBF) during the postoperative chronic phase with preoperative CBF. A retrospective cohort of 24 MMD patients (representing 31 surgeries) who received single photon emission computed tomography (SPECT) before and more than 6 months after surgery was investigated. The CBF value of each vascular territory was extracted from SPECT data, and the value relative to the ipsilateral cerebellar value (relative CBF, or RC

Longitudinal neurocognitive outcomes and predictors after unilateral combined cerebral revascularization in adult moyamoya vasculopathy.

OBJECTIVE: While most studies on adult moyamoya vasculopathy (MMV) have focused on the stroke incidence after bypass surgery, neurocognitive outcomes and predictors have rarely been investigated. The aim of this study was to evaluate the neurocognitive outcomes of adult patients with MMV after combined revascularization surgery and identify factors contributing to unfavorable outcomes. METHODS: Adult patients with MMV who underwent combined revascularization surgery from March 2019 to July 2024 at a single center were prospectively observed. Neuropsychological assessments and DSA were perform

Troubleshooting the bypass: intraoperative management of initially failed anastomosis in direct cerebral revascularization surgery.

OBJECTIVE: Direct cerebral revascularization is considered as one of the most technically challenging operations in neurosurgery. Technical errors are often not identified during the case, but only after the recirculation stage, making management crucial at that time of the procedure. In this study, the authors sought to describe troubleshooting of the technical errors encountered in initially failed bypass cases. METHODS: A retrospective analysis describing a single-surgeon, single-institution experience between 2014 and 2021 was performed, based on operative reports and videos, including a

Cerebral monitoring during transcarotid artery revascularization with flow reversal via transcranial doppler ultrasound examination.

OBJECTIVE: Transcarotid artery revascularization (TCAR) is a hybrid technique for carotid artery revascularization that relies on proximal carotid occlusion with flow reversal for distal embolic protection. The hemodynamic response of the intracranial circulation to flow reversal is unknown. In addition, the rate and pattern of cerebral embolization during flow reversal has yet to be investigated. The aim of this study was to characterize cerebral hemodynamic and embolization patterns during TCAR. METHODS: A single-institution retrospective study of patients with carotid artery stenosis under

Temporary cerebral revascularization using a radial artery in the hand-eyeshade posture: technical note.

Kamiyama introduced a unique method of temporary cerebral revascularization using a radial artery graft (RAG) in his technical review. We tried original method with Sugita frame, and pointed out some disadvantages that include to avoid taking instruments or hands in and out, stability of hands, and unrestricted usage of brain retractors during temporary bypass between radial artery and M2 potion of middle cerebral artery (RA-M2 bypass). To solve those disadvantages, especially for Sugita head frame users, we here present a modified Kamiyama's method of temporary cerebral revascularization with

Microsurgical Revascularization for Thrombosed Middle Cerebral Artery Dissecting Aneurysms: Classification Scheme and Surgical Strategies.

OBJECTIVE: Thrombosed dissecting aneurysms represent highly challenging lesions in neurosurgery. Among these, thrombosed middle cerebral artery (MCA) dissecting aneurysms are clinically rare and exceptionally difficult to manage. Their defining feature is the extensive organized thrombus within the aneurysmal lumen and the subsequent progressive mass effect, which presents a significant dilemma for traditional endovascular treatment and microsurgical clipping to balance safety and efficacy. To address this, the authors summarize the clinical experience of a single senior surgeon at our center

Coronary-Driven Cerebral Revascularization in Type 2a Takayasu Arteritis: A Rare Multisystem Collateral Phenotype.

Takayasu arteritis (TA) is a chronic, granulomatous large-vessel vasculitis affecting the aorta and its branches. This report describes a 33-year-old woman with longstanding, unsupervised disease who developed extensive collateral revascularization, including rare coronary-carotid and coronary-vertebral pathways. Coronary angiography revealed normal epicardial coronary arteries but demonstrated collateral filling of the carotid system from the left coronary circulation. Computed tomography angiography identified complete occlusion of all aortic arch branches and extensive systemic, thoracic, p

Treatment of Progressive Herpes Zoster-Induced Vasculopathy with Surgical Revascularization: Effects on Cerebral Hemodynamics.

