Pityriasis rosea during etanercept therapy.

Multiple bioengineered agents has been recently developed to attack other parts of the immune/inflammatory system, achieving therapeutic implications for dermatologic diseases. On the other side, each new therapy has the potential for adverse cutaneous reactions. We present herein the first two cases of Gibert's pityriasis rosea occurring during etanercept therapy and discuss possible links between this common, etiologically still unexplained exanthema and anti-tumor necrosis factor-alpha drugs.

Erythrodysesthesia: An Unusual Complication With Trastuzumab Monotherapy.

Palmar-plantar erythrodysesthesia (PPE), more commonly known as hand-foot syndrome, is a dermatologic complication following chemotherapy with selective agents. In this article, we present the case of a young lady with stage III breast cancer who developed palmar-plantar erythrodysesthesia following treatment with Herceptin (trastuzumab), an unusual complication of this particular drug. From our review of literature, this is the second known occurrence of PPE secondary to trastuzumab monotherapy.

Cutaneous manifestations of viral outbreaks.

As the world tries to grapple with the COVID-19 pandemic, dermatologists are left in a lurch as there is a lacuna in dermatologic literature as well as training regarding the cutaneous manifestations of varied viral agents capable of causing epidemics/pandemics or the potential to be bio-weaponised. Such outbreaks have the potential to become a pandemic given this age of globalisation. The quote by George Santayana stands true 'Those who cannot remember the past are condemned to repeat it'. Thus, this article lends a perspective to the recent viral outbreaks and is aimed at summarising these a

State-of-the-Art Review: Vaccination in Pediatric Dermatology Patients Receiving Immunosuppressive or Immunomodulatory Therapy: A Review.

Immunomodulatory agents have become a mainstay treatment for various dermatologic conditions within the pediatric population. Increased use of off-label and FDA-approved medications, especially at younger ages, decreases disease burden and mortality. The earlier usage of these immunosuppressants often parallels the pediatric vaccine schedule. Successful treatment of dermatologic conditions while enabling children to mount an adequate immunologic response to preventable diseases necessitates partnering between dermatologists and primary care physicians. Understanding the guidelines will enable

Under Development JAK Inhibitors for Dermatologic Diseases.

Molecular targeting therapies represent a new exciting era in dermatology. A promising novel drug class, subject of intense research, is Janus kinase (JAK) inhibitors. Multiple cytokine receptors signal through the Janus kinase and signal transducer and activator of transcription (STAT) pathway. The pathway plays a central role in innate and adaptive immunity, and haematopoiesis. The understanding of the contribution of JAKs to the immunologic processes of inflammatory diseases led to the development of JAK inhibitors, initially for rheumatologic and hematologic diseases. Soon, their efficacy

Modulation of mu-opioid pathway for the treatment of pruritus: a systematic review of 1,146 patients.

BACKGROUND: Pruritus is a debilitating symptom and conventional therapies often provide incomplete relief. The μ-opioid receptor (MOR) pathway is a key regulator of itch, and μ-pathway-modulating agents are increasingly used off-label for refractory pruritus. AIM: This systematic review evaluates the efficacy, safety, and real-world use of μ-opioid pathway modulators for pruritus of dermatologic and systemic origin. METHODS: Following PRISMA guidelines (PROSPERO CRD420251207101), a comprehensive search of four databases identified 3,274 records, of which 69 studies met inclusion criteria, co

Perioperative use of antibiotics: preventing and treating perioperative infections.

Prevention of postoperative wound infection in dermatologic surgery and appropriate use of antibiotics to prevent endocarditis and joint-replacement infections are controversial issues. Dermatologists often may misunderstand the use of antibiotics to prevent endocarditis, surgical site infections, and prosthesis infections. In order to prevent endocarditis associated with surgical procedures, the American Heart Association (AHA) has developed clinical practice guidelines that apply to surgical patients with prosthetic cardiac valves, previous bacterial endocarditis, mitral valve prolapse with

Psychoneuroimmunologic aspects of skin diseases.

As mental and psychological issues are important in the development of many dermatologic diseases, these factors are of special interest in research. Psychoneuroimmunology is the study of interaction between psychological processes and the nervous and immune systems of the human body, and it was comprehensively described for the first time about 30 years ago. Communication between the mind and the skin involves the psycho-immuno-endocrine-cutaneous system, encompassing the activities of the brain, the immune system and the skin, with participation of different neuropeptides, interleukins, and

Morphea Induced by Golimumab in a Patient With Ankylosing Spondylitis: A Case Report and Literature Review.

