The Impact of Compulsivity and Impulsivity in Cerebellar Ataxia: A Case Series.

BACKGROUND: The cerebellum has recently been identified to have a key role in reward processing, and individuals with ataxia have been found to be more impulsive and compulsive as part of cerebellum-related cognitive and behavioral disturbances. CASE REPORT: We reported five individuals with cerebellar ataxia who demonstrate impulsive and compulsive behaviors, including hobbyism, gambling, and compulsive medication use, to illustrate that these symptoms can be highly disabling. DISCUSSION: These five cases provide examples of behavioral symptoms in cerebellar ataxia. Further investigations o

A neuroeconomic theory of obsessive-compulsive disorder.

While there are many studies on the neural bases of obsessive-compulsive disorder (OCD), there is a dire need for a neurocomputational framework to explain its symptoms. To this end, we use the Expected Value of Control (EVC) theory to conceptualize the information processing deficits underlying OCD. Specifically, we argue that when experiencing obsessions, weak cognitive control is favored due to the affective cost of disregarding anxiety-provoking obsessions (akin to ignoring a fire alarm). This affective cost leads to the overvaluation of the cost of cognitive control (deeming it expensive)

Efficacy of exposure and response prevention therapy in mixed reality for patients with obsessive-compulsive disorder: study protocol for a randomized controlled trial.

Many patients with obsessive-compulsive disorder (OCD) do not receive cognitive behavioral therapy with exposure and response prevention (first line treatment for OCD), for example, due to patients' fear of the exposure and reservations of the therapists. Technology-supported exposure (e.g., exposure therapy with response prevention in mixed reality [MERP]) for patients with OCD may help to overcome this obstacle. Building upon findings of our pilot study objectives of this study are to evaluate the efficacy, expectations of treatment success, feasibility, and acceptance of MERP as well as to

Psychological changes in young people with anorexia nervosa during an inpatient treatment: exploration of optimal length of stay predictors.

OBJECTIVE: The current study aimed to monitor psychological changes in young people (YP) with anorexia nervosa (AN) during a specialist inpatient treatment in order to explore possible predictors of an optimal length of stay. Outcome measures were analysed to determine if significant changes occur earlier or later on during the treatment. METHODS: Eating Disorder Examination Questionnaire (EDE-Q), Children's Depression Inventory (CDI), State-Trait Anxiety Inventory (STAI) and Compulsive Exercise Test (CET) measures administered to YP (n = 42) at three time points-admission, 85% ideal body wei

Obsessive-compulsive disorder induced by donepezil in a patient with Alzheimer's disease.

BACKGROUND: Donepezil, an acetylcholinesterase inhibitor commonly used to treat Alzheimer's disease (AD), is generally well tolerated. There have been no previous reports on donepezil-induced obsessive-compulsive disorder (OCD). CASE PRESENTATION: The patient, a retired man in his 70s diagnosed with AD, displayed OCD symptoms following donepezil initiation, exacerbating post-stroke-specifically, a cerebral infarction in the right posterior limb of the internal capsule. Remarkably, the symptoms abated upon discontinuation of donepezil. CONCLUSION: Caution should be exercised when using donepe

Examining the momentary relationships between body checking and eating disorder symptoms in women with anorexia nervosa.

Body checking is common among individuals with anorexia nervosa (AN) and increases risk for dietary restriction. However, no study has examined whether body checking increases the immediate risk for engaging in other harmful weight loss behaviors, or whether this relationship is moderated by person-level traits. The current study utilized ecological momentary assessment (EMA) to examine whether (a) body checking predicted rapid use of weight loss behaviors, and (b) whether eating-related obsessionality/compulsivity moderated this relationship. Women with full or subthreshold anorexia nervosa (

The Investigation of the Effects of Group Exercise Treatment on Psychological Status, Muscle Strength, Respiratory Performance, and Balance Parameters in Hospitalized Patients in Psychiatry Departments: A Randomized Clinical Study.

BACKGROUND: Patients hospitalized in psychiatry departments often experience both psychological distress and physical deconditioning due to reduced activity levels and prolonged stays. Exercise interventions may support recovery by addressing both physical and mental health domains. AIM: This study aimed to investigate the effects of group exercise on psychological symptoms, muscle strength, respiratory capacity, and balance in patients hospitalized in the psychiatry department. METHODS: Forty-six patients with psychiatric disorders were randomly assigned to an exercise group (n = 15) or con

Sex differences in addictive disorders.

Gender-dependent differences in the rate of initiation and frequency of misuse of addicting drugs have been widely described. Yet, men and women also differ in their propensity to become addicted to other rewarding stimuli (e.g., sex, food) or activities (e.g., gambling, exercising). The goal of the present review is to summarize current evidence for gender differences not only in drug addiction, but also in other forms of addictive behaviours. Thus, we first reviewed studies showing gender-dependent differences in drug addiction, food addiction, compulsive sexual activity, pathological gambli

The use of Image and Performance-Enhancing Drugs (IPED) in behavioural addictions.

BACKGROUND AND AIMS: Digitalisation and rapid technological advancements have intensified socio-cultural pressures surrounding body ideals and performance, contributing to the increased use of Image and Performance Enhancing Drugs (IPED). Body dissatisfaction, muscularity-oriented concerns, and distorted self-perception represent key drivers of this phenomenon and are further amplified by digital media, which increasingly promote IPED use. Although IPED use has been consistently associated with body image disturbances and eating-related psychopathology, its relationship with behavioural addict

Driven exercise in the absence of binge eating: Implications for purging disorder.

