The acceptability, feasibility and impact of a lay health counsellor delivered health promoting schools programme in India: a case study evaluation.

BACKGROUND: Studies in resource-limited settings have shown that there are constraints to the use of teachers, peers or health professionals to deliver school health promotion interventions. School health programmes delivered by trained lay health counsellors could offer a cost-effective alternative. This paper presents a case study of a multi-component school health promotion intervention in India that was delivered by lay school health counsellors, who possessed neither formal educational nor health provider qualifications. METHODS: The intervention was based on the WHO's Health Promoting S

Rediscovering medical humanism in the age of artificial intelligence: lessons from historical medical traditions.

Artificial intelligence (AI) is increasingly transforming diagnosis, treatment selection, prognostic assessment, and administrative work in medicine. Although these technologies may improve efficiency and clinical performance, their integration also raises questions about medical humanism, professional responsibility, and the physician-patient relationship. This narrative historical review examines the contributions of Hippocrates, Asclepiades, Galen, Avicenna, and Maimonides to the relationship between medical knowledge, clinical judgment, and moral responsibility, and considers how these his

Decision fatigue and coping responses among primary caregivers of older adults with disabilities: A qualitative study.

BACKGROUND: Primary caregivers of older adults with disabilities often coordinate repeated health-related and daily care decisions in long-term home-based care. Decision fatigue may represent a less visible dimension of caregiver burden, yet it remains poorly understood in this setting. Burden of Treatment Theory may provide a useful lens for understanding decision-making as part of caregivers' ongoing care-related work. OBJECTIVES: To explore the experiences, manifestations, and coping responses related to decision fatigue among primary caregivers of older adults with disabilities in China.

Medical associations' guidance on caring for patients experiencing incarceration in the United States.

The United States has one of the highest incarceration rates in the world. Carceral status can complicate clinical encounters in community and academic settings for an already medically vulnerable population. While it is likely physicians will encounter patients experiencing incarceration in their practice, there are few educational opportunities dedicated to ensuring delivery of healthcare that protects patient dignity, autonomy, and privacy. Professional medical associations can play a role in filling this physician knowledge gap. The goal of this analysis is to catalogue and analyze the cur

Management of Medication-related Osteonecrosis of the Jaw (MRONJ) risk in patients due to commence anti-resorptive/anti-angiogenic drugs - how should pre-drug-treatment dental preventive care be organised?

BACKGROUND: Medication-related osteonecrosis of the jaw (MRONJ) can be difficult to treat and causes significant morbidity, but is largely preventable. Published guidelines strongly recommend dental assessment and necessary remedial treatment before such drugs are commenced. Specific guidance on who should provide or arrange this care is lacking, and it may often be delegated to the patient arranging it with their own dentist. However, numerous factors can make this difficult. AIMS: To review published strategies for organising timely and effective dental preventive care in patients due to be

Co-Producing Peritoneal Dialysis Nursing Sensitive Indicators for Quality Care: A Multinational Consensus Building Design.

BACKGROUND: Nursing sensitive indicators are a way of measuring aspects of patient care that are most affected by the actions of the nurse. Despite the existence of nursing sensitive indicators, these are largely not suitable to measure peritoneal dialysis nursing practice. OBJECTIVE: This project aimed to co-develop a set of peritoneal dialysis nursing-sensitive indicators. DESIGN: Informed by the Donabedian quality framework (structure, process, outcome), a multinational co-production consensus design was used. PARTICIPANTS AND MEASUREMENTS: First, an expert panel of seven professionals p

Time on task limits psychotherapy's role in reducing the societal burden of aggression.

INTRODUCTION: Aggression and violence, people's inhumanity to one another, are perhaps society's foremost problems. One approach to this problem is the provision of traditional clinical services through psychotherapy. Anger control is a learnable skill, but such learning requires "time on task." Our goal was to shed light on the potential impact of psychotherapy as a public health remedy, by studying how much psychotherapeutic intervention is being delivered to patients with impulsive aggression classified as Intermittent Explosive Disorder (IED). METHOD: Using de-identified electronic health

Evaluating the Feasibility and Validity of Using Trained Allied Health Assistants to Assist in Mealtime Monitoring of Dysphagic Patients.

Growing patient numbers, within a context of finite resources, has placed increased demands on dysphagia services in acute settings. Delegating some aspects of dysphagia management to other trained professional groups, such as allied health assistants (AHA), may help speech-language pathology (SLP) service efficiencies. The primary aim of this study was to explore the feasibility and initial validity of using trained AHAs to complete structured mealtime observations of patients. The secondary aims were to explore costs and user perceptions. The study used a mixed methods design. All AHAs who p

Occupational health in surgery: risks extend beyond the operating room.

Surgeons routinely work with potentially infectious materials. The risk of acquiring a disease from one percutaneous exposure is 0.3-0.4% for human immunodeficiency virus (HIV) 6-30% for hepatitis B virus (HBV) and 2.7-10% for hepatitis C virus (HCV). Rates of blood contacts vary but may reach up to 11.9 per 100 h in the operating room. Residents are at highest risk, and obstetrics and gynaecology surgeons suffered the highest rate of exposures (10%) as a group. Contributing risk factors include trauma or emergency orthopaedic procedures, high patient blood loss, long procedures and holding ti

Effects of task-shifting from primary care physicians to nurses: an overview of systematic reviews.

BACKGROUND: Task-shifting from primary care physicians (PCPs) to nurses is a means of overcoming PCP shortages and meeting the needs of patients receiving primary care. The aim of this overview of systematic reviews is to assess the effects of delegation or substitution of PCPs' activities by nurses on patient relevant, clinical, professional and health services-related outcomes. METHODS: We conducted a systematic literature search for secondary literature in Medline, Embase, Pubmed, the Cochrane Library, and the Cumulative Index of Nursing and Allied Health Literature (CINAHL). Having and eating the cake? The centralisation paradox in decentralised healthcare governance: a scoping review.

BACKGROUND: Decentralisation in healthcare governance is widely promoted to enhance managerial autonomy, responsiveness, and performance. However, empirical evidence suggests that managers operating in decentralised systems continue to rely on centrally coordinated organisational structures. This interdependence remains insufficiently conceptualised in existing research. METHODS: A scoping review was conducted to synthesise empirical studies examining how central coordination in organisational systems shapes managers' effective decision space in decentralised healthcare organisations. Understanding the Barriers to Effective Bone Health Management in People With Multiple Sclerosis.

BACKGROUND: The prevalence of osteoporosis is higher in people with multiple sclerosis (MS) compared with the general population. Poor bone health, including higher rates of osteopenia and osteoporosis than in the general population, has been established in people with MS. This qualitative study examines how much a group of health care professionals (HCPs) knows about osteoporosis as an MS comorbidity, as well as current practices, barriers to effective bone health management in people with MS, and recommendations for improvement. METHODS: Thirty-two Australian HCPs participated in qualitativ
