[Syringocele or cystic diagnosis of Cowper's gland ducts].

Cystic dilatation of bulbo-urethral gland ducts (Cowper's syringocele) is an uncommon abnormality, frequently asymptomatic, showing various radiographic patterns. The lesion is more commonly seen in children, but it may be found in adults too, and may cause dysuria, pollakiuria, stranguria and posturinary dribbling. Fourteen patients with syringocele were observed: 6 of them were asymptomatic, while in 8 the most common symptoms were posturinary dribbling and stranguria. X-ray examination showed 7 cases of perforated syringocele, 4 cases of ruptured syringocele, 2 cases of simple syringocele a

A test for patency of the cystic duct in acute cholecystitis.

A procedure was devised to quickly and reliably determine the patency of the cystic duct in patients suspected of having acute cholecystitis. First the gallbladder was stimulated to empty by a cholecystokinin injection. Thirty minutes later a radiolabeled biliary marker, either 150 muCi 131-I rose bengal or 2 mCi 99-mTc dihydrothioctic acid, was injected, and the accumulation of radioactivity in the liver and gallbladder regions was monitored by external gamma emission imaging and recording devices. The images of diagnostic importance were obtained between 60 and 90 minutes after injection of

Mirizzi syndrome in a patient with anomalous connection of the cystic duct into the right hepatic duct.

In 1948, Mirizzi described a syndrome characterized as the obstruction of the common hepatic duct by a stone located in the gallbladder's neck or in the cystic duct. We present a rare case of Mirizzi syndrome resulting from a fistula involving the cystic duct, the neck of the gallbladder, and the right hepatic duct. This finding was possible because the patient had a rare biliary anatomic variation: the insertion of the cystic duct straight in the right hepatic duct. The diagnosis was suggested by abdominal ultrasonography and confirmed by endoscopic retrograde cholangiopancreatography. The su

Clinical significance of the confluence of the cystic duct in patients with anomalous arrangement of the pancreaticobiliary duct.

BACKGROUND/PURPOSE: In anomalous arrangement of the pancreaticobiliary duct (AAPBD), there is anatomical diversity of the cystic duct. In this study, we evaluated the influence of the level of insertion of the cystic duct into the extrahepatic bile duct on the pathophysiology of AAPBD. METHODS: Thirty-two children with AAPBD were examined using cholangiopancreatography. If the cystic duct entered the hepatic duct at the lower middle point of the extrahepatic bile duct, it was defined as low confluence; otherwise, it was considered as high confluence. Clinical details and radiological variable

Right hepatic duct opening into the cystic duct: the role of pre- and intraoperative cholangiography.

Although an aberrant hepatic duct entering the cystic duct is not especially rare, the main right hepatic duct entering the cystic duct is extremely rare. A 69-year-old woman developed severe intermittent right upper quadrant pain and high fever. A diagnosis of acute calculus cholecystitis was made by radiographic examinations. Magnetic resonance cholangiopancreatography demonstrated dilatation of the right hepatic duct, but could not identify the junction of the right hepatic duct and the cystic duct. Endoscopic retrograde cholangiopancreatography established that the right hepatic duct joine

Recognition and Surgical Management of Aberrant Right Hepatic Duct Originating From the Cystic Duct.

An aberrant right hepatic duct is a rare congenital anomaly of the biliary system. Failure to recognize these anomalies can result in serious complications. In this case, we present a patient who underwent laparoscopic cholecystectomy for chronic cholecystitis. Post-operatively she developed a bile leak for which she underwent reoperation. On re-exploration, she was discovered to have a cystic stump leak and a rare Hisatsugu type V anatomic anomaly of the right hepatic duct originating from the cystic duct. She was subsequently managed with oversewing of the cystic duct stump and drainage. Thi

[Congenital cystic dilatation of the cystic duct associated with an anomalous pancreaticobiliary ductal junction].

We report a case of congenital cystic dilatation of the cystic duct detected in a 40-year-old woman. She had intermittent pain in the right upper quadrant of the abdomen independent of the intake of meals for the last two years. In the ultrasound we found a cystic formation in the vicinity of the enlarged gallbladder, the common hepatic duct/common bile duct and the portal vein. The endoscopic retrograde cholangiography confirmed a cystic duct malformation, which was associated with an anomalous pancreaticobiliary ductal junction (APBDJ). A cholecystectomy with excision of the whole cystic duc

Magnetic Resonance (MR) Cholangiopancreatography Demonstration of the Cystic Duct Entering the Right Hepatic Duct.

