Clinical and laboratory assessment of the subjective experience of drug craving.

Measures of subjective drug craving - often defined as the experience of an intense or compelling urge or desire - may be used to predict relapse, evaluate psychological and pharmacological treatments, and test theories of addiction and craving. This review summarizes both direct self-report questionnaires and indirect behavioral, physiological and reaction time measures designed to assess craving for alcohol, amphetamines, cocaine, heroin, marijuana, and tobacco. Multi-item questionnaires have typically been based on one of four underlying conceptualizations of addiction or craving (obsessive

Reduction of food cravings through concurrent visuospatial processing.

OBJECTIVE: Two experiments adopted a working memory approach to evaluate the effectiveness of visuospatial tasks as a technique for reducing food cravings. METHOD: Dieting and non-dieting women were asked to form images of both food-related and nonfood items, induced by either pictures (Experiment 1) or verbal cues (Experiment 2). They were required to concurrently perform one of three tasks that load on the visuospatial sketch pad of working memory: saccadic eye movements, dynamic visual noise, or spatial tapping. RESULTS: In support of the working memory model of limited visuospatial capac

Effects of nicotine deprivation on craving response covariation in smokers.

Most models of craving propose that when cravings are strong, diverse responses--thought to index an underlying craving state--covary. Previous studies provided weak support for this hypothesis. The authors tested whether nicotine deprivation affects degree of covariation across multiple measures related to craving. Heavy and light smokers (N = 127) were exposed to smoking cues while either nicotine deprived or nondeprived. Measures included urge ratings, affective valence, a behavioral choice task assessing perceived reinforcement value of smoking, and smoking-related judgment tasks. Results

Personality traits and craving in patients undergoing alcohol withdrawal treatment.

BACKGROUND: Alcohol use disorder (AUD) is a public health problem in France. Links between personality dimensions and AUD have already been established, but few studies focused on individuals seeking alcohol withdrawal. The main objective of the study was to describe the personality profile of patients seeking alcohol detoxification in a complex residential care unit. The secondary objective was to investigate the relationship between personality dimensions and alcohol craving. METHOD: The observational longitudinal exploratory study included 88 patients with AUD who were hospitalized during

Neural mechanisms underlying incubation of methamphetamine craving: A mini-review.

Cue-induced drug craving and seeking progressively increases during abstinence. This "incubation of drug craving" phenomenon has been observed in both laboratory animals and humans. Preclinical studies identified several neural mechanisms underlying incubation of drug craving after forced abstinence, primarily focusing on cocaine. Recently, studies started focusing on another powerful psychostimulant, methamphetamine (Meth), and developed new incubation procedures (choice-induced and punishment-induced abstinence). Here, we review mechanistic studies at the circuit, synaptic and molecular leve

Drug Craving Terminology among Opiate Dependents; A Mixed Method Study.

OBJECTIVE: Drug craving is defined as an urge to continue substance abuse. Drug dependents use different terms to express their subjective feeling of craving. This study was an attempt to generate an understanding of craving terminology among different groups of Persian speaking Iranian opiate dependents. METHOD: Terms used for the meaning of drug craving were listed by 36 ex-opiate dependents in focus group discussion meetings in the first phase of the study. These terms were composed from Craving Terms Questionnaire. In the second phase, 120 subjects in 3 groups of opiate dependents and a g

Hyperventilation, anxiety, craving for alcohol: a subacute alcohol withdrawal syndrome.

Hyperventilation leading to respiratory alkalosis is part of the acute alcohol withdrawal syndrome On the basis of clinical observations and a literature review on withdrawal symptoms the following was hypothesized: (a) hyperventilation is also part of a subacute alcohol withdrawal syndrome and (b) hyperventilation appears together with anxiety and craving for alcohol. These hypotheses were tested in a sample of 37 male alcoholic inpatients, abstinent for periods from several weeks to 9 months. Subjects were administered a questionnaire dealing with drinking history and craving for alcohol dur

The impact of virtual alcohol-related stimuli on craving in university students: an exploratory virtual reality intervention study. VR alcohol stimuli and university student craving.

AIMS: This exploratory study aimed to examine how virtual reality (VR) environments as an experimental tool can evoke and modulate alcohol craving among university students. Specifically, we investigated how exposure to a party scene with alcohol-related stimuli versus a relaxing environment influenced craving levels. METHODS: Thirty-seven university students were sequentially immersed in two VR scenarios-a party scene with alcohol cues and a nature-based relaxation scene. Craving was assessed at three time points using a VAS, and post-exposure using the Alcohol Urge Questionnaire (AUQ). A li

rTMS Suppresses Tobacco Craving via Enhanced Prefronto-Striato-Thalamic Connectivity.