OBJECTIVE: Purging disorder (PD) is characterized by recurrent purging without objectively large binge-eating episodes. PD has received relatively little attention, and questions remain about the clinical significance of "purging" by exercise that is driven or compulsive (i.e., as extreme compensatory or weight-control behavior). The little available research suggests that individuals who use exercise as a compensatory behavior might have less eating-disorder psychopathology than those who purge by vomiting or laxatives, but those studies have had smaller sample sizes, defined PD using low-fre

Vector symbolic algebras for the Abstraction and Reasoning Corpus.

BACKGROUND: Individuals with anorexia nervosa frequently feel ambivalent about treatment and weight restoration, and drop out and relapse rates in treatment are high. Increased insight into the function of the eating disorder is considered essential for achieving long-lasting, meaningful change. However, research investigating the functions of anorexia nervosa tends to focus on the role of the disease per se. Distinctions are rarely made across features. In particular, the subjective experience, understanding and sense making of the engagement in compulsive exercise in individuals with anorexi

Scoping review of therapeutic approaches among individuals with secondary exercise addiction.

Secondary exercise addiction shows high comorbidity with eating and body image disorders. Despite its substantial impact on physical and mental health and daily functioning, evidence on effective therapeutic interventions remains limited. The aim of this scoping review was to identify and describe therapeutic interventions applied to adult individuals with secondary exercise addiction. This review followed the PRISMA Sc-R guidelines and covered the years 2002-2024. Ultimately, five studies were included (four randomized controlled trials and one quasi-experimental study). Three studies applied

Virtual Reality-Based Running Exposure to Target the Acute Urge to Be Physically Active in Anorexia Nervosa: A Case Series.

INTRODUCTION: Anorexia nervosa (AN) is a severe and complex eating disorder (ED), whose incidence has increased following the COVID-19 pandemic, straining specialized healthcare systems worldwide. Although patients often resort to compulsive exercise as a means of weight control, prolonged fasting intensifies the urge to move, leading to increased motor restlessness. Despite progress in understanding AN, interventions often overlook the crucial role that the urge to be physically active plays in reinforcing the ED itself. This case series tested the feasibility of a Virtual Reality-based Runni

What influences food choices in anorexia nervosa? Disentangling cognitive and emotional components of decision-making by translational research.

Anorexia nervosa is a serious mental illness characterized by voluntary restriction of food intake and avoidance of high-calorie food. Anxiety, highly comorbid with anorexia nervosa, appears to be a significant, yet underexplored, factor affecting core behavioural symptoms such as food restriction and compulsive physical exercise. The aims of this review are to disentangle the influence of anxiety in food decision-making in anorexia nervosa and to offer a comprehensive model connecting the mechanisms involved. The shift from food approach to avoidance seems to be a conditioned response, underp

Exploring the link between hedonic overeating and prefrontal cortex dysfunction in the Ts65Dn trisomic mouse model.

Individuals with Down syndrome (DS) have a higher prevalence of obesity compared to the general population. Conventionally, this has been attributed to endocrine issues and lack of exercise. However, deficits in neural reward responses and dopaminergic disturbances in DS may be contributing factors. To investigate this, we focused on a mouse model (Ts65Dn) bearing some triplicated genes homologous to trisomy 21. Through detailed meal pattern analysis in male Ts65Dn mice, we observed an increased preference for energy-dense food, pointing towards a potential "hedonic" overeating behavior. Moreo

Demographics and physical and mental health of clients at a sports center with and without exercise addiction.

BACKGROUND: While exercise is widely recognized for its health benefits, excessive engagement can lead to exercise addiction (EA), a behavioral condition characterized by compulsive and harmful physical activity. Limited research exists on the demographic, physical, and mental health profiles of individuals with EA. This descriptive cross-sectional study examines demographic factors and related health outcomes among clients at a university sports center with and without EA. METHODS: A total of 386 participants were randomly recruited through convenience sampling at the Chulalongkorn Universit

Exercise addiction and disordered eating in young Lebanese regular sport practitioners: the indirect role of body appreciation and body dysmorphic concerns.

BACKGROUND: Disordered eating, encompassing behaviors like restrictive dieting, binge eating, and purging, is a prevalent issue with significant psychological and physical consequences. Among athletes, it is often exacerbated by sociocultural pressures and performance demands, making it a critical area of study. Exercise addiction is characterized by compulsive exercise behaviors and is frequently associated with disordered eating; however, the underlying mechanisms of this relationship remain unclear. Limited research has explored the factors that may contribute to the risk of both exercise a

The contribution of anxiety and food restriction on physical activity levels in acute anorexia nervosa.

OBJECTIVE: Excessive exercise is present in 40%-80% of anorexia nervosa (AN) patients. Hyperactivity often plays a role in developing and maintaining AN and represents an obstacle to weight gain in refeeding. Interconnections among caloric restriction, psychopathology, and physical activity in humans with AN are poorly investigated. METHODS: Physical activity and food restriction during the last 3 months and status of body image/slimness ideal were assessed by the Structured Interview of Anorexia and Bulimia Nervosa (SIAB) in 30 adolescent patients with acute AN at admission to inpatient trea
