Resolving conflict realistically in today's health care environment.

Conflict is a natural part of human interaction, and when properly addressed, results in improved interpersonal relationships and positive organizational culture. Unchecked conflict may escalate to verbal and physical violence. Conflict that is unresolved creates barriers for people, teams, organizational growth, and productivity, leading to cultural disintegration within the establishment. By relying on interdependence and professional collaboration, all parties involved grow and, in turn, benefit the organization and population served. When used in a constructive manner, conflict resolution

Empowering Collaboration With Patients: Healthcare Staff's Perspectives on Conditions for Shared Decision-Making in Swedish Psychiatric Care.

RATIONALE: Shared decision-making (SDM) is advocated in psychiatric care to facilitate patients' engagement in treatment discussions about their own care. However, applying SDM in practice involves more than simply referring to an advocated method. It is therefore interesting to study healthcare professionals' perceptions and experiences of the conditions necessary for patients to truly participate in SDM. AIM: To explore healthcare staff's experiences of patient involvement in psychiatric care, paying particular attention to perceived prerequisites, barriers and strategies relevant to shared

[Planning for the Possible in Healthcare: From PEIMAF to the Management of the Unknown.].

This paper examines the evolution of healthcare planning in light of the increasing complexity of contemporary emergencies, which are characterized by high levels of uncertainty and the frequent absence of established interpretative models. Starting from the historical role of Hospital Emergency Plans for Mass Casualty Incidents (PEIMAF), the article highlights the limitations of approaches based solely on known scenarios and proposes a conceptual shift toward managing the unknown. It outlines the need to broaden the operational meaning of PEIMAF, extending it to all types of patients and tran

Strengthening advocacy efforts with empirical evidence: A case example of the conduct, uptake and utilisation of research in drug policy decision-making in Vietnam.

During the last decade, international aid agencies and advocates have been working with Southeast Asian governments to move away from punitive responses towards people who use drugs to more public health, humane approaches. The lack of local scientific evidence about the effectiveness of different treatment approaches has made this advocacy work more challenging. This paper reflects on a generation of treatment research evidence and how it can assist advocacy efforts. The case example is the cost-effectiveness research, comparing centre-based compulsory treatment with community-based voluntary

Development of a risk-priority score for category A bioterrorism agents as an aid for public health policy.

In developing public health policy and planning for a bioterrorist attack or vaccination of military personnel, the most common method for assigning priority is using the probability of attack with a particular agent as the single criterion. Using this approach, smallpox is often dismissed as an unlikely threat. We aimed to develop an evidence-based, systematic, multifactorial method for prioritizing the level of risk of each category A bioterrorism agent. Using 10 criterion, anthrax scored the highest, followed by smallpox. Tularemia was the lowest scoring agent. We suggest that such a system

The influence of management and environment on local health department organizational structure and adaptation: a longitudinal network analysis.

OBJECTIVE: The nation's 2862 local health departments (LHDs) are the primary means for assuring public health services for all populations. The objective of this study is to assess the effect of organizational network analysis on management decisions in LHDs and to demonstrate the technique's ability to detect organizational adaptation over time. DESIGN AND SETTING: We conducted a longitudinal network analysis in a full-service LHD with 113 employees serving about 187,000 persons. Network survey data were collected from employees at 3 times: months 0, 8, and 34. At time 1 the initial analysis

Spine Surgery in Italy in the COVID-19 Era: Proposal for Assessing and Responding to the Regional State of Emergency.

In December 2019, coronavirus disease 2019 (COVID-19) was discovered in Wuhan, Hubei province, from where it spread rapidly worldwide. COVID-19 characteristics (increased infectivity, rapid spread, and general population susceptibility) pose a great challenge to hospitals. Infectious disease, pulmonology, and intensive care units have been strengthened and expanded. All other specialties have been compelled to suspend or reduce clinical and elective surgical activities. The profound effects on spine surgery call for systematic approaches to optimizing the diagnosis and treatment of spinal dise

Damage compounded: disparities, distrust, and disparate impact in end-of-life conflict resolution policies.