Studies have used repetitive transcranial magnetic stimulation (rTMS) to stimulate the left dorsolateral prefrontal cortex (DLPFC) in patients with tobacco use disorder (TUD); consequently, decreased craving for smoking was observed. However, the neural mechanism underlying this process remains unclear. Functional MRI data were collected from 59 valid TUD participants (31 rTMS and 28 shams) when they performed a Go/No-go task before and after a continuous 5-day treatment (rTMS on the left DLPFC). Three approaches of data analyses were performed: event-related data analyses to identify the brai

Network state dynamics underpin basal craving in a transdiagnostic population.

Emerging fMRI methods quantifying brain dynamics present an opportunity to capture how fluctuations in brain responses give rise to individual variations in affective and motivation states. Although the experience and regulation of affective states affect psychopathology, their underlying time-varying brain responses remain unclear. Here, we present a novel framework to identify network states matched to an affective experience and examine how the dynamic engagement of these network states contributes to this experience. We apply this framework to investigate network state dynamics underlying

Moroccan version of the short Tobacco Craving Questionnaire (MTCQ-12).

INTRODUCTION: Craving is a key clinical dimension in the assessment and management of substance use disorders. It is also a recognized predictor of relapse in addictology. To date, no scale for measuring tobacco craving is available to Moroccan clinicians. This work proposes a cross-cultural validation of the Tobacco Craving Questionnaire (TCQ) in its short version. METHODS: This work consists of the creation of a translated version according to a rigorous methodology and the evaluation of its psychometric properties in a statistical study. RESULTS: The validation study was done on a sample

Stressor-elicited smoking and craving during a smoking cessation attempt.

Stressors can undermine smokers' attempts to quit smoking. Although contemporary theories and animal models support this idea, human research has struggled to demonstrate definitively the relationship between stressors and smoking. Researchers have employed more ecologically valid methods like ecological momentary assessment to address this question, but studies focusing explicitly on stressors remain sparse and findings inconsistent. The purpose of this study was to examine the effect of stressful event intensity on smoking and craving among cigarette smokers during a quit attempt. We conduct

[Neural mechanisms underlying stress-induced enhancement of cocaine craving behaviors].

Stress potentiates craving for addictive drugs including cocaine. To elucidate neural mechanisms underlying this effect of stress, we developed an experimental paradigm combining cocaine-induced conditioned place preference (CPP) with a restraint stress. Acute restraint stress exposure immediately before posttest significantly increased cocaine CPP scores. It has been suggested that the extracellular noradrenaline (NA) level is increased by stress in the laterodorsal tegmental nucleus (LDT), which sends cholinergic projections to dopamine (DA) neurons in the ventral tegmental area (VTA), and m

Craving in Opioid Use Disorder: From Neurobiology to Clinical Practice.

Opioid use disorder (OUD) is a major public health issue that has reached epidemic levels in some parts of the world. It is a chronic and complex neurobiological disease associated with frequent relapse to drug taking. Craving, defined as an overwhelmingly strong desire or need to use a drug, is a central component of OUD and other substance use disorders. In this review, we describe the neurobiological and neuroendocrine pathways that underpin craving in OUD and also focus on the importance of assessing and treating craving in clinical practice. Craving is strongly associated with patients re

Cue-induced craving in dependence upon prescription opioids and heroin.

BACKGROUND AND OBJECTIVES: Cues associated with heroin use (eg, needles, powder) elicit robust craving responses in individuals dependent upon heroin. Elevated cue-induced craving may be a risk factor for relapse and can persist after periods of drug abstinence. Despite the growing prevalence of opioid dependence involving prescription opioids, published studies have yet to examine whether cue-induced craving is also present in prescription opioid dependence. METHODS: A sample of 50 adults diagnosed with opioid dependence (20 prescription opioid users, 25 heroin users, and 5 mixed opioid user

Association between cholesterol plasma levels and craving among heroin users.

OBJECTIVE: Lipids may play some roles in the central nervous system functions that are associated with drug addiction. To date, cholesterol is known to influence relapse of cocaine use. However, the relationship between cholesterol and heroin craving is unclear. This study examined the concurrent association between cholesterol and craving. METHOD: The serum lipid levels of 70 heroin users who were undergoing or had undergone a methadone maintenance therapy were measured. Their craving and demographic data were assessed. RESULTS: Total cholesterol and low-density lipoprotein cholesterol are