Morphea is a localized autoimmune fibrosing disorder that may paradoxically develop during anti-TNFα agents in ankylosing spondylitis (AS). We report a second case of morphea associated with long-term golimumab use in a patient with AS, presenting with indurated plaques and confirmed histopathologically. This case highlights the importance of clinical vigilance and timely dermatologic assessment to determine whether morphea represents a drug-induced reaction or an extra-articular manifestation of AS.

Recurrent panniculitis in a man with asthma receiving treatment with leukotriene-modifying agents.

Leukotriene-modifying drugs are novel agents introduced recently to treat asthma. Both 5-lipoxygenase inhibitors, such as zileuton, and leukotriene receptor antagonists, such as zafirlukast and montelukast, have proved effective in the treatment of asthma. To our knowledge, there have been no detailed reports regarding dermatologic manifestations of this class of drugs. This article describes an unusual case of erythema nodosum in a 46-year-old asthmatic man who received 2 different leukotriene modifiers.

Tumor necrosis factor alpha inhibitors for the treatment of dermatologic diseases.

Tumor necrosis factor alpha (TNFalpha) is involved in cell differentiation, mitogenesis, cytotoxic responses, inflammation, immunomodulation, and wound healing. Because of its numerous roles, it was thought that inhibition of TNF may aid in the treatment of certain dermatologic diseases such as psoriasis, hidradentitis suppurativa, pyoderma gangrenosum, Behcet 's syndrome, and graft versus host disease. The efficacy of these agents has proven impressive and short-term side effects have been few and relatively benign.

Cannabis and Dermatological Implications: A Traditional Review of Adverse Cutaneous Reactions and Systemic Risks.

The growing use of cannabis for both recreational and medicinal purposes has raised concerns about its cutaneous adverse effects. While systemic implications of cannabis are widely reported, dermatological manifestations remain underrecognized and underreported. This review aims to consolidate existing literature on cannabis-related dermatological complications, including allergic contact dermatitis, urticaria, Stevens-Johnson syndrome (SJS), and cannabis-induced arteritis. Emphasis is placed on mechanisms involving the endocannabinoid system in the skin, case reports of dermatological reactio

Outpatient dermatology consultations for oncology patients with acute dermatologic adverse events impact anticancer therapy interruption: a retrospective study.

BACKGROUND: Dermatologic adverse events (dAEs) of anticancer therapies may negatively impact dosing and quality of life. While therapy interruption patterns due to dAEs have been studied in hospitalized cancer patients, similar outcomes in outpatient oncodermatology are lacking. OBJECTIVES: To analyse the therapy interruption patterns, clinico-histopathologic characteristics and management outcomes of outpatient dermatology consultations for acute dAEs attributed to the most frequently interrupted class of oncologic agents. METHODS: We performed a retrospective cohort study of all cancer pat

Interleukin-2-induced dermatotoxicity resembling toxic epidermal necrolysis.

Interleukin-2 is a promising new immunotherapeutic antineoplastic agent, but it can cause severe multiorgan toxicity. Although dermatologic toxicity is seen in most patients receiving IL-2 therapy, it is usually manifested as pruritus and an erythematous macular rash, which resolve quickly once infusion is terminated. We have described a patient who was allergic to multiple drugs and who had sloughing of large sheets of epidermis over 75% of body surface area during IL-2 therapy. Clinically, this was indistinguishable from toxic epidermal necrolysis, but the findings on skin biopsy were nonspe

Glucagon-like peptide-1 receptor agonists and related incretin therapies in dermatology: Patient selection and shared decision-making.

Glucagon-like peptide-1 receptor agonists and related incretin therapies have moved rapidly from second-line diabetes agents to widely prescribed cardiometabolic medications, and dermatologists increasingly encounter patients who are already taking them. Obesity is common among dermatology patients and is particularly prevalent in psoriasis and hidradenitis suppurativa, yet most eligible patients are not receiving glucagon-like peptide-1 receptor agonist therapy, and nearly all prescriptions originate outside dermatology. This positions dermatologists at the intersection of two distinct clinic

The use of antidepressant drugs in dermatology.

This paper provides an updated review of the use of antidepressant drugs in dermatology. Some of the psychiatric disorders that are usually comorbid with dermatological disorders and respond to antidepressants include major depressive disorder, obsessive compulsive disorder, body dysmorphic disorder, social phobia and post-traumatic stress disorder usually secondary to trauma and abuse during early life. Cutaneous symptoms may be the feature of a primary psychiatric disorder, e.g. cutaneous body image problems, dermatitis artefacta, neurotic excoriations and trichotillomania, or psychiatric sy