BACKGROUND MR cholangiopancreatography is widely performed before laparoscopic cholecystectomy to rule out choledocholithiasis and to avoid iatrogenic injuries that may be related to the high frequency of anatomical variations of the biliary tree. Although most of these variants have already been demonstrated surgically and by endoscopic retrograde cholangiopancreatography and CT cholangiography, there are no references in which MR cholangiopancreatography has shown a cystic duct draining into the right hepatic biliary duct. CASE REPORT A 51-year-old woman with a history of recurrent abdominal

Perivascular epithelioid cell tumor (PEComa) of the cystic duct.

Perivascular epithelioid cell tumors, also known as PEComas, are rare mesenchymal tumors composed mainly of epithelioid cells found in perivascular tissue. PEComas occur most frequently in the kidney, uterus, the gastrointestinal tract, liver, and retroperitoneum; those originating in the biliary tree are extremely rare. We report a case of benign PEComa of the cystic duct with positive TFE3 staining on immunohistochemistry.A 66-year-old woman was referred for a 20 mm mass adjacent to the common bile duct discovered incidentally on abdominal ultrasound. Laboratory data including tumor markers

Remnant Cystic Duct Disease After Cholecystectomy: A Case Series.

BACKGROUND: Remnant cystic duct (RCD) may be responsible for postcholecystectomy syndrome. We present our experience with the management of remnant cystic duct disease (RCDD) after cholecystectomy. METHODS: Over a period of 5 years, 10 patients underwent reoperation for RCDD in our hospital. Cystic duct was identified by intraoperative exploration. RESULTS: There were 4 men and 6 women ranging in age from 37 to 76 years (median, 60.40 y). All 10 had biliary pain, 5 had jaundice, and 2 had pancreatitis. The time from initial cholecystectomy to reoperation ranged from 4 to 28 years (median, 12

Compression of cystic duct caused by focal nodular hyperplasia.

A 29-year-old female presenting with symptoms of biliary colic was found to have a liver mass compressing the cystic duct. Due to the anatomical placement of the growth, the compressed duct produced symptoms mimicking acute cholecystitis. The mass was diagnosed as focal nodular hyperplasia (FNH) upon biopsy. FNH is commonly found incidentally with nonhepatic clinical presentation or during an unrelated surgical procedure. The scope of this paper is to bring awareness to uncommon causes of biliary colic. To our knowledge, there has been one other paper published with FNH being the primary cause

Cystic duct closure during partial cholecystectomy: ten years' experience.

BACKGROUNDS/AIMS: When surgeons face difficulties in dissecting the Calot triangle during cholecystectomy due to severe inflammation or fibrosis, the proximal portion of the gallbladder is left in place to avoid injury to the bile duct; this procedure is called partial cholecystectomy (PC), and it is associated with a much higher complication rate after the operation. METHODS: We surveyed the clinical outcomes of 25 cases of PC by laparotomy during ten years from January 1998 to December 2007, for a total of 95 months of the mean follow-up period. Patients were separated in two groups for com

Cystic duct remnant mucocele in a liver transplant recipient.

Cystic duct remnant mucocele is an extremely rare complication of liver transplantation in children. Surgical correction is usually required for cystic duct remnant mucocele when it causes biliary obstruction. We describe a 14-month-old liver transplant recipient who presented with biliary obstruction 1 month after orthotopic liver transplantation with an end-to-end choledochocholedocal biliary anastomosis for hepatoblastoma. US, CT and cholangiography findings were consistent with mucocele of the allograft cystic duct remnant. Surgery was not needed in our patient because the mucocele and bil

Anatomical variations of the cystic duct: two case reports.

Anatomical variations of the cystic duct often occur and may be encountered during cholecystectomy. Knowledge of the variable anatomy of the cystic duct and cysticohepatic junction is important to avoid significant ductal injury in biliary surgery. Here, we present two unusual cases with an anomalous cystic duct, namely, low lateral insertion and narrow-winding of the cystic duct. The first case was a 64-year-old man with cholelithiasis and chronic cholecystitis. During surgery, the entrance of the cystic duct was misidentified as being short and leading into the right hepatic duct. Further ex

[Congenital cystic bile duct dilatation: an incidental intraoperative finding].

In adulthood, choledochal cysts are often surprisingly discovered during cholecystectomy. We report here on a case of an unsuspected congenital type-IA biliary cyst, according to Todani's classification encountered during laparoscopic cholecystectomy in a 30-year-old woman complaining of acute abdominal pain with an unremarkable preoperative workup. The well-known risk of developing cystic cancer, mainly in the adult, means that an excisional operation is indicated to prevent such complications. Cyst excision and Roux-en-Y hepaticojejunostomy is the definitive treatment of choice, performed at