BACKGROUND: Herpes zoster ophthalmicus (HZO) is caused by reactivation of the herpes simplex virus in the trigeminal nerve. HZO-initiated cerebral vasculopathy is well characterized; however, there are no documented cases that report the efficacy of surgical revascularization for improving cerebral hemodynamics following progressive HZO-induced vasculopathy. We present a case in which quantitative anatomic and hemodynamic imaging were performed longitudinally before and after surgical revascularization in a patient with HZO and vasculopathic changes. CASE DESCRIPTION: A 57-year-old female wit

Percutaneous revascularization of sole arch artery for severe cerebral ischemia resulting from Takayasu arteritis.

PURPOSE: Takayasu arteritis (TA) is a rare form of inflammatory arteriopathy affecting the aorta and its major branches. Obstructive lesions of all arch vessels may lead to disabling neurologic symptoms. There is limited experience with endovascular revascularization in this situation. The present report describes immediate and follow-up results with stent-supported angioplasty of severely stenosed single patent arch arteries. MATERIALS AND METHODS: Angioplasty and stent placement were performed in 10 consecutive patients with TA (age, 11-42 years; mean, 28.3 y +/- 4.1) with cerebrovascular s

Symptomatic cavernous sinus aneurysms: management and outcome after carotid occlusion and selective cerebral revascularization.

BACKGROUND AND PURPOSE: Therapeutic internal carotid artery (ICA) occlusion for symptomatic intracavernous artery aneurysms can result in ischemic infarction despite normal clinical balloon test occlusion (BTO). We evaluated outcomes in patients with symptomatic cavernous sinus aneurysms in whom clinical BTO was normal, who underwent carotid occlusion with selective bypass surgery guided by physiologic BTO using quantitative cerebral blood flow (CBF) analysis by means of stable xenon-enhanced CT. METHODS: After a normal clinical BTO, 26 consecutive patients with symptomatic cavernous sinus an

EFFECT OF DIRECT SURGICAL REVASCULARIZATION ON CEREBRAL HEMODYNAMICS AND STROKE DEVELOPMENT IN PATIENTS WITH MOYAMOYA DISEASE.

Decreased cerebral blood flow (CBF) leads to impaired cerebral hemodynamics, which causes an increased risk of stroke. Revascularization has been shown to improve CBF in patients with moyamoya disease. The study is devoted to the retrospective study of clinical features and cerebral hemodynamic characteristics of 17 patients with moyamoya disease before, during and after surgical treatment using extracranial-intracranial (EC-IC) bypass by STA-MCA type. Patients underwent superficial temporal artery-middle cerebral artery bypass surgeries. All patients were carried out by DSA, MSCT-angiography,

Effects of Aspirin Therapy on Bypass Efficacy and Survival of Patients Receiving Direct Cerebral Revascularization.

Background: Both patency maintenance and neoangiogenesis contribute to cerebrovascular bypass efficacy. However, the combined impact of the aforementioned two indicators on postoperative revascularization following superficial temporal artery-to-middle cerebral artery (STA-MCA) bypass has been less well elucidated. Meanwhile, there is a paucity of evidence with conflicting results about postoperative aspirin therapy. Objective: The objective of the study was to investigate the correlation between aspirin use and STA-MCA bypass efficacy, including patency, postoperative neoangiogenesis, and fol

Surgical Revascularization for the Treatment of Complex Anterior Cerebral Artery Aneurysms: Experience and Illustrative Review.

BACKGROUND: Many methods for treating complex anterior cerebral artery (ACA) aneurysms are available; however, there is substantial variation among methods because of various aneurysm locations, the relationship of the aneurysm to arterial branches, aneurysm size and other morphologic characteristics, and the diameters of the parent or branching arteries. METHODS: We reviewed complex ACA aneurysms based on both our own experience and the available literature. Each unique case is analyzed in terms of the characteristics of the aneurysm, along with analysis and classification of the revasculari

The anterior temporal artery: an underutilized but robust donor for revascularization of the distal middle cerebral artery.

OBJECTIVE The anterior temporal artery (ATA) supplies an area of the brain that, if sacrificed, does not cause a noticeable loss of function. Therefore, the ATA may be used as a donor in intracranial-intracranial (IC-IC) bypass procedures. The capacities of the ATA as a donor have not been studied previously. In this study, the authors assessed the feasibility of using the ATA as a donor for revascularization of different segments of the distal middle cerebral artery (MCA). METHODS The ATA was studied in 15 cadaveric specimens (8 heads, excluding 1 side). First, the cisternal segment of the ar