For a little more than a decade, professional organizations and healthcare institutions have attempted to develop guidelines and policies to deal with seemingly intractable conflicts that arise between clinicians and patients (or their proxies) over appropriate use of aggressive life-sustaining therapies in the face of low expectations of medical benefit. This article suggests that, although such efforts at conflict resolution are commendable on many levels, inadequate attention has been given to their potential negative effects upon particular groups of patients/proxies. Based on the well-doc

Implications of conflicting definitions of probability to health risk communication: a case study of familial cancer and genetic counselling.

The question of what probability actually is has long been debated in philosophy and statistics. Although the concept of probability is fundamental to many applications in the health sciences, these debates are generally not well known to health professionals. This paper begins with an outline of some of the different interpretations of probability. Examples are provided of how each interpretation manifests in clinical practice. The discipline of genetic counselling (familial cancer) is used to ground the discussion. In the second part of the paper, some of the implications that different inte

Patient‑Reported Satisfaction in Relapsing-Remitting Multiple Sclerosis: Advantages of At‑Home Self‑Administered Subcutaneous Ofatumumab Versus Other High‑Efficacy Therapies.

BACKGROUND: Multiple sclerosis is an immune-mediated disorder characterized by demyelination and neurodegeneration. Disease-modifying therapies are available with a range of mechanisms of action, efficacies, safety profiles, places and routes of administration, and degrees of patient autonomy; however, comprehensive evaluations of satisfaction with high-efficacy therapies in patients with multiple sclerosis are lacking. OBJECTIVE: We aimed to evaluate satisfaction of patients treated with high-efficacy therapies for relapsing-remitting multiple sclerosis and identify factors influencing prefe

A theoretical analysis of prophylactic common iliac arterial occlusion for potential massive bleeding during cesarean delivery: Decision-making considerations - A 2-year retrospective study.

OBJECTIVE: According to the American College of Obstetricians and Gynecologists, there has been a rapid increase in the total cesarean birth rates. The rate of placenta accreta is increasing, and previous cesarean delivery is the most common risk factor. Labor is a major challenge in cases with an abnormally invasive placenta, considering the risk of massive blood loss during cesarean delivery and patient wishes for uterine preservation. MATERIALS AND METHODS: We retrospectively obtained clinical data and surgical outcomes of high-risk cases of placenta previa totalis and placenta accreta adm

Role of the clinical microbiology laboratory in infection control--a Danish perspective.

Clinical microbiology laboratories in Denmark are located in hospitals and staffed by clinical microbiologists who are clinically trained medical doctors. Each county has its own clinical microbiology unit, serving a population of 0.3-0.6 million. The responsibilities of clinical microbiology unit cover many different aspects of infection control. They include detection of outbreaks of hospital-acquired infections, screening for multi-resistant organisms, advice to clinicians about disinfection, sterilization and isolation procedures, and the rational use of antibiotics. Clinical microbiologis

How effective incident management retains market share.

This paper discusses the need for business continuity practitioners to make incident management a focal element of their programme. Particularly during the first few minutes and hours of a business disruption, an established incident management methodology is not only key to achieving a successful, coordinated recovery, but it can play an even more important role in maintaining customer confidence following a disruption or crisis.

Why good leaders make bad decisions.

Decision making lies at the heart of our personal and professional lives. Yet the daunting reality is that enormously important decisions made by intelligent, responsible people with the best information and intentions are nevertheless hopelessly flawed at times. In part, that's due to the way our brains work. Modern neuroscience teaches us that two hard-wired processes in the brain--pattern recognition and emotional tagging--are critical to decision making. Both are normally reliable; indeed, they provide us with an evolutionary advantage. But in certain circumstances, either one can trip us

The value captor's process: getting the most out of your new business ventures.

The high failure rate among new business ventures is usually chalked up to the fundamental uncertainty of the process. In actuality, say McGrath and Keil, flawed ways of assessing and managing ventures may account for the disappointing amount of value they generate. Instead of taking the go/no-go approach, whereby a project either advances toward launch or is killed, decision makers should consider a range of alternatives: recycling the venture by aiming it at a new target market; spinning it off to other owners or a joint venture; spinning it in to an established business unit; or salvaging u

Do you know who will be vaccinated at your ED?

The administration of the smallpox vaccine is imminent, so emergency department managers are making difficult decisions about which individuals should be vaccinated. If all attending physicians are vaccinated, these individuals can care for patients with suspected smallpox. Consider offering vaccine first to staff who have been immunized previously. Survey staff to find out which individuals have contraindications for the vaccine.
